Kernel Panic: ...in module "ip" due to a NULL pointer dereference
I already had 2 panic-s in the IP module over the last 24 hours. I am running Solaris Express 11 - snv_151a - 2010.11.
I am trying to:
- decode which function is failing
- determine what's (who since its network) causing them
- find a bug fix for the problem
I have preserved both dumps via snapshots to be analyzed further as needed.
Here's the details:

I'm not seeing any obvious bugs relating to the stack. There's something similar but it was fixed in b147. Can I suggest and recommend that you raise a Service Request so we can review the data and establish if this is a known or new bug. When creating the new service request it will need to go to the network team so the following will need to be used:
Product: Oracle Solaris Express
Component: Protocols and Routing
Sub-Component: IP - TCP/IP
Once the SR is raised, we'll need the crash dumps and explorer from the host in question.
If the crash dumps are of the form vmcore.n, then please gzip them
If the crash dumps are of the form vmdump.n then they are already in bzip2 format so please do not compress them.
Prefix the filenames with the SR number and upload them to https://supportfiles.sun.com and select "/cores" from the list of options in Step #2
Explorer will collect the configuration from the system which is useful for reviewing alongside the crash dump. It should live in /opt/SUNWexplo/bin/ and can be executed using /opt/SUNWexplo/bin/explorer. If it's not installed, please download it from Doc ID 6972175 - "Oracle Services Tools Bundle (STB) - Explorer, SNEEP, LWACT, RDA"

Steve

Panic with Raw Socket-Page fault in module "ip" due to a NULL pointer deref
I see a panic when using raw sockets with Solaris 10 10/09 (u8). I included a sample program that can cause this issue (panic happens when a udp datagram is received on port 60000). This sample code works as expected with the previous version I was using - 5/08. If I bind with a port number of 0 I don't see the panic but I don't receive anything either.
I believe I have all the latest patches installed. I'd appreciate any assistance in resolving this. Thanks...

Kernel panic "kernel NULL pointer dereference in pacpi_set_dmamode"
Hi. After upgrade to linux-3.6.2-1 my PC fails to boot with this panic: http://imageshack.us/photo/my-images/5/imag0085.jpg/ How do I fix it? I can provide more specific info if needed.

It's 404
I fixed the link (dropped the l in html).
glenn69 wrote:How do I accomplish this when I cannot get past boot into the OS?
Boot a live cd and chroot (see the wiki on chrooting).
glenn69 wrote:Also, headkase, which log file are you displaying in your post? I can access my disk via a live cd, but cannot find the appropriate log for my machine.
That's the content of the email that I failed to link to properly.
Does anyone know if there is a bug number for this? I'm experiencing this bug on my MacbookPro 7,1 (2010) too (I am forced to use IDE as sata is not supported on this machine in bios compatibility mode afaik).
The title of this thread is rather non descriptive. How about name it something like:
kernel 3.6 panic ("kernel NULL pointer dereference in pacpi_set_dmamode")

Does it reboot successfully?
Also, you really need to read this:

reboot is success.
use pppoe,switch to wifi, has this issue.

IPQoS panic: BAD TRAP in module "flowacct" due to NULL pointer.
I've recently started using IPQoS and the flowacct module to do some very simple tracking of bandwidth usage (along with one tokenmt action to meter traffic to and from one particular zone) on a host with multiple zones, running Solaris 10 x86, 118844-30. This host has been otherwise stable to date. A couple of hours ago, I got a kernel panic, apparently caused by flowacct:

I had a single "kstat -m flowacct 90" running at the time, but the timing doesn't particularly coincide. My flowacct setting look like this:
action {
module flowacct
name zone1
params {
global_stats TRUE
timer 10000
timeout 10000
max_limit 2048
next_action continue
Pretty much a cut and paste from the Solaris 10 IPQoS documentation. This configuration didn't suffer any similar problems on a test host.
I've searched various places, not had any joy. Obviously, I'm down a while on kernel patching (ironically, I've been holding off to minimise downtime for users, and now this.) But, while 118855-19 does replace some of objects in question, I see no mention of any particular fix for this problem. I'm going to go ahead with patching to 118855-19 (and whatever else smpatch recommends) within the next few days, but I've disabled IPQoS. I'm a little nervous about switching it back on on a production server. Shame.
Anyone had any similar experiences with flowacct?
Cheers,
Chris.Good news, everyone! A repeat on a test host that's patched up to date as of last week, including kernel 118855-19. I'd tell you what smpatch thinks is missing, but "Failure: Response code was 403".

