Form Inside InDesign

We have manuals for tools we sale, and I put them together with InDesign. These manuals are interactive PDF's. I am wanting to add a Quality Assurance form inside the manuals. So if customer has a problem with a product or service they can fill in this form and click the email button to send this form information to us.
I use Livecycle Designer ES4 to build all forms we use inhouse and to send out to customers. I have also used Acrobat & Indesign for build forms.
The problem I'm having is with Indeisgn when you place a interactive PDF into Indesign (be it from Livecycle or Acrobat) you lose all info for that form. And making a form inside the document using InDesign and adding a email button. When the email button is used to email the form you don't just get that one page you get the complete manual.
I would think there may be a script you can use to have it email just the one page this form is on. I know some javascript and have wrote some scripts, but I'm still new to the script world.
Is there a way to embed this form so that it will work the way I'm needing?

I inserted the form but the form I am using is an XML form and Acrobat will not let you insert an XML form. You can add it as an attachment.
The only problem with the attachment is, if you don't have something that info's you that there is an attachment present it could be missed.
I made a quick form from InDesign and exported it to an interactive PDF. Then went and inserted this PDF form into the PDF manual and it worked like I am needing. The only problem is when you click the email button it still emails the complete manual. I have to now figure out how to be able to email only the one page.
I thought I may have to add a link that takes you to this form on our website so it could be completed and sent from there.

    Did a search, so I trust this has not been asked/answered previously.
    I am creating an InDesign document that I would like to embed a fill-in form that can be submitted back to me.
    If anyone has seen the Clemson World folio that is free on iTunes, you will see a form embedded in one of the pages that asks for name, address, etc, etc.  There is also a submit button, that then sends the form to a predesignated email.
    I would like to add this type of form directly into my InDesign document and being a complete beginner, I have failed to understand how thus far.  I developed a fill-in form in Acrobat X pro and then placed the form on my page, but there was no fill-in capability.  Only a view of the form.
    Any help would be greatly appreciated.

    What version of Reader are they using, exactly? And are you sure they're
    using Reader itself and not opening the file inside a browser window, for
    example?

    If you ave created the form and then filling it yourself and saving the form, the filled data should be there when you reopen the same form.

    I have been creating forms in InDesign and pulling into Acrobat XI Pro to do form fields. My client would like to fill in the PDF form using Reader, save under a different name locally on their device, and redistribute. The problem I have is that, sometimes, they want to redistribute the form as Read Only. I know how to flatten the form on a mobile device, but uncertain how they can do that with a PC or Mac using Adobe Reader. I looked at FormsCentral thinking there might be a solution going that route, but don't believe there is. Any advice?

    FormsCentral won't help with flattening a form. You could set up the form in Acrobat so that the fields are set to read-only, using JavaScript. Not quite the same as flattening but it's the best Reader can do.
    Note that when Reader is used to e-sign a document, it gets flattened, but this won't be possible if you need to Reader-enable the form and under certain other conditions.

  • H.264 - flash video inside InDesign

    hi there,
    I'm using AME to convert movies for use in InDesign to create folio's for Adobe Digital Publishing Suite.
    I've also poted this in the AME forum but don't know if this is an InDesign issue or it has something to do with AME.
    Inside AME6 I found a setting for iPad 1080p 25fr and one for iPad 720p 25fr.
    But after using these settings, when imported inside InDesign the infopanel shows it as Flash H264 video
    Now, talking to tech support about troubles with the Adobe DPS folio, the support guy told me this is an error with the video.
    He told me the format has to be H264 and not Flash H264. But all video I convert with AME and the iPad settings has that 'format' inside InDesign.
    Don't know where InDesign is looking for that 'format' but I really want to figure out why some video's function well while others, converted with the same AME6 and the same settings, won't.
    Hope someone can help with this,

    I'm pretty positive that an MP4 file with H264 encoding is NOT a Flash format. It is supported in Flash Player but it is not a Flash format. Flash-specific formats would be F4V and FLV.

    The only way to use Forms Central would be to export it the method that gives you the embed code (can't remember what the actual term is off hand) and then use Object--->Insert HTML and past your code in.

    I think I'm messing up everything and i'm confused with how jsf works.
    I've noticed now that my problem doen't have todo with if i'm usin one form or nested forms :(
    The problem is (as i understood) that if the backing bean that I'm using for the chart part is a request chart, every time i submit the form a new instance of the bean is created and so the initialization of the selected checkboxes is redone.
    How can I avoid this? I would like to have only the choosen checkboxes selected.

    Can we have form inside a form? Yes. However you could find out that , regardless of which form gets submitted, all input fields of both forms would get submitted anyway.
    That would render nested forms quite useless.
    But as it has been said, why don't you try it and see for yourself?
    Keep in mind that, you may think you need nested forms, but you really do not need that, and there is a problem with your design.

    Is it possible to define a form (subprocedure) inside a user exit?  When I try it complains that I'm trying to define a form inside of a function.  Of course I have no choice because the whole user exit is a function.  What's the best way to work around this?

    i came to know from one of the posts here in SDN, the options are,
    1) create a custom function module and call it
    2) write ur subroutine in a report prog. and call that subroutine of that report prog.
    3) develop a INCLUDE and add it in MAIN prog.

    If you want to open another form from inside a form, you can use these built-ins. The help document describe the differences between each.
    call_form
    open_form
    new_form
    But when you open a form, it will have its own window. If you don't want that, then use a stacked canvas.

  • Multiple forms inside a tabset

    I have a tabset inside a main form. Is it possible to have another forn inside individual tabs?
    When I put another form inside a tab, tabset gets stuck on single tab and you unable to click through the tabs.

    I tried adding a form component to the tab set. The form component appears as a new form node for the page. The IDE creates a form for you when it creates a new page, so you are not required to add this component to a page created by the IDE. If you need a form within a form, you must create a virtual form instead. You can right-click a component and choose Configure Virtual Forms to create a new virtual form or to add the component to an existing virtual form.
    If you do add a form component to a page, you see the form appear as a selected box and a new form node for your page appears in the Outline window. You can select the form in the Outline window, and then you can drag components onto the form.

  • Problem submitting pdf form in Indesign CS6

    I've designed a form in Indesign CS6 (trial version) and added the submit button with a mailto email address as in the tutorials etc. I have opened it in Adobe Acrobate 10 and extended the forms functionality to Adobe Reader.
    When opened online and the submit button is clicked, a box opens asking which option which best describes how you send email. After selecting Outlook my email client opens with the message 'Form returned; sample_form.pdf attached. the attached file is the filled-out form. Please open it to retrieve the data'. Just above the message bar states 'this message has not been sent'.
    I've also downloaded the Michael Jarott tutorial which I cannot get to send either.

    Thanks for all your suggestions. Unfortunately the form still returns the 'no matches found for [email protected]' I've changed the URL field on the submit button to mailto: xxxxx@xxxx and I've also tried it with no space after the mailto:, unfortunately both these return the above error message. This is the same whatever email address I try. I'm starting to tear my hair out with this one. I've tried it on my home PC arunning Norton Internet Security and on my works PC running Avast. Same problem on both.

    the fm can not test direct in se37, it should be wrapped by a program.
