You cannot and need to ask Apple to reset your security questions. To do this, click here and pick a method; if the article doesn't list one for your country or you're unable to call, fill out and submit this form.
Changing the rescue email address requires supplying two of the answers. They wouldn't be security questions if they could be bypassed without Apple verifying your identity.

Is it possible to insert a form in a image that only becomes visible when it is clicked?

I am trying to add a basic subscription form to my landing page in dreamweaver. I have an example of it below, and so I am wondering how do I add a submission form to an image that isn't visable half the time?

The example you provided was created using Edge Animate.
It could also be created in Dw using html and css. The two possibilities using html and css are -
Use css transforms, transitions and animations, (this would give you the same result as when using Edge Animate, but with much less code) this would only work in modern browsers.
Use absolute positioning, z-index and if required a little javascript. This would work across all browsers.

Basically you need to have a credit card and billing address in a country that FormsCentral is sold in. You can use your subscription from anywhere, so you can purchase if you have a card/billing in a supported country and use in Singapore.

WANTING TO UPGRADE TO FORMS CENTRAL

I work for a university here in Southern California and we would like to be invoiced yearly to use Forms Central. How can we get an invoice so we can make this a reoccurring payment yearly?

You can purchase the Plus annual plan using a credit card from our website. We don't support purchasing using a purchase order.
If you want to purchase using a purchase order or something besides a credit card you would have to go through a reseller. The minimum number of seats you can purcahse from a reseller is 10.

Every time I go to upgrade there's no options to upgrade so I can have more than 50 responses, how do I get more than 50 responses then? There's no upgrade or buy functions, just information, not even price or anything, it just takes me here..

Is there no way I can increase my responses for my current form as this is the form that was distributed to everyone. Because we were unsure if the event was going to go ahead a subscription wasn't purchased but since it is going ahead and the form has been previously dispersed I now need to increase response allowance to cater for numbers before everyone else fills in the form which was asked to be done by the end of January, are there any other options to continue with the form?

Forms central - creating fields that total

Anyone know if you can create fields that total based upon user's response? example field A is 50, Field B is 25, Field C the total is 75?

Not with fields on a web form created with FormsCentral. You can use PDF forms that include automatic calculations with FormsCentral, but you'd have to use Acrobat to set them up.

Trying to Purchase Forms Central Plus

I am trying to purchase the above product however when I am taken to the check out and asked to log in qwith my email details and password I get a message stating that this id was created in AU (Australia) and I should create a new id for america.
I do not want to create a new ID. How do I make this purchase so I can upgrade

When you created your Adobe ID with your email address, you chose Australia as your country. Unfortunately, the Adobe Store in a particular country (e.g. Australia) can only complete a purchase when the country of the Adobe ID matches the country of the Adobe Store.
The only way to circumvent this would be to sign into Adobe.com with your existing Adobe ID email address. Go to Welcome (in the upper right) > My Preferences > Adobe ID > Change. Change your email address to a new email address or to a dummy email address like [email protected] This will free up your original email address. Now, create a new Adobe ID using your original email address and choose the US as your country. Again, you can then complete your purchase via the USA Country Store.
IMPORTANT NOTE: If you have other Adobe products or services registered currently with your original email address, they will now be associated with this new or dummy email address. However, if you don't have any other products or services registered, this shouldn’t be an issue.

Place the iOS device in Recovery Mode and then connect to your computer and restore via iTunes. The iPod will be erased.

My iPod has a cracked screen, but that isn't the problem. Can I replace it?

For about two to three months my iPod Touch 4th gen has had no sound. It doesn't work. I've tried all resets and the sound comes back for a couple hours, then dies again. This iPod was a replacement, but it's still covered by the first one's warranty. I've been planning to take it back but never gotten around it, because its a big hassle and costs a lot in petrol. I sort of regret not doing it now. My screen just cracked (Of all things, my crystal necklace dropped onto it) so can I still get it replaced seeing as the screen isn't the problem, but the sound issue is?

I cannot sign into iCloud on my iMac. I have read all of the help information, but that isn't working. I cannot do anything with my system preferences - like add a new printer - because it won't log me in, sign me out, forget password doesn't work, etc.

I have been unable to log in to iCloud on my iMac. None of the hints online have worked. I can't sign in (with any password), sign out, the forget password doesn't work, the support button doesn't work. It's locked. I can't find anyway to fix this - I need to add a new printer ASAP - help!!!

The "Mail, Contacts & Calendars" window popped up a couple of weeks ago and is asking for my Apple ID password, which it will not accept, It won't sign me out (I am not logged in), the support or forgot password buttons won't work. WiFi says I need a password but there isn't one when I connect with my phone

The title says it all. I'm trying to connect to McDonald's public WiFi and when I connect with my phone, it works normally, but when I try to connect with my Mac, it says that either the connection has timed out or I entered the wrong password.
I'm running OS X 10.7.4 Lion.

I've never actually managed to connect to the network, it automatically disconnected and reconnected to my usual one.