Kernel panic with ipf and patch 125014-02?
Hi,
After bringing a bunch of Sparc servers running Solaris 10 up to current patch level I've been experiencing strange and unstable behavior. All servers are running with an ipfilter configuration.
One server has been freezing on the network interface at least once every day without any syslog notice. A reboot is the only way up again.
Now today one server (a Internet proxy server) did a kernel panic twice - never seen that before on this server.
It seems like it was caused by the kernel module ipf. And since it has never happend before I guess it could be caused by the IP filter patch 125014-02.
Anyone experiencing something similar and am I on the right track with suspecting this patch to be bad?
Thanks
Kasper

Case# 65340046 - panic due to IPF firewall
From: Fletcher Cocquyt
To: <[email protected]>
Date: Feb 12 2007 - 10:09am
We had another panic & reboot, so we will be forced to disable IPF firewall
(is svcadm disable sufficient, or do I need to do more to prevent the
panics?)
How can I be notified when there is a proper Sun patch to fix this?
thanks

Is it IPF firewall related?
Will disabling ipf (until there is a fix) avoid this bug ?
Thanks

Fletcher,
This system panic'd due to bug 6490522: S10 System panic bad mutex in
ipf:fr_derefrule. Unfortunately at this time we do not have an offical fix
for
this bug, however we do have a test binary built. Please let me know if you
would like to test this binary on this system.
Thanks.

1-800-USA4-SUN (option 1, option 1, then case #) -
Solaris 10 / RSYNC / ZFS Kernel Panic
I've already raised a call with sun about this but am wondering if anyone else is getting anything like this. We have a new set of V215's we're attempting to roll out for a project, however since Sunday each one of them has crashed at least once when rsync'ing data across from the older servers to the new zfs filesystem on these boxes...
Below is the crash messages we are getting, anyone had anything similar or any idea?

Cheers,
MikeI've already raised a call with sun about this but am wondering if anyone else is getting anything like this. We have a new set of V215's we're attempting to roll out for a project, however since Sunday each one of them has crashed at least once when rsync'ing data across from the older servers to the new zfs filesystem on these boxes...
Below is the crash messages we are getting, anyone had anything similar or any idea?

Powerbook G4 Unresolved Kernel Panics
17” Powerbook G4
System would receive Kernel Panics at random intervals (while running Mac OS 10.3 Panther).
Originally believed to be an Operating System a clean install of Mac OS X 10.4 Tiger was attempted but it could not be completed (or at times even started) due to additional Kernel Panics during the Tiger installation.
At this point corrupt system memory (RAM) was thought to be a possibility as soon troubleshooting and knowledge bases pointed to. The RAM module (a 512 MB module) was removed from its original slot and placed in the open slot. When the Tiger installation was attempted again the entire process went through without any problems or panics.
After Tiger was installed all updates were installed, and software added. The system functioned without any problems but only for a matter of days, and then the panics returned.
New RAM was purchased at this time believing that the original module was corrupt, but this did not seem to solve the problem. The unit was taking to the Apple Store where the techs ran a Hardware Test and all things passed, but they noted the Hard Drive was performing irregularly and stated a bad Hard Drive as a possibility.
A new Hard Drive was purchased and did not solve these problems at all; all methods were attempted at this point:
1. New Hard Drive, New RAM
2. New Hard Drive, Old RAM
3. New Hard Drive, Both Old/New RAM
The system continued to receive Kernel Panics. A full copy of Panther and Tiger were installed to a Maxtor Firewire 250GB external Hard Drive months prior, booting from this external drive seems to not exhibit any problems or panics.
Any ideas of what could be the problem? Appreciate any help?I can't tell you about the error log, but do you have the latest OS updates? For example, this last weekend's Security Update 2007-009 v1.1 is supposed to specifically fix a problem where Safari would crash when visiting certain webpages.
OTOH, stuff sometimes happens no matter what level of update you have. Is this repeatable? Kernel panics seldom seem to stem from hard drives or logic boards. -
Update 2012-03-31: Workaround confirmed to work, see tenthirteen's post.
Update 2012-02-03: I've narrowed down the problem to the 3.x kernels. Downgrading to kernel26-2.6.38.5-1, with the other packages up-to-date (using nouveau instead of nvidia driver), solves the problem. Still, I still consider this a problem since it forces me to use an older kernel, so if anyone has any idea how to find the problem I would be very thankful!
When I came back home after the summer vacations I did a huge upgrade with pacman -Syu (of course I read the announcements before). This included linux (3.0.3-1) from kernel26 (2.6.38.5-1) and udev (167-2 -> 173-3). After this point, the boot process randomly fails from time to time; today I had to restart more than 20 times before my machine would boot successfully.
It always occurs during "Waiting for UDev uvents to be processed". Some of the errors are:
1. The computer freezes when the message is shown (cursor stops to blink).
2. Nothing happens for a long time, then Waiting for UDev uvents to be processed [ FAIL ]s.
3. Short and long error messages that look similar to this guy's (except for the last line).
I greped for some of the error messages in /var/log but they don't seem to be logged, so the examples below are from my handwritten notes and thus not very complete (there are a lot of other error messages except these lines). It's also possible that there are some mistakes. Note that they're from different boots.

