Failed Hardware Check 4MEM/1/40000000, programs crashing erratically

I have been having many troubles with my MacBookPro 1.83GHz Dual Core Intel. Recently many programs just crashed without apparent reasons, especially MS Office ones, and many others I did not even know they were there (programs called directly by the system, or by other programs). I tried to reinstall the system, but the second DVD would never complete installation. I installed then only the basic system, excluding everything on the DVD 2, that's how I am using it now (I made a fresh installation reformatting the disk, after backing up my data). Then I ran the Hardware Test and I got the message indicated in the subject, I guess that it means there's a problem with the second memory extension (I have two 512)? At the moment the version of the system running is 10.4.5, as I did not update to 10.4.9 (as I connect via 56k modem, it will take a long time). Should I just change the second memory, or there could be something else going on?

Hello, thanks for the info, I did get memtest, which however considers all RAM to be ok. I checked both RAMs with the Hardware test again, and took out the one which did not work. I am just wondering now, if it is a problem of the RAM bank or rather of the connections (slot), I will check that later. In any case, with only one memory it is working fine so far (even though slower). What do you think it would be better, to get a new 512 RAM, or rather a 1GB, in substitution? As I read somewhere that RAMs of the same size make performance better.

I have also run a hardware test after the disk verify didn't work from the install cd.

now I thought that sorted it. but after doing the updates. and then going to repair the disk permissions/repair the disk it came back with same error. and also the hardware test is still failing.
The system will never be stable until the HD directory is repaired.
- is the version of the hardware tester I have not compatible with the OS or the firmware i have upgraded to? or does the tester just check the integrity of the hardware regardless of firmware & os vers.
If this is the AHT disc that shipped with your MBP, it is fine, regardless of what OS you have installed.

I believe that's a waste of your time. As has already been stated, at least one of the issues is the damaged HD directory. Until/unless that is repaired, what you install on the HD isn't going to work correctly, if at all. The big question is, is some other part of the hardware causing the directory damage, or did the directory take a freak "hit" and repairing it will resolve things? It's quite possible that bad RAM or the logic board could have caused the directory damage. You're either going to have to bring it in to an Apple Store "genius" bar and see how much they'll test for free, or do some things yourself. For things you can do yourself, apart from trying one RAM module at a time, and running Repair Disk repeatedly (I don't think that's going to do it), you're going to need to either get your hands on Disk Warrior (or comparable utility), or a different HD, or try the HD in an external enclosure, or get your hands on some different RAM to try.

hello.
I am having issues with my 15inch retina mac 2012. Some apps for example safari crash every once in a while. Some times i get the grey screen of death.
My configuration is: CTO. 2.6 GHz Intel core i7 with 16 GB 1600 MHz DDR3 and 512GB flash memory.
I have recently replaced my screen as it was with burn in problems.
I tried to install Yosemite but failed several times. Just got the 10.9.5 Mavericks going again.
I ran the apple hardware test and got FAIL!
The error code read as follows: 4MEM/9/40000000: 0x79093f90
What do i do now? I am out of warranty and i can't upgrade to the new software.
I keep on having app crashes. I keep on sending reports to apple and there is no response.
Who should i hold responsible for hiding that there was a problem with my machine?
My warranty recently expired but i had the problems prior and i figured that if there was no response that maybe one day better software would fix app crashing.
Please suggest a course of action other than replacing main board with ram and cpu (too expensive).

Macbook Pro shutting down. Hardware error code 4MEM/62/40000000:0x7f537f90

Macbook Pro shutting down. Not the same action making it shutdown. Anywhere from being online to attaching a photo to an email to working in Word or Photoshop. Even after it's closed and not being used. I come back to it being shut off and saying it had to restart because of a problem. I ran the Hardware check and got the Hardware error code 4MEM/62/40000000:0x7f537f90.
I must say I am quite disappointed. I have used Mac for work but had a PC since day one, and I JUST got this Macbook Pro in Dec. I was delighted that Mac usually didn't have as many problems with viruses etc as PC. And now last couple days, constantly shutting itself down. The closest store to me is a few hrs away. I am in South Dakota, store is in Minnesota Any help would be appreciated.

Can you post a screen shot of About This Mac -> (More Info) -> System Report -> Memory and click on each DIMM and post the bottom right pane. You can also do the same without a screen shot using just text version using the following highlighted command in OSX Terminal. The output is also listed.

Program crashes - hardware or software problem?

