Disk utility failure - cannot start OS-X

Conclusions
It appears that the HDD needs replacing.
I'm concerned the motherboard may also be faulty? How can I check (without purchasing another disk drive first)?
It seems strange (to me) that all information relating to the disk can be accessed, & all of the folders can be viewed using disk utility.
Yet the disk cannot be reformatted, or recovered from Time Machine back-up
Nor can the operating system be re-installed
Since I have Time Machine I presume I'll be able to restore the full profile to a new disk drive?
If I need to purchase a new disk (& the motherboard is ok), any recommendations which disk to buy?
Any suggestions why this disk failed? Is it related to 'sleep' and applications not shutting down properly.
Any advice most welcome.

  • Applications not responding leads to Disk Utility failure

    Hi everyone.
    I have a mac mini running 10.4.8 and recently it started hanging up on applications (particularly firefox). I'd get "application not responding" messages and even when I wasn't doing anything at all to my machine, it would stop responding.
    I tried to boot up off the install CD and run disk utility but got "The underlying task reported failure on exit"
    Unable to repair
    Unable to verify
    Additionally, I cannot mount my volume. Nor can I get any options to reinstall/reformat as from the Installer my hard drive does not show up as an available volume to install on.
    Also, when I try to startup, if I pick something other than the CD, it just flashes the grey apple screen for about 5-10 minutes and then shuts off.
    It did, however, pass hardware test.
    The error I get in First Aid before repair stops is "Invalid Node Structure"
    But...I'm not sure where to go from here.
    I haven't installed anything new and it was totally fine previously.

    You have serious damage to the disk and will need to either buy Disk Warrior and see if it can repair the damage, or zero the drive and reinstall. If you have a recent clone the reinstall would be relatively painless. If you don't have your drive backed up to an external drive you now know why you should have done this. If you are lucky Disk Warrior will be able to repair the damage. I suggest that you immediately buy a firewire drive and install Tiger on it and then boot from the external and see if it is possible to recover your files that haven't been backed up from the internal drive. Do this even if Disk Warrior succeeds. Drives fail. The older the drive, the more likely it is to fail. Rule of thumb: don't fully trust any drive that is 5 years old or older. And always have backups. Always.

  • Hard Drive and Disk Utility Failure on Macbook Pro

    Ok, so I've had this Macbook Pro since Sept. 2006 and I upgraded the operation system to Leopard about a year and a half ago. I've had no problems until now.
    A few days ago I did a software update for the OS, and didn't seem to have any issues. The next day when I booted up the computer, it was very slow in starting up. When it did start up, it only displayed the Macintosh HD and not my files, but a few minutes later everything else loaded.
    When I ran disk utility it just said that the hard drive needed to be repaired. The error was talking about a file on the server that was supposed to be numbered 1002 and instead was labeled 501 (?)
    Now my issues is, I can't find the Leopard disc, (i moved and I think I lost it?) and all I have is the original Tiger OS disc. I tried to reinstall the HD after backing up the files I needed onto my external. The Tiger OS disc will not work, it says there isn't enough space to be installed onto the computer. Also now whenever I turn on the computer the screen is grey with the apple logo, goes through the blue screen launch and just pops up the window that says "Mac OS can't be installed on your HD"
    So right now I am frustrated. If anyone has any advice, please let me have it. Am I completely out of luck or could Apple help me? I no longer have applecare btw, it expired this past October.

    Your system info confirms you have Snow Leopard (10.6.2) installed.
    Leopard (10.5) & Snow Leopard (10.6) are 2 separate OS systems.
    You need your Snow Leopard DVD to repair the damage if that is the OS that is currently installed on your MBP.

    If SL is the DVD you cannot locate then you will need borrow a retail SL DVD & run the +"repair disc"+ setting or purchase one of the following to repair:
    DiskWarrior
    Highly recommended for *directory damage* repairs.

    Multi-functional utility.
    Drive Genius Multi-functional utility.
    You will need to make your own decision on which to purchase. Read up on them on their websites because each does something a little different.
    You must use the versions that are compatible w/your OS system & keep the utilities updated to avoid damaging/harming/trashing your system.

