Need to create a form

Hi
Can anybody advise me on how i can create a form with two fields:
The first field should allow me to select a record which already exists in my database (gameid)
The second field should allow me to update a section of that record (gamereview)
Also a, submit button would be required to update the record
Thanks

<form name="myForm" action="yourActionPage">
<input type="text" name="gamerID" />
<input type="submit" value="Submit" />
</form>
Thats about as specific as I can get. You dont specify a server side you are using. You would need to query your database in order to specify a particular record.
WHERE gamerID = Form.gamerID
Not a complete answer, but again depends on the server side.

    I NEED TO CREATE A FORM IN WHICH I CAN UPLOAD PICTURES TO.  CAN A BLANK BE ON THE FORM THAT YOU CAN CLICK THE PICTURES INTO

    Hi;
    There is an "Attachment" field that allows the form filler to upload attachments (images are supported).  Here is a tutorial on adding the attachment field: http://forums.adobe.com/docs/DOC-2656
    I was assuming you meant the form filler, as the author you can of course add pictures to the form using the "Image" form field which allows you to add images from your computer or from Google/Flicker searches:

  • I need to create a form on my website that viewers fill out and send to me on my iWeb created website. Is there a template or widget that can create fill-in forms?

    I need to create a form that viewers can fill-in and send back to me from my website.  I created the website in iWeb and would like a template or theme that has the form creator included.  It there a widget?  Sorry, I'm a novice.

    If you want to raise your web design skill level above that of the average iWeb user, you can try creating your own. The actual form layout is fairly easy if you use CSS to style it since the amount of HTML is minimal now that table layouts are a thing of the past.
    Here's some examples of forms created in iWeb...

    The formmail.php script is probably the hardest part to get your head around and the example provided in the download on the above page uses a very simple one along with a formmail-thank.php.

    Nowadays we need to consider mobile device users more and more so forms need to have the special inputs required to launch the various types of keyboard on these devices.
    Here's an example of a form which responds to browser/device width. On mobile devices, the
    jQuery UI Datepicker disappears to be replaced by the mobile version.
    For those of you who don't have a mobile device to test it on, this page also has illustrations of the various keyboard types and the alternative date pickers...

    the action of the form is also in iscript1 function getinputs
    iscript getinputs will just print out the firstname and lastname
    Could I get some help with this?

    Your site is running on an Apache server, so it definitely doesn't support ASP. Apache frequently supports PHP, but I couldn't tell from the headers sent by your server whether it's supported on your site.
    I suggest that you contact BT Business support to see if they offer a formmail script. Most hosting companies do. The support pages should describe how to set it up.

  • Need to create a form for printing

    I need to create a printout of statistics.
    My first thought is to use a JEditorPane or JTextPane to display my data and then shoot it off to ActiveTree's SmartJPrint lib where I can either print it or save it as a pdf file.
    Now I have already played with using html in the JEditorPane but I would like to know what others think?
    i.e. one problem I am already noticing using html is making sure everything fits on the page. Some printouts turn out to be to large for the paper size.
    Thanks for any feedback.

    Well I use JFreeReport from www.jfree.org
    It is easily to configure, if you use a TableModel for displaying your data!
    It provides printing, *.xls, *.csv, *.pdf

  • Need help creating a form

    We use forms now that are printed for us and we hand write the information on them.
    They "carbon copy" themselves onto the same form of different color (ie; white copy, yellow copy)
    We would like to develope a form that will "carbon copy" iself to additional copies.
    the "carbon copies" don't have to be different colors we could simply put a separate header on each page for where that copy needed to go.

    FormsCentral does not support this kind of functionality.

    I'm tring to add javascript to a drop down box so that, when a selection has been made in the drop down box, it will display a image/button. I need the drop down box to have a default value that will hide all images/buttons.
    The list in the drop down box is going to be a list of products e.g. product A, B, C, and the default value to be "please select a product..."
    Can this be done in Acrobat Pro X? If so can someone point me in the right direction?

  • I want to create a form in IWeb can you help?

    I need to create a form page in Iweb on my website for visitors to complete and submit back to me.
    A Club membership application form.
    Anyone able to help??

    There are online form sites that you can use like JotForm, Wufoo, etc.  Jotform creates some very nice looking forms which you can embed into an iWeb page.  The free package gives you 100 submissions per month and 10 SSL secure submissions per month.  Over those numbers you will need to pay a monthly fee.
    Here are examples of Jotform and Wufoo forms: Embedding Forms
    Or you can create your own Form Action form which uses the visitor's own email client to send the information. This is not best integrated method to obtain the information.   An example of this type of form is also on the page linked to above.

  • How to create a form based on table using dynamic page?