I just purchased a 15" MacBook Pro. I am a Windows-switcher and just getting used to the OS, as this is my first Apple computer since my old Apple IIGS.
I am trying to get the piano emulator, Pianoteq, to work. According to the system requirements, the MacBook Pro should excel. However, when I use it, the program crashes at random times ranging from a few seconds to a few minutes of use. My question is, does anyone know, basically, what the problem is? Is there a problem with my hardware that I need to address? Maybe something I should fix while under warranty. Or there a problem with Pianoteq's software working with Leopard?
I have a Yamaha digital piano and a Midisport Uno MIDI-to-USB cable. As this is a new computer, I do not have any 3rd party utilities installed.

Well, I checked into the driver issue and thought I found the answer. The M-Audio website showed that there is no Leopard driver for my MIDI-USB cable. I discovered a generic driver at: namedfork.net/midisportloader and installed it.
My piano keyboard worked with Pianoteq for the rest of the night, I thought the problem was history. However, this morning it only worked for a couple of minutes and then the same problem. Re-installing the generic driver didn't help this time.
I believe this probably is an issue with the M-Audio MinisportUNO MIDI-USB. Hopefully they will have a Leopard driver soon.

What does Apple Hardware Test error code 4MEM/1/40000000:OX3bb6b190 mean?

Mozilla kept crashing so I did the Apple Hardware Test and got error code 4MEM/1/40000000:OX3bb6b190. I reinstalled MacOs 10.4.4 (after saving the important stuff to disk of course) and it took about 10 tries to get the 2 disks installed. After the installation, I tried to update to 10.4.5, and when I restarted, the screen was blank. The only disk the computer will start with now is the No. 1 installation disk. What can I do?

A MEM error code generally indicates a faulty RAM Module. In my experience most RAM manufacturers offer at least a 5 year warranty of their RAM products. Some warranties are for 10 years. Sometimes reseating (taking it out and putting it back in again - properly) the Module will 'cure' the problem. Run AHT afterwards if you try this option first.
If you can remove the offending module and still have enough RAM to be able to do what you need to do then do so.
If not take it to your nearest Apple Repair Specialist who should fix it for you or replace the faulty module yourself and deal with the RAM manufacturer directly.
Judging by the footer to your post you don't seem to have had much luck with this model have you?

2007 MBP freezes, Ran hardware test got error code 4MEM/11/40000000:505c99b0 Help?

Hi all, my 2007 MBP freezes and requires me to hold power button to restart. It's happening somewhat frequently. ( installed new SSD and new battery in the last 2 months)
I ran the quick hardware test ( am running the extended one now) and recieve the following error code 4MEM/11/40000000:505c99b0
Hoped to squeeze another year or two out of this old warrior. Any ideas what this code might be?

This is a memory error of some sort. We don't have the Rosetta stone for these errors, so I don't know what the rest means.

Ordered RAM on Amazon.de with the right Specs: 204 pin DDR3 SO-DIMM (1333 MHz, PC3-10600S, CL9),
Manufacturer: CSX (http://www.csx-memory.com , while it's maybe not the most renown seller, it was described on another website as reliable )

Crashes:
Didn't really have any, except..... well,
- In some instances Firefox (I think 32.02. or sth., the latest Version at this moment) reproduceably keeps crashing, for example right now, can't even start it, that's why I can't tell you the exact version, maybe I edit that later. Safari so far hasn't crashed.
- Once I had a System Crash, but it happened when I wanted to reboot after updating the firmware of a hooked on external WD drive to be usable again as time machine backup volume under mavericks (without endangering my data).
The RAM seems to work fine; for example when I was doing a long overdue backup of 77GB on said drive, Activity Monitor showed a RAM Usage of 15,99 GB and nothing crashed. Only the fans were powering at some point but i thought that normal..
- Actually produced two more Crashes when trying to run Apple Hardware Test (AHT), the one on the second disc that originally came with my Computer (probably made for OS 10.6.x    x= 6 or sth., don't remember which version my MBP was shipped with), which leads to
Tests:
Memtest86
- I put Memtest86 v.5.01 on a USB Stick and bootet from that and let it test "the whole" RAM
(btw Do you know how you can put the bootable USB image of Memtest only to one (of a few) partition of my 32 GB USB Stick? The instructions (command line stuff) only let me put it on the whole Stick which makes it otherwise useless as a volume; ok here I'm not sure if I'm using the correct technical terms....)
- I let Memtest86 do 3 and a half passes with one pass consisting of 10 different tests, so it took over 8 hours!
--> RESULT:   0   (in words: zero) ERRORS,
--> That is why I think my RAM is ok, because I think (think NB, don't know) Memtest86 does good testing,or do you know      better?
AHT
- After that, out of curiosity, I tried to run AHT from the DVD with said results, then I ran AHT (also called Apple Diagnostics under Mavericks) from the Internet, this worked, but produced the famous 4MEM/9/40000000 Error Code,
first one for example was: 4MEM/9/40000000: 0x8477df98, the last part after the colon is variable, but stays the same in one instance, i.e. if you repeat the AHT directly. If you restart the Computer an rerun the test then it changes, e.g. some other it showed 0x84770a18
- I did the short and the extended version, it took ca. 2-3 and 16 min respectively till producing the Error Code (only 1 at a time)
- Did it 4 times or so; once in short/simple version it showed no error, the rest the 4MEM/9 s.