  • MacBook won't boot after trying to repair with Disk Utility. Cannot boot in Safe Mode. Just powers down after about 5 minutes. Apple Hardware Test says error 4SNS/1/40000001:IG0C-0.265 But I cannot find anything on the net about it. MacBook 4,1 Intel.

    Hello Everyone,
    I am in dire need of assistance I am hoping someone could provide. I have a MacBook 4,1 with Intel Core 2 Duo processor 4GB Ram. It is taking over 10 minutes to boot up past the gray apple screen when I turn it on. First I cleared the PRam (?) then I checked the disk using Disk Utility Verify, and it said the disk needed repairs. So I ran the disk repair and it said it was unable to fix the errors on the HD.
    I did a little research and tried booting into the Single User mode, then running /sbin/fsck -fy. It said that it found errors but could not fix them. I ran it two more times as suggested and got the same result each time. I then tried rebooting and now instead of taking 10 minutes to boot up, it takes about 5 minutes on the gray apple screen and then just turns off. I started in Verbose mode to try to troubleshoot the error and it appears the last thing to come across the screen before power down is a message "Apple Yukon 2: RxRingSize <= 1024....etc".
    I decided next I would run the Apple Hardware Tester. The test came back with an error code "4SNS/1/40000001:IG0C-0.265". I am very good at searching the web but I could not find any errors that had the IG0C or IGOC or any combination at the end, but plenty of 4SNS/1/40000000(1) errors with different endings. From what I can tell people are saying anything with 4SNS/1/4000000 is a logic board failure, but this computer was literally just booting this morning until I did the /sbin/fsck -fy.
    Any suggestions are welcome and appreciated.

    You've a couple of issues .... when Disk Utility tells you it can't repair a drive, if the error is a soft error, like an invalid tree or node, Disk Warrior (or TechTool Pro) can handle it. However, if Disk Warrior (or TechTool Pro) can't handle it, you can erase and write zero's to the hard drive and restore to it, or you can buy a new hard drive. Sometimes running SMART Utility can provide additional insight into the health of the hard drive. 
    THe 4SNS error is a sensor error.  You could install iStat Pro to see if something is indeed getting too hot or remaining room temp or less.  You may need to take it into an Apple shop so they can use their thermal diagnostics to test and isolate it further.

  • Disk Utility won't start up in 10.5.3

    I have been unable to open up my Disk Utility at all. I was able to do so earlier, perhaps with 10.5.0 while I was mounting my external hard drive. Now, I cannot mount the hard drive, and therefore cannot use Time Machine all because DIsk Utility won't open. I've diagnosed my hardware with "Disk 1" and found no problem. And, I've reinstalled the operating system. Any ideas?

    I just had the same issue on my son's MacBook. As long as you have a wired keyboard, the following will work.
    1. Shut down the computer if it is on.
    2. Press the power button to start the computer.
    3. Immediately press and hold the Command (Apple) key and the "s" key for single-user mode.
    4. Type mount -uw / (then press the Return Key) wait for next line to appear with # root/
    (there is a space between t and - and another space between w and /)
    5. Then Type "passwd" (without quotation marks then press the Return Key)
    6. Enter new password (this will be for the root user account) (watch your fingers when you type because it doesn't show what you are typing, then hit the Return Key after you type it.
    It will prompt you to retype the new password again, so retype it again and then hit the Return Key) wait for next line to appear...
    7. Then Type "reboot" (without the quotes and hit the Return Key)
    after the computer restarts and comes up with the login window....
    8. Login with any of your usernames just to get the desktop up.
    9. After desktop comes up choose System Preferences, then choose accounts. Unlock if you have to and using the username: root ..... and the password you just setup in the single user mode You now should be able to make users including your existing user account an admin.
    After you log back in under your real account, it is advisable to deactivate the root user account. To do this,
    1. Click the Finder icon in the Dock.
    2. From the Go menu, choose Utilities.
    3. Open Directory Utility.
    4. Click the lock in the Directory Utility window if you need to unlock the utility.
    5. Enter an administrator account name and password, then click OK.
    6. Choose Disable Root User from the Edit menu.

