How do you create a form to capture customer information

Hello,
I have been looking for a form to capture customer information.  Name, address, phone, email, & maybe even which service they're interested in. They fill out the form and then there's a submit button at the end which they click to send the information.  Is there a form like this in the widgets? Or can someone help me with what is a simple way to capture customer information.  Thanks for all of your help.
Matt

HTML cannot do anything with data.  You need scripts to collect and process form field data.  It would be helpful to know what you intend to do with your captured customer data.
Send data to your e-mail address.
Store it in a database.
Append data to another file on your server.
Forward data to a 3rd party.

  • How do you create a form that has user permissions per form field for different user?

I need to create a form that is used by multi people but each user needs to fill out different fields within the form. I need the first user to make selections from a list and save it to send to another (the selection fields then need to be locked). Then the other user checks one of the selections made by the first user and saves the document to send back to the first user. Not sure if this is possible. I would like the first form field selections not to even show as a highlighted field to the other user (like just be more like a text field). I want the document to be able to be used more than once. The list selections will change and need to be sent to the other user at least once a month or more. Has anyone done this?

There's no built-in way to do that, but it's probably doable using a custom-made script to hide and/or lock the fields.

  • How do you create THIS "grid" in Oracle Forms

    Hi,
I am new to Forms. I have to develop a grid very similar to the one shown here.
How can you create the grid in the bottom??
I can't see using any of the standard Oracle toolbox items (like multi-record blocks) for this?
Any help would be greatly appreciated.
    Thanks
    User 122...

    Hi Soofi,
    Appreciate your quick response.
In the image, can you see a huge grid?
Grid column headings are days of the week, Mon, Tue, Wed etc.
Grid Rows also have names, like Room, Status and Rm. Type.
Cells have various values in various colors (red, blue, green etc) and signs like ">>".
    Can you see that??
My question is, how to create a grid like this in Oracle Forms??? We cannot do it using a multi record block.
So, how have they done it??? The form in the image is an actual Oracle Form from a hotel reservation system.

    Hi,
We do not support this feature right now.  Someone else has posted this to our ideas forum, here - http://forums.adobe.com/ideas/1587.  I suggest you vote for it, because we look at the popular ideas to help guide us on what new features work on.

  • How can I create a form in FormsCentral be a self-correcting quiz for my students?

How can I create a form in FormsCentral be a self-correcting quiz for my students?

    Hi;
You can't make it self correcting.
The only related idea I have is you could publish the Response Table (the "View Response" tab).  You could add an entry for the "right" answers so after a student completes the form the re-direct URL could take them to the Response Table where they could see their entry along with the right answers that you had added.
They could not edit their response to correct it unless they have a FormsCentral account (free is fine) and you share the form with them, otherwise they can still view the link to see how they did (but would also see all of the other students answers).
If this sounds interesting I can give more detail on how to set it up.
    Thanks,
    Josh

  • How can I create a form for users wherein the text field will expand to accommodate additional text?

How can I create a form for users wherein the text field will expand to accommodate additional text?

You need to use LiveCycle (PC Only) to create a dynamic form like that.
The best you can do with Acrobat to view all of the text in a field is to set the field to multiline, and set the size to "Auto" (If you don't set the size to 'Auto', you can enter as much text as you wish, but the user will need to use the scrollbar to view all of the text.)

How do i create a form in live cycle E4 that i can save as a PDF that the user cannot save over?

You can't. If the file is made read-only via the file system, the user will prompted to save as a new file if they attempt to save.

You cannot create a Form from code at run time.
For one thing, the user running your application will, almost certainly, not have  a copy of Visual Studio to hand with which to build anything.  (Not that you actually need 'Studio at all; Notepad and csc.exe are all you actually
    need), but that's another story).
You can do this with an awful lot of pain, using the innards of the compiler from iside your own program, compiling the new Form's source code on the fly and then executing the resulting assembly within your application.  IMHO, that's a simply terrifying
prospect especially as the code you will be compiling will be on someone else's machine and over which you will have absolutely no control.  If you're lucky, they'll spell something wrong and it won't compile.
The "usual" way to do this sort of thing is to create additional, "plug-in" forms as library assemblies and have your program load and execute these at run-time.

  • How do you create a database link in Forms9i

I am writting and application that is to connect to multiple databases and do given task, but the triggers I assign to items only work in the database that you connect to for forms to run.
How do you create a database link in Forms9i?
Thank you in advance for your reply.
    Travis

It seems like you will have to have database links set up in all your databases to point to all other remote databases and have a common global naming scheme for all your objects so that no matter what database you log into you will have access to all objects in the universe.

  • Can you have a form that you create in forms central a routable form?

Can you have a form that you create in forms central a routable form?

    Hi;
There isn't anything specifically to route a form in FormsCentral but I have proposed to a few users they could take advantage of the new "Save Web Form" feature and by instructing users in the form how to route it would be able to accomplish a routing "work flow": http://forums.adobe.com/message/5917475#5917475

  • How do i create a form setup in landscape?

How do I create a form set in landscape?

OK - let me ask you first, how are you delivering your form to others to fill out, are you using the URL so that they fill out the Web/HTML form?  Or are you downloading a PDF of the form that you email to people to fill out?
I think you are likely using the URL (Web Form on the Distribute tab), in which case you want to set the "Form Width" to something wider than 700, that setting controls the width of the form in a web browser.  That controls the width so you can get more of a landscape appearance, you would also add "Page Breaks" where you would want the pages to end so they look the way you want them to.
If you are using PDF let me know and I will share additional info.  The "PDF paper size/orientation" settings in form setup are for the PDF only and you won't see those changes in a web form.
I hope that was clearer but am happy to try again;-)
    Thanks,
    Josh

    Bryan;
On CS-MARS, you can create a user with one of the following roles:
•Admin—has full use  of the MARS.
•Notification Only—for  a non-user of the MARS appliance, use this to send alerts to people who  are not administrators, security analysts, or operators.
    •Operator—has  read-only privileges.
•Security Analyst—has  full use of the MARS, except read-only access to the Admin tab.
You should be able to make use of the Operator role.

  • How do you create aligned interactive text boxs in a PDF with the same width and height?

how do you create aligned interactive text boxs in a PDF with the same width and height?
Without free hand creating the sizing?

Assuming by "interactive text boxes" you mean form fields; in Acrobat, make a form field, then copy it and paste. (GIve the pasted copy a different name so they don't genetate the same field feedback.) Now you have two fields of the exact same size. Shift-click or marquee-drag to select both, then right-click and choose a command from the Align, Distribute or Center menu.
