How do you submit an online form?

I am using Contribute to edit our webpage, it has been fine
for what we've needed so far, but that has been very basic stuff.
We're now wanting to add an online employment application that can
be submitted (directly emailed) to out HR department. I've done a
little research and it appears that Contribute does not have the
ability to create a submit action button, if this is true, what
program can do this that works well with Contribute?
I’ve heard that Dreamweaver can but I wanted some more
input before I ran out and bought something. Something else to
consider…..we eventually want to have a Browse/Upload button
so that someone can upload and send their resume to us. Thanks for
the help everyone!

I have been playing around with Acrobat, and I've found a
submit button that emails the form to you, but that requires our
applicants to have email and we can't assume they have email. Do
you know if I can find what I need in Acrobat Pro?
We are a nonprofit agency, so I'm trying to use the
software/programs we have rather than going out and buying
something. I'm hoping I can find what I need in Acrobat. fingers
crossed.

  • How can I have each online form have a unique and appropriate http address - they are all under one

    How can I have each online form have a unique and appropriate http address - they all have an Adobe address that includes one of the form details that is inappropriate for the customers completing the form.
    e,g
    Form 1 for customer 1  LMD Community
    Form 2 for customer 2  Turner Expo Jan 2014
    Form 3 for customer 3  Grade A Distribution
    Form 3 online is accessible for fill-out  at https://adobeformscentral.com/?f=p2CwBtW*G6zkt3X*lBDEPQ
    The form opens properly but ...
    The tab that opens in the browser reads "Turner Expo FLHS Jan 3-5, 2014"
    Why?
    How do I get the proper tab label for each form?
    Thanks
    Bob Pye

    You can set the form title (the text that appears in the Browser tab/Window) in the Form Setup dialog.  With the form in question open in FormsCentral for editing, click on the Form Setup button in the toolbar.  In the resulting dialog set the Form Title to the text you want.

  • How do you fill in a form that was saved as a template? When I try, the "template" shifts, which is not how a template should work!

    How do you fill in a form that was saved as a template? When I try, the "template" shifts, which is not how a template should work!
    Templates, as I understand them, are fixed; you can "fill them in" without shifting them. Yet, I've saved doc as a template AND it shifts. Argh!

    Thanks for your response.
    I've managed to fix it but I wouldn't recommend the following as I'm sure its unsupported and I'll probably have problems down the line
    On the host
    * mv /etc/zones /etc/zones.bak
    * reboot single user
    * apply kernel patch (the one that ultimately I had to get on and which was giving pre-req dependency errors when running zones)
    * create a new zone now that host system is all up to date
    * Copy the operating system directories of the new zone (the one that's all patched) over the troublesome zone (the one that can't take the patch). Take backups first
    * cd /zones/new-zone/root
    * find sbin -print | cpio -pudm /zones/bad-zone/root
    * find usr -print | cpio -pudm /zones/bad-zone/root
    * find lib -print | cpio -pudm /zones/bad-zone/root
    * cd /zones/new-zone/root/var
    * find sadm -print | cpio -pudm /zones/bad-zone/root/sadm (this copies patches and pkgs directories and I'm sure a lot of other stuff that I didn't want)
    * mv /etc/zones.bak /etc/zones
    * reboot host
    * boot the old zone up. It should now have up to date patches
    I'm sure there are real no-no's in taking this approach but it seems to have worked for me. I'll update if I find there are some horrors down the line

    Personally I think you are going about this all wrong.  I believe someone recommended to you in a previous post to look into the HTML5  ( http://forums.adobe.com/thread/874917 ) you need to end up creating the site with Adobe AIR as opposed to the way you are going about doing it.  That would be creating an application like Tweetdeck that connects to the internet and uses interactive elements.  http://www.adobe.com/devnet/air.html
    If you want to link a file within a Flash document you would receive a quicker and more in depth answer in the Flash forum as it will require ActionScript.
    http://forums.adobe.com/community/flash
    It sounds like you have a ton of ideas, but your lack of information suggests that 1. you don't have all the knowledge necessary to get the project off the ground and 2. your reluctance to share it suggests that you have an idea that you feel the program is proprietary.  Given that this is the case you might be best served looking for an experienced Flash or AIR developer to help get your ideas going in motion. There is nothing wrong with trying to learn but the information you are giving up to go on is very broad and unfortunately it does not appear that you are getting the answers you are looking for.

  • How can recipients submit completed PDF forms using a MAC?

    We have created fillable PDF forms using LiveCycle Designer 8.0.  They include
    submit buttons that are set to submit the completed forms as PDFs.  Rights have been extended for Adobe Reader users.  They work great when the recipient is using a PC, but when they are using a MAC we have a number of problems:
    1) the Print and Submit buttons do not work
    2) when the recipient saves and e-mails the file manually, the data they entered is only visible when you click on that field
    Help!  How can our MAC recipients submit their completed form as a PDF that displays all of the data!
    Thanks!

    Thanks Niall.  This information proved really helpful in confirming the problem.  We ran some tests and the problem does seem to only occur when MAC recipients are using Preview.
    We recieved this link from Adobe, which provides a solution if you have access to all the recipients:

    Thanks to Niall and Paul for all your help!

  • How do you fill out a form more than once from a single computer?