  • Disk Utility stalls from start up DVD disk?

    Has anyone experienced this or can confirm this problem:
    On my Mac Pro 2.66 with single Superdrive (Optiarc AD-7170A), I can start up from the Tiger OX Install disk one, no prblem. But I cannot seem to sucessfully run Disk Utility from the DVD. When I Launch Disk Utility from the DVD, the application opens and I get an eternal beachball. The DVD drive whirs up a short bit, then goes quiet. The only way out is to powerdown with the front power button. I only have one hard drive -- the stock 250GB WD2500AAJS
    Disk Utility seems to launch fine from the hard drive itself -- although "Verify Disk" does take about a minute to process when inspecting my hard drive. Could it be it's just taking a REALLY long time when running from the startup DVD?
    Please note what DVD drive you have if things work fine for you

    I presume you are using the install DVD that came with the MacPro? If not, you need to.
    Make sure there is no non-Apple hardware in the system. Added RAM, hard drives from another source, etc. should all be removed, and the try again.
    If still having the same issue, hen there is a hardware issue somewhere. Either the install DVD has a problem with it, or the computer itself has a problem. If you have the disk from another MacPro, try using it. If you have the same result, then the problem is with the computer and it will need to be repaired.

  • Disk Utility Failure?

    Hi all!
    Before I start, let me first say that I did search for my problem here before posting, however the results I found were not exactly under the same circumstances as my own.
    The long and short of it is this: I was running "Disk Utility" from my installation CD (I started up from the CD). While running "Disk Verify" and "Disk Repair," I received the following message just as the progress bar was complete:
    "Disk could not be repaired because Disk Utility failed on exit."
    According to the utility, it claimed my hard drive only needed "minor" repairs, and obviously my computer is still very usable (lol), but this still freaked me out a little bit.
    Is there something I can (or should) be doing to rectify this? My computer isn't horridly slow or buggy, so perhaps I should go on about my business--unless there is a fairly straightforward way of getting everything back to normal.

  • Disk Utility doesn't start

    I had this wired problem. I was just hangin' around when this funny coloured, round, spinning thing came. I just waited but nothing happend for 10 minutes. So i pushed the power button and restarted my Mac.
    I tried to start the Disk utility... I tried. There is a log from the Crash Reporter which say that a library is missing. While i tried to write tho the discussions forum, Safari crashed, with a nearly the same message:

    Good Morning,
    It's me again. The day before yesterday I called the apple support. My iMac has some really strange behaviours, for example disk utility from the install DVD hung up while trying to access the local disk. Further did the TechTool CD from the carepack not boot, and and and.
    I deleted my installation (overwriting with zeros) and installed OS X again, which went fine. Then, after setting up, I made softwareupdates and on the last update, the one for OS X 10.4.9, my iMac hung up itself again. Yeah, pushing the button again, calling the Apple support again.
    So, yesterday I set up my new iMac for the 4th or 5th time, very nice. This time the softwareupdates worked fine and I'll see if it was "just" a Sofware failure, or multiple Software failures. If my Mac is going to hang up the nix time, I'll bring it to my service provider, no call's to the support anymore.

  • Install errors and Disk Utility failure

    Hello!
    My current install of Mac OS X must have been a newer install of Panther. The retail install I bought is v10.3.5. I think the install was originally 10.3.7.
    I've tried to run Disk Utility from the install Disc 1 and get a message: "*The underlying task reported failure on exit.*"
    And I tried to also perform an Archive and install of Mac OS X Panther 10.3.5 and get a message: "*You cannot install Mac OS X on this volume without changing your install settings. A newer version of Mac OS X is installed*."
    So, I'm asking what my options are as to keep my current user Home folder that I mainly use and delete the Administrator Account to get a fresh install that I can maintain properly without problems like I just quoted.
    This is for my DA 533 MHz.