These errors are seemingly arbitrary which makes it very hard to find the problem. Sometimes it works without a single complaint, sometimes it says something about "No caching mode page present, Assuming drive cache: write through", and sometimes I get different errors (the examples above are only from today!). As you can imagine, it gets pretty annoying.
So far I've tried to:
* Use the fallback mode - nope.
* Rerun mkinitcpio -p linux - nope.
* Append noapic and nomodeset to the kernel in GRUB - nope.
* Install and try kernel26-lts - seems to work, although I don't understand what other differences there are (e.g., it doesn't start X). At least this seems to suggest that it's not a hardware problem.
* Unplug all USB devices before boot - sometimes works, sometimes not. Doesn't seem to be a direct connection.
* Remove my custom settings in /etc/modprobe.d - nope.
* A lot of other things I've forgotten.
Edit: I forgot to mention, once it's booted, the system works perfectly fine. No kernel panics etc. That's the second reason why I suspect it's not a hardware failure.
I've been thinking about trying to downgrade the kernel and udev, but with the renaming and all dependencies I'm not sure how to do that.
Since I've run out of ideas how to troubleshoot this issue, I would appreciate if I could get some suggestions here. I've search around and found similar posts but no solution that works for me.
Finally, here are some more information.

Last edited by Mogger (2012-03-30 23:33:38)I've temporarily fixed the problem by doing the following:
removed cryptsetup (1.3.1-2)
removed lvm2 (2.02.88-1)
removed mdadm (3.2.2-4)
removed linux (3.0.6-2)
upgraded mkinitcpio-busybox (1.18.5-1 -> 1.18.2-1)
installed gen-init-cpio (2.6.36-1)
upgraded udev (173-3 -> 167-2)
upgraded mkinitcpio (0.7.3-1 -> 0.6.11-1)
installed kernel26 (2.6.38.5-1)
upgraded nvidia-173xx-utils (173.14.31-1 -> 173.14.30-1)
upgraded nvidia-173xx (173.14.31-2 -> 173.14.30-1)
upgraded initscripts (2011.07.3-1 -> 2011.04.1-2)
upgraded net-tools (1.60.20110819cvs-1 -> 1.60-18)
I also restored my old /etc/mkinitcpio.conf, /etc/inittab and /etc/rc.conf.
I'm still interested in a solution to the original problem. Is it a problem with my configuration, the new kernel, the udev version or something else?
The difference in inittab is just a -s option and the mkinitcpio.conf files are the same except for some comments. rc.conf has the biggest changes and is now (removed comments):

You may find the troubleshooting steps in the section "Troubleshooting a recurring kernel panic" toward the bottom of the article below helpful.
OS X: About kernel panics
http://support.apple.com/kb/TS3742
As pointed out in the article, please keep this in mind:
Diagnosing a recurring kernel panic can be difficult. If you would like to avoid this process, or do not know how to perform any of the following steps, consider bringing the Mac to a Genius at an Apple Store or an Apple Authorized Service Provider for assistance. Be sure to ask that, if the drive needs reformatting or replacing, they contact you about escalating your case to a special data recovery service. If you plan to visit an Apple Retail store, make a reservation at the Genius Bar using http://www.apple.com/retail/geniusbar/ (available in some countries only).

Multiple kernel panics & black screen on Macbook Pro Retina early 2013
My Macbook Pro Retina (early 2013, 8GB ram, 250 GB hard drive) has been experiencing multiple kernel panics forcing rapid-fire restarts of the computer multiple times within a very short time frame. This is followed by the screen staying black even if the computer is on (keyboard lit up, can hear volume buttons) but nothing is on the screen. Once the screen goes black, I have to employ some combination of manually shutting it down (by holding down the power button) and then turning it back on (+/- doing an SMC reset) to get the screen working again. I also tried a PRAM reset once, but it didn't seem to have any effect, and I've repaired the disk permissions.
This issue has happened multiple times in the past week. It seems to vary as to how long it takes to happen again (could be a few days, could be less than an hour), and it has happened during multiple different activities (surfing the web, using Word, using Mail). I have experienced this both on Mavericks and after updating to Yosemite over the weekend.
I took it to the Apple Store on Saturday and while the Genius saw that the screen was black when I initially brought it in (even though the computer was on), we couldn't recreate the issue and the diagnostic tests he ran showed nothing. It even started back up fine for him. I thought I was OK, until I had another round of kernel panics + black screen this afternoon. At this point, doing a normal restart of the machine results in a kernel panic before it even makes it off of the loading screen and then a black screen.
This time, I checked the logs and saw the panic report included below. I don't know enough about this to understand what it means, but hopefully someone else does! Some internet searching has suggested that it might be a problem with the GPU (and the log below says GPU panic). Help, please!