What other than bad RAM can 4MEM/9/40000000 mean? I hope there are a few tech and computer savvy people around this place who know what they are talking about and can give me a good answer, or at least help to narrow it down. Actually I also think, the RAM sticks are not loose, but before I screw open my MBP again I would like to explore the other possibilities.
Is it probable that this Error Code has got sth. to do with me installing a) 16 GB of b) 1600 MHz RAM, or maybe with CAS Latency (i don't know the actual CL of this RAM)? and if so, is it only because some Apple Programmer/Developer/Technician wrote some values in a list on the System Level, so it doesn't accept RAM it would normally work with?? If it works fine and is faster, why change it?
Final Words:
I repeat, apart from the Firefox crashes, my Mac seems to be working fine, at least I don't notice anything, (well, that disk utility and System Profiler (or whatever it's called nowadays) sometimes take time to start up is nothing special i think, is it?)
But now I'm scared that perhaps there could be a future danger for my computer I do not foresee...I hope you can at least exclude that.
So I'd rather spend my time starting a discussion here with people who don't have to abide by some corporate policy (right?) than having the hassle of repeatedly screwing my Mac open and shut and doing expensive calls with all the companies involved who won't tell me anything more in depth than "we have to send it there, do more tests and you get it back then".
I hope that this thread won't rot in the wastelands of unanswered posts and that maybe all the people who take part in it learn sth. and all the people seeking help in future will find it here.

Crashes:
Didn't really have any, except..... well,
- In some instances Firefox (I think 32.02. or sth., the latest Version at this moment) reproduceably keeps crashing, for example right now, can't even start it, that's why I can't tell you the exact version, maybe I edit that later. Safari so far hasn't crashed.
- Once I had a System Crash, but it happened when I wanted to reboot after updating the firmware of a hooked on external WD drive to be usable again as time machine backup volume under mavericks (without endangering my data).
The RAM seems to work fine; for example when I was doing a long overdue backup of 77GB on said drive, Activity Monitor showed a RAM Usage of 15,99 GB and nothing crashed. Only the fans were powering at some point but i thought that normal..
- Actually produced two more Crashes when trying to run Apple Hardware Test (AHT), the one on the second disc that originally came with my Computer (probably made for OS 10.6.x    x= 6 or sth., don't remember which version my MBP was shipped with), which leads to
Tests:
Memtest86
- I put Memtest86 v.5.01 on a USB Stick and bootet from that and let it test "the whole" RAM
(btw Do you know how you can put the bootable USB image of Memtest only to one (of a few) partition of my 32 GB USB Stick? The instructions (command line stuff) only let me put it on the whole Stick which makes it otherwise useless as a volume; ok here I'm not sure if I'm using the correct technical terms....)
- I let Memtest86 do 3 and a half passes with one pass consisting of 10 different tests, so it took over 8 hours!
--> RESULT:   0   (in words: zero) ERRORS,
--> That is why I think my RAM is ok, because I think (think NB, don't know) Memtest86 does good testing,or do you know      better?
AHT
- After that, out of curiosity, I tried to run AHT from the DVD with said results, then I ran AHT (also called Apple Diagnostics under Mavericks) from the Internet, this worked, but produced the famous 4MEM/9/40000000 Error Code,
first one for example was: 4MEM/9/40000000: 0x8477df98, the last part after the colon is variable, but stays the same in one instance, i.e. if you repeat the AHT directly. If you restart the Computer an rerun the test then it changes, e.g. some other it showed 0x84770a18
- I did the short and the extended version, it took ca. 2-3 and 16 min respectively till producing the Error Code (only 1 at a time)
- Did it 4 times or so; once in short/simple version it showed no error, the rest the 4MEM/9 s.