    How to Perform an Archive and Install
    An Archive and Install will NOT erase your hard drive, but you must have sufficient free space for a second OS X installation which could be from 3-9 GBs depending upon the version of OS X and selected installation options. The free space requirement is over and above normal free space requirements which should be at least 6-10 GBs. Read all the linked references carefully before proceeding.
    1. Be sure to use Disk Utility first to repair the disk before performing the Archive and Install.
    Repairing the Hard Drive and Permissions
    Boot from your OS X Installer disc. After the installer loads select your language and click on the Continue button. When the menu bar appears select Disk Utility from the Installer menu (Utilities menu for Tiger.) After DU loads select your hard drive entry (mfgr.'s ID and drive size) from the the left side list. In the DU status area you will see an entry for the S.M.A.R.T. status of the hard drive. If it does not say "Verified" then the hard drive is failing or failed. (SMART status is not reported on external Firewire or USB drives.) If the drive is "Verified" then select your OS X volume from the list on the left (sub-entry below the drive entry,) click on the First Aid tab, then click on the Repair Disk button. If DU reports any errors that have been fixed, then re-run Repair Disk until no errors are reported. If no errors are reported, then quit DU and return to the installer.
    2. Do not proceed with an Archive and Install if DU reports errors it cannot fix. In that case use Disk Warrior and/or TechTool Pro to repair the hard drive. If neither can repair the drive, then you will have to erase the drive and reinstall from scratch.
    3. Boot from your OS X Installer disc. After the installer loads select your language and click on the Continue button. When you reach the screen to select a destination drive click once on the destination drive then click on the Option button. Select the Archive and Install option. You have an option to preserve users and network preferences. Only select this option if you are sure you have no corrupted files in your user accounts. Otherwise leave this option unchecked. Click on the OK button and continue with the OS X Installation.
    4. Upon completion of the Archive and Install you will have a Previous System Folder in the root directory. You should retain the PSF until you are sure you do not need to manually transfer any items from the PSF to your newly installed system.
    5. After moving any items you want to keep from the PSF you should delete it. You can back it up if you prefer, but you must delete it from the hard drive.
    6. You can now download a Combo Updater directly from Apple's download site to update your new system to the desired version as well as install any security or other updates. You can also do this using Software Update.

  • How do I fix a volume if  Disk utility says cannot be repaired?

    Am having some annoying issues with my PowerMac G5 hard drive, nothing very specific, just lots of small issues like crashes and hangs and the fan going into hyper-drive every now and then. Started up from my system disc/Disk Utility and ran repair permissions and repair disc.
    On repair disk it gets to the end then says:
    1 HFS volume repaired
    1 volume could not be repaired
    I cannot find out what this means so can anyone advise?
    I also run repair permissions over and over and it never stops finding something to repair.
    Any advice much appreciated.

    Do you mean the System install DVD? - that's what you should do as you cannot repair an HD you're booted from.
    On repair disk it gets to the end then says:

    This is a little odd: is there another hard disk connected? Or possibly it was trying to repair the install disk as well?
    I also run repair permissions over and over and it never stops finding something to repair.
    Repairing permissions does thrown up a lot of things which can't be repaired: this is a bug arising from a system update and is harmless: you can ignore it.
    You could try downloading Applejack which you run in single-user mode and provides an easy way of using the built-in disk repair procedure: this is quicker than starting from the DVD and you won't get any confusion with the DVD. It's quite safe: it doesn't do the check itself, just invokes the repair that you would get by typing arcane Linux codes. But note that you must run it only in single user mode (starting up and holding command-s while booting), never while booted normally.