Make a "Genius" appointment at an Apple Store, or go to another authorized service provider. You may have to leave the machine there for several days.
Back up all data on the internal drive(s) before you hand over your computer to anyone. There are ways to back up a computer that isn't fully functional—ask if you need guidance.
If privacy is a concern, erase the data partition(s) with the option to write zeros* (do this only if you have at least two complete, independent backups, and you know how to restore to an empty drive from any of them.) Don’t erase the recovery partition, if present.
Keeping your confidential data secure during hardware repair
Apple also recommends that you deauthorize a device in the iTunes Store before having it serviced.

Downgrading quad G5 to 40GB SATA - Kernel Panic
Howdy folks - I own a G5 Quadcore that came with a 250GB SATA HD. I purchased a second drive (both are Western Digital Caviar SE) and installed it as my backup drive. Everything works great, no issues.
I bought a Western Digital 40G SATA (also a Caviar SE). What I want to do is take out my 250's, put this 40GB in and install a base copy of OSX 10.4. This is so if I have to take it in for repair to CompUSA, those pinheads won't have access to, well, my life. I had a G4 that lost a power supply, and had to take the whole PC in. So I decided I'd never leave years of documents, images, emails, finances, etc, in the hands of the 'techs' at CompUSA. I put an 8GB drive in that G4 and installed OSX on it. So if I ever had to do that again, I'd simply swap drives and bring it in. They have a fresh copy of OSX to test my machine with, I have peace of mind that I still possess my data.
Anyway, I'm trying to do the same thing on this Quad. So I bought the smallest HD I could find, this 40GB WD SATA. Here's where my trouble starts:
I stuck in the OSX install DVD. I shutdown the G5. Unplugged both existing drives, attached the 40GB. Rebooted, held down the C key, after the gray apple logo appeared, I got a kernel panic. Tried again, same issue.
Fearing the drive could be bad, I hooked the existing primary drive back up, along with this new 40GB, rebooted to the OS on the 250GB. It found the blank disk, I initialized it. Ran verify disk, everything checked out A-OK. Shut down again.
I unhooked the primary 250GB, and left just the new 40GB hooked up. Rebooted again, held down the C key, kernel panic at the same spot.
At this point, I just hooked my two 250's back up, rebooted into the HD, and all is back to normal.
So, is there some type of incompatibility with these 40GB drives? Are they -too- small? Any reason why this could be happening? None of the drives are jumpered, so not sure what else to check. Seems like my other Macs I could toss in any blank drive, then boot up with the CD, format, install, everyone was happy.
Any suggestions?
Quadcore G5 2.5Ghz Mac OS X (10.4.4)I don't know why the 40G didn't work, but I like your reasoning. If you're stuck with that drive, and don't want to spend anymore money, try this:
Install your normal boot drive as primary, the 40 as secondary. Boot up, then use Carbon Copy Cloner to make a bootable copy on the 40G drive. It might take some fiddling if your current boot drive is more than 30GB, but you should be able to limit what it copies to just the essentials.
Then try to boot the clone. If it doesn't work, cut your losses (in wasted time) and get a new 160 0r 250 WD drive ($85 and $110 respectively) just like the PowerMacs come with. Newegg has the 250 for $103 plus shipping.
Before I shipped my PowerBook back for service, I cloned the drive (and ran the clone on an iBook for 3 weeks), cleaned everything off the internal drive but the basic OS. Of course, that wouldn't have been an option if the computer wasn't runnable. It had a splotchy display, but ran fine. -
Mid 2010 Macbook Pro Keeps Restarting due to Kernel Panic. Ideas for how to fix?
So my Mid 2010 Macbook Pro keeps randomly restarting, and giving me a screen that says something like "your computer restarted due to a problem."
Looks like it as a kernel panic, but I have no idea what this is. Seems to be no pattern for when it crashes. Used to do it maybe once a month and now this is the 5th time in two days.
Any idea what might be going on? Looked at past threads but nothing seemed to apply.

Looks like it as a kernel panic, but I have no idea what this is. Seems to be no pattern for when it crashes. Used to do it maybe once a month and now this is the 5th time in two days.
Any idea what might be going on? Looked at past threads but nothing seemed to apply.

After upgrade to OSX Mavericks my 2009 MBP keeps crashing due to kernel panic
After upgrade to OSX Mavericks my 2009 MBP keeps crashing due to kernel panic (happens every 20 minutes or so). Please help me with this, it's getting very frustrating. Have tried disk utility, there were errors (file system was corrupted), the disk utility managed to repair the disk but the crashes continue.