What other than bad RAM can 4MEM/9/40000000 mean? I hope there are a few tech and computer savvy people around this place who know what they are talking about and can give me a good answer, or at least help to narrow it down. Actually I also think, the RAM sticks are not loose, but before I screw open my MBP again I would like to explore the other possibilities.
Is it probable that this Error Code has got sth. to do with me installing a) 16 GB of b) 1600 MHz RAM, or maybe with CAS Latency (i don't know the actual CL of this RAM)? and if so, is it only because some Apple Programmer/Developer/Technician wrote some values in a list on the System Level, so it doesn't accept RAM it would normally work with?? If it works fine and is faster, why change it?
Final Words:
I repeat, apart from the Firefox crashes, my Mac seems to be working fine, at least I don't notice anything, (well, that disk utility and System Profiler (or whatever it's called nowadays) sometimes take time to start up is nothing special i think, is it?)
But now I'm scared that perhaps there could be a future danger for my computer I do not foresee...I hope you can at least exclude that.
So I'd rather spend my time starting a discussion here with people who don't have to abide by some corporate policy (right?) than having the hassle of repeatedly screwing my Mac open and shut and doing expensive calls with all the companies involved who won't tell me anything more in depth than "we have to send it there, do more tests and you get it back then".
I hope that this thread won't rot in the wastelands of unanswered posts and that maybe all the people who take part in it learn sth. and all the people seeking help in future will find it here.

RAM trouble (4MEM/9/40000000: 0xb865f398)

I have an iMac 21.5" (mid 2010). Off and on over the past year I have had random shut downs "because of an unexpected error". Recently it has gotten much worse. Firefox has crashed a lot, or any other app that is more intensive. I reformatted my iMac, but that didn't help. I ran the Apple Hardware Test, and it gave the error code:
4MEM/9/40000000: 0xb865f398
From what I know, this is related to the RAM. I have four 4GB Kingston Tech modules installed. I removed all but one, then ran the Hardware Test again. After going through all four modules, all the tests stated "No Trouble Found". Then I ran I module in each port to check if a port was bad, and still found no trouble. I've run two at a time and three at a time, but still "No Trouble Found". The only time I get the error message is when all four modules are installed.
Does anyone know what the problem could be? Even if I leave only three modules installed (which didn't give an error message when I ran the Hardware Test) I still have frequent crashes and freezes. I've got OS X Yosemite 10.10.3 installed.

Install the original RAM that shipped with the Mac and then test. If it is OK (I suspect it is) replace the Kingston RAM (which tends to be problematic in iMacs) with a higher quality RAM from either Crucial or OWC, purchase directly from either vendor. iMacs are sensitive to RAM and demand high quality RAM rather than value RAM.
Your issue is one of the following:
Failed RAM
Incorrect RAM
Unseated RAM

My computer is an Intel Imac that is about 6 months old.
Over the last several weeks my Intel Imac has been experiencing problems. It started with a few programs closing unexpectingly every other day. It climaxed the other day with programs crashes right after opening and finally kernel panics (the black screen that comes over the screen with all the different languages saying to restart the computer).
I ran the Hardware Test and got the error message:
4MEM/1/40000000: 0x7bb1a790
I can't even start my computer without it going to the black screen after 30 seconds.
After researching this it seems I have some sort of hardware problem that is pretty serious. I was wondering if anybody knows the specifics of this particular error message I recieved. Can factory RAM just malfuction out of nowhere?

It's a bad RAM error. You need to take the computer to an Apple service center to either repair the computer or replace defective RAM.
Any RAM can malfunction out of nowhere. However, the error may not be due to defective RAM. It could also be due to other hardware failure on the logic board such as the memory controller.

