    If you don't have a rescue email address (you won't be able to add one until you can answer your questions) then you will need to contact iTunes Support / Apple in your country to get the questions reset.

    Go to http://appleid.apple.com and select "Manage your account". Log in with your Apple ID and password, then select Password and Security from the left column.
    You need to know the answers to your existing security questions before you can create new ones. 
    If you don't remember your answers, there may be a link on that page where you can have a reset email send to your "Rescue email address", as long as you provided one in the past. This option will not appear if you have not provided a Rescue email address.

If you are an American I recommend having an English-literate person review your work. There are over 400 million iTunes account holders and Apple receives a great many of these questions every day. Make yours stand out!
    You will receive a confirmation email immediately. Check your email spam / junk folder for it because a humanoid should respond in a business day or two. You may need to provide more information before your questions can be reset. These back-and-forth emails may result in a week's worth of correspondence before it is fixed.

    You need to ask Apple to reset your security questions; ways of contacting them include clicking here and picking a method for your country, phoning AppleCare and asking for the Account Security team, and filling out and submitting this form.
    They wouldn't be security questions if they could be bypassed without Apple verifying your identity.

    Ok, same here, purchases cant be made, becouse as an addition to the password, apple wants your answer to security questions now... The problem with that questions is, the answers were never needed and now long forgotten!
    A quicklink from Neil DOESNT solve nothing. As I first sign in to Apple, got My Apple ID, no RESCUE EMAIL was needed. With no such email I have NO CHANCE to reset this security questions.
    If I reset my password, there in NO OPTION to reset this security questions as well.
    So, I am strandend with some 30 bucks on my Apple acount and for now no way to spend it, thats OK, but some Apps want authentication for even updating, so I am stuck with no updates either... I purchased for about 500 bucks in last years from Apple, maybe thats it for me...
    (Apple ***** more and more, maybe ut ist time to move to Android; a personal opinion).