    Hello,
    I get the following error when "Repair Disk" fails:
    "The underlying task reported failure on exit (-9972)"
    Does anyone know what this DU error means?
    I went through several pages of technical articles and none mentioned this particular error message. I get this error by running DU from my Panther DVD and strangely enough, when the same thing is done using my Tiger DVD, I do not get the (-9972) part of the message. However, the bottom line was the same for both DVD's: the HD could not be repaired.
    At the moment, I am experiencing no problems with my PB, however I could be enjoying the 'calm before the storm'. I just recently had a serious sys crash, so this could be a lingering symptom of those problems.
    This may or may not be serious, but I would like to know what this means before I proceed with the installation of Tiger. I can Repair Permissions using either DVD, but the fact that I cannot successfully perform a 'Repair Disk' is troubling.
    All comments or suggestions are most welcome.

    I want to pass this on to you because you were very kind to help me out and I can add some good news to those who may have this same problem in the future.
    My progress will serve as an update in regards the following Disk Utility error and Mr. Kawashima's very descriptive and informative post as provided above.
    In his post of Oct 4, 2005, Mr. Kawashima stated that DISKWARRIOR could not repair problems such as
    "Error: The underlying task reported failure on exit (-9972)"
    I received the identical error message using Disk Utility and could not make the repair using either TechTool Pro or DISKWARIOR. I had a two-year old version of DW but went to Alsoft's website and downloaded the latest update. I had emailed Alsoft about my problem a few days ago and was told to download the update and try to make the repair again.
    Once the download was completed, I discovered that they provide a very simple way of creating a fresh DW CD which contains the latest updated code. Bottom line: I booted this updated DW CD and it repaired my HD. I no longer get the above stated error.

  • Disk Utility failure to erase/initialize

    I have a 1 TB WD MyBook Studio Edition. I sent this drive to a firm that does video conversions (from film to digital). Drive returned with expected (good) results, but now I can't write anything onto the drive - says I can only read. So I copied all files on the drive to another disk, then used Disk Utility to initialize. First Aid tab action buttons are all greyed out. Erase tab shows default format as MS DOS. I changed to Apple Extended (Journaled) and pressed OK. Process starts, but fails after a few seconds with error "Write Failure". Now disk won't mount when I attach it - only Disk Utility "sees" drive, but fails each time I try to Initialize/Erase.
    What's up, and how do I correct?

    If you haven't fixed this yet, you may be able to use Disk Utility to completely erase and reformat the disk.
    Click the top line of the drive, then the Partition tab. Click +1 partition+ (or however many you want) under *Volume Scheme*, then Options at the bottom. Select GUID for an Intel Mac, +Apple Partition Map+ for a PPC Mac. Setup the partition(s) you want to use on the Mac as +OSX Extended (Journaled),+ then click Apply.

  • Disk Utility will not start

    Hello I need some help here.
    I am trying to format a new Western Digital 2tb my book essential. To do this I apparently need to open Disk Utility... but it will not open.
    the icon flashes for a second but then closes immediately. I have 15% of my harddrive free (hence getting the new hard drive to free up space).
    If I am told to use a start up disk or something... I have find the box and the disc etc... but if I use this will I lose everything on my mac book?
    I can't lose what I have on here, I had things time machine backed up on a drive which was damaged in my recent move... so losing things is not an option.

    Don't confuse partitioning to create a volume on a drive with partitioning to create multiple volumes on a drive. The first is always necessary. The latter is something some people with Windows backgrounds are used to doing.     The short answer is that you should be formatting with journaling.
    how much space should i partition for time machine... if i should be doing that...
    Time Machine is one of the cases where partitioning is useful. By design Time Machine will consume all the space on its output volume. If you plan to put Time Machine data and other data on the same drive, you should partition the drive to keep those types of data separate. As for size, see item 1 of this post:

  • Why can't I select Disk Utility when I start in recovery mode?

    Disk utility varification of HD detects requirement for repair and directs recovery mode startup.  When I startup in recovery mode I cannot select Disk Utility or any of the other options.  Likewise, I cannot select File, Edit, or any other options across the top of the screen?  Any ideas?

    The menu comes up, but I can't select any of the options or the continue button.  I can move the arrow by moving my finger on the touch pad, but nothing is touch sensitive. 
    The menu box has a blue line around the inside of it when it comes up, as though it has been selected.  I'm not at means anything.