Program crashes regularly

I've got a group of programs that crash way too often to be considered coincidental. The group includes Firefox 2.0, SuperDuper, XCode 2.4.1, and iTune 7.0.2. All are the latest Universal versions and I've removed and reinstalled all of them multiple times.
My setup:
Mac Pro 2.66 GHz, 4 GB RAM (2 GB Apple, 2 GB Crucial certified to Mac Pro specifications). 3 500 GB harddrives (1 stock 500 GB Seagate, and 2 identical to stock Seagate 500 GB's). I use one of the Seagates as a destination for an image of the main drive. I use the 3rd Seagate as the repository for all my Parallels VMs (Win XP, Ubuntu 6.10, and Fedora Core 5).
I've run the TechTool Deluxe multiple times and find no evidence of any hardware problems. I've removed the Crucial Memory and the problem still occurs. I've swapped the Crucial for the stock Memory, and the problems still occur. I've done deep verifications of all the hard drives. I can't get a bead on the causes of these problems. I've sent EVERY crash report to Apple and I've tried to dissect what the programs were doing at the time of crash. Nothing jumps out. I can say that the problems occur more frequently when the machine comes "out of sleep" than at any other time, but I don't leave programs running while the machine is "sleeping".
The fact that these programs come from both Apple and 3rd Parties doesn't suggest a particular programming issue unless it is in the SDK for Tiger.
I is hard to seek help from Apple when the problems aren't consistent. It is hard to seek help from the program developers when their program isn't alone with the problems.
I'm running out of ideas short of completely reinstalling the OS and reinstalling all of the apps. At this point, that would be a really time consuming process as I have quite an array of 3rd party applications installed.
Any thoughts or suggestions on whether my problems are common or rare and whether there are other things I might check out before taking the big hammer. These crashes occur often enough to be really annoying, but not consistently enough that I can predict when they'll occur.
Help or ideas appreciated.

I don't know exactly what the problem is, but the crash reports in the log folder have been reviewed by the developers of SuperDuper! and Onyx. They both confirm that the error is in 10.4.8 and that they're getting many reports of problems that emerged when 10.4.8 was released. That led to careful evaluation of the crash logs and bug reports filed to Apple regarding the crashes. I don't know what the bug numbers are and the crash reports don't mean a whole lot to me without access to the program source code. I know that lots of people use these programs without problems, but with the vendors acknowledging significant problems under 10.4.8, one can only wait it out until Apple addresses the problem(s) in a future update.
The problem with SuperDuper is replicable and occurs under a very specific circumstance that many people may not use. I use the scheduler to image my harddrive nightly (I've got 3 identical 500 GB drives, not in a RAID configuration). Typically SuperDuper has to awake from sleep to run as I don't want it interrupting my own work. If the machine has been asleep for a long time (more than 2 hours), it seems to have some difficulty awaking for SuperDuper. The program runs for a few minutes and then crashes, typically right after repairing permissions on the source drive and before starting to actually image to the new drive. If I run SuperDuper directly, without waking the machine from sleep, it runs flawlessly. I have the same problems with ONyX - it runs unattended, requiring the machine to awaken from sleep. These two problems seem related to one specific problem with 10.4.8.
The problems with Firefox are harder to duplicate, except that they are pretty consistent. Firefox seems to crash for no apparent reason. Typically I'm navigating on a page that has a lot of fancy html code. Firefox just barfs and tosses me out. Other times, I can load Firefox cold and have it fail run out of the box.
iTunes crashes under a variety of circumstances, none predictible. And the list goes on.
I've done heavy troubleshooting, including removing RAM, moving RAM around, removing hard drives, switching hard drives from Bay 2 to Bay 1 (Bay 2 is a clone of Bay 1 and I've made the drives interchangeable). I've done extensive RAM testing 100's of replications. I've tested hardware with the AHT as well as TechTool and I've run these tests immediately after one of these crashes occurs and before they occur.
I'm convinced the problem is not hardware. Were it not for the solid responses from the SuperDuper and ONYx developers, I would do a reformat and reinstall, but I'm convinced it is a waste of time unless I'm willing to do it for a downgrade. Since that is the only way to do it, I'm going to proceed slowly.

I have been having issues with my Mini, its crashing, really snow and overall is being very naughty.
I was told to run a hardware test after i could do a repair job. This is one of the few error codes that kept coming up. 4mem/1/40000000: 1c044014
I know the code "4m3m" is a Memory issue but i cant find out what the rest of the code means.

Hi chaps, thanks for your help. So i tried to run AHT but it kept stopping after a few seconds. I managed to pick up some more RAM which is now in the machine and i am running another AHT as the machine would not run properly. Results of the AHT came back clean so i decided to start the machine up in DU, i ran VerifyDisc and it came back with ''Keys out of order, The volume Mac HD needs to be repaired and Error: Filesystem failed. So i then tried to run RepairDisc and these issues came up "Keys out of order, The voloume Mac HD could not be repaired & Error: Filesystem verify or repair failed.
So now i am lost and dont know what to do.....
Any ideas anyone?