    URGENT QUESTION...
    I am using a FormsCentral form as a Job Interview form.  The interviewer needs to clear the form after each submission to fill the form out again for the next person interviewed.
    She tells me that she can't see how to clear the form to fill it out for the next interview, and I don't have the answer.  Would appreciate receiving that answer real fast, and the next interview will be at 11am EST today.
    Thanks.

    Randy,
    Answering your question about how I created the initial PDF form...
    I created it with the Employment Application template in FormsCentral, adding many additional questions to the template.  When I finished doing that, my recollection is that I clicked on “Save as PDF Form.”  And that worked fine.
    The PDF form was then accessed by the interviewer without any problem.  She dutifully filled out the blanks, clicked on the SUBMIT button, and the first report then showed up as expected in FormsCentral under “My Forms.”
    We don’t particularly want to have this form online, although we can put it online if necessary.  Since there is only one interviewer who will be filling out this PDF form during the interview of each job applicant, our only issue is how to most easily clear the form after each interview is posted, without losing the SUBMIT button.
    Thanks,
    JS

  • How do you update the Complete form?

    How do you update a form in FormsCentral?  Specifically, the form that appears after pressing the "Submit" button?  In addition, how do you add a "Reset" button?

    You control what happens after clicking the submit button using settings in the Options tab.  The Confirmation Message setting will allow you to customize the message that appears.  The Redirect URL setting allows you to control where the user goes after the Confirmation Message.  You can reload the form or direct the user do a different URL.

  • How do you make sure the form data prints (commercial offset printing)?

    We are commercial printers and have been using a PDF workflow for quite a while now. We recently received a PDF form to print but any information filled in the form just dropped out (all non-data printed fine). When we send it to a laser printer, everything is there but when it goes through the RIP and imposition software (both Heidelberg), all the data is missing.
    When I open the PDF from the client, I am not able to change or even highlight the data. There is a message saying the information is locked.
    We were able to get around the problem by importing the PDF into InDesign and making a new PDF from that but that seems to be an extra step that coulb be avoided.
    Anyone else with a similar problem (and solution)?

    How can you tell that it is an XFA form? If I can identify the form type from when I get jobs like this, I can use the workaround from the beginning.
    Here are a few more screen shots of things that are happening:
    When I try to Print to a PDF:
    When I use the Mac "Save to Adobe PDF" in the print dialog:
    I get this message often:
    and when I try to embed the missing font (PitStop), I get this:
    The only thing that has worked so far is placing the PDF in InDesign and Exporting to a new PDF from there. I guess this is making the flattened PDF that you talked about.

  • How would we create an online form for doctor review?

    We need to replicate an OSHA form on a secure website for employees to fill out. The system would alert a doctor via email that forms need to be reviewed. The doctor logs in, reveiws the answers and clears or fails the employee, then attaches a digital signature. The form would be routed via encrypted email to receipient.

    We don't really support this specific workflow.
    You might be able to achieve what you want but it would include some manual steps :
    You can create a form and distribute it via FormsCentral
    The doctor can receive email notification when data is submitted
    The doctor can log in FormsCentral to see the data and "Download Response as PDF Form" to see the data in a PDF format instead of looking at the table.
    The doctor can then delete the row in the Response Table (see he now has a PDF version of the data)
    I'm not sure what clear or fail the employee mean in regard of the form (is this something you mark on the form?)
    The doctor would need to manually to use another Adobe service, EchoSign, to sign the PDF response.
    https://www.echosign.adobe.com/en/home.html
    The doctor would need to manually contact the email the signed PDF or contact the receipient.

  • How do you Save a pdf form automatically to a designated folder using designated fields in the form?

    Hello,
    I am in the process of creating a “service excellence feedback” form.  The idea of this form is to have a blank form living in our company’s intranet page to be used by multiple departments that receive feedback and then have a chain of updated investigations and follow up to be documented within the same file.
    The top portion is to be filled out by the department receiving the feedback. It collects all the pertinent data needed including the person giving the feedback’s First and Last name. Once the top portion of the form is filled out I want to have a “submit” button that will do;
    1. Save the pdf automatically to a designated folder AND automatically save it under the last and first name that is entered in the form. So the form will be saved in the designated folder under “Smith, John.pdf”.  I have already figured out how to format the button to “save as” however I would like to eliminate the person filling the form out to have ANY responsibility to naming and saving the file to the correct folder.
    2. After it is saved I want it emailed to a specific email address (which I have figured out.)
    Once the steps above are completed and it is emailed an investigation will be performed by me and I will document my findings and then send it by another “submit” button which will save the document (writing over the previously saved version with this new version) and then emailed on to the next person to provide the next step of follow up. When the next person in line finishes they will hit another “submit” button that will again save the new version and send it back to me.  We continue this back and forth with a few more follow-ups performing the same save and send options.
    I need help in how to write a script/java to perform this task of saving. I have already figured out how to program the button to email the form.  Help please!!!  I am using Adobe Acrobat XI Pro

    It can also work if you certify the document and each user chooses to trust it to execute privileged JavaScript, which may be feasible in this setting. The method you'd use is doc.saveAs: http://livedocs.adobe.com/acrobat_sdk/9.1/Acrobat9_1_HTMLHelp/JS_API_AcroJS.88.524.html