    Hi,
    I need to create a form using dynamic page. How do you pass values from the html form to a oracle procedure that will get executed on submission of the form ? I could not find any documents which shows how to do that. Can anyone please help me out with an example ?
    thanks,
    Mainak

    Hi,
    Something seems to get added to the form action because of "http". Hence I am removing it.
    You need to write a procedure with the values in the as parameters. Say for example you want to insert a record into dept
    table then
    Dynamic page code
    <html>
    <body>
    <form action="portalschema.insert_dept">
    <input type="text" name="p_deptno">
    <input type="text" name="p_dname">
    <input type="submit" name="p_action" value="save">
    </form>
    </body>
    </html>
    Procedure code.
    create or replace procedure insert_dept
    (p_deptno in number,
    p_dname in varchar2,
    p_action in varchar2)
    is begin
    if p_action = 'save' then
    insert into scott.dept(deptno,dname) values(p_deptno,p_dname);
    commit;
    end if;
    end;
    grant execute on insert_dept to public;
    Hope this helps.
    Thanks,

  • How to create a Form based on a dynamic table?

    Hello,
    The Select statement below creates a table based on a string (string is a value of an item):
    select * from table (pkg_util.fn_get_table (:P18_VALUE))I need to create a region on a page with a Form based on this table.
    I was able to create a Report, but not a Form.
    I need to create a Form, which would return updated string to the page item.
    How can I solve this please?

    Hello Gentlemen,
    I have created a Tabular Form, based on APEX_ITEM API, as you suggested, here is the code below:
    SELECT apex_item.checkbox (30,
                               CATALOG_ID,
                               'onclick="highlight_row(this,' || ROWNUM || ')"',
                               NULL,
                               'f30_' || LPAD (ROWNUM, 4, '0')
                              ) delete_checkbox,
           CATALOG_ID,
              apex_item.hidden (31, CATALOG_ID)
           || apex_item.text (32,
                              LANG,
                              80,
                              100,
                              'style="width:100px"',
                              'f32_' || LPAD (ROWNUM, 4, '0')
           || apex_item.hidden (33, wwv_flow_item.md5 (LANG, DESCRIPTION)) LANG,
           apex_item.text (34,
                           DESCRIPTION,
                           80,
                           100,
                           'style="width:255px"',
                           'f34_' || LPAD (ROWNUM, 4, '0')
                          ) DESCRIPTION
      FROM V_SYSTEM_CATALOGS_PR
    UNION ALL
    SELECT     apex_item.checkbox
                              (30,
                               TO_NUMBER(9900 + LEVEL),
                               'onclick="highlight_row(this,' || ROWNUM || ')"',
                               NULL,
                               'f30_' || TO_NUMBER (9900 + LEVEL)
                              ) delete_checkbox,
               NULL,
                  apex_item.hidden (31, NULL)
               || apex_item.text (32,
                                  NULL,
                                  80,
                                  100,
                                  'style="width:100px"',
                                  'f32_' || TO_NUMBER (9900 + LEVEL)
               || apex_item.hidden (33, NULL) LANG,
               apex_item.text
                                               (34,
                                                NULL,
                                                80,
                                                100,
                                                'style="width:255px" '  ,
                                                'f34_'
                                                || TO_NUMBER (9900 + LEVEL)
                                               ) DESCRIPTION
          FROM DUAL
         WHERE :P180_TEMP = 'ADD_ROWS1'
    CONNECT BY LEVEL <= 1however, the update process doe not work on that form:
    DECLARE
      lc_string VARCHAR2(4000);
    BEGIN
      FOR i IN 1..APEX_APPLICATION.G_f*30*.COUNT
      LOOP
          lc_string := lc_string|| '[' ||APEX_APPLICATION.G_f*32*(i) || '|' || APEX_APPLICATION.G_f*34*(i) || ']';
      END LOOP;
      --Database processing using the concatenated string here
    END;Can you please see what's wrong with the code?
    Also, I tried to set a temp. item with the value, to see if the process returns something, like that:
    DECLARE
      lc_string VARCHAR2(4000);
    BEGIN
      FOR i IN 1..APEX_APPLICATION.G_f*30*.COUNT
      LOOP
          lc_string := lc_string|| '[' ||APEX_APPLICATION.G_f*32*(i) || '|' || APEX_APPLICATION.G_f*34*(i) || ']';
      END LOOP;
         :p18_temp := lc_string;
    END;and it did not work.
    Also, it is the second Tabular Form on this page. The first one is created using wizard, and it works perfect, with the same update process:
    DECLARE
      lc_string VARCHAR2(4000);
    BEGIN
      FOR i IN 1..APEX_APPLICATION.G_f*01*.COUNT
      LOOP
          lc_string := lc_string|| '[' ||APEX_APPLICATION.G_f*03*(i) || '|' || APEX_APPLICATION.G_f*04*(i) || ']';
      END LOOP;
      --Database processing using the concatenated string here
    END;Also, both forms are opening in a modal pop-up dialog window.
    I use a Dialog Region plug-in for that.
    May be this is causing a problem?
    But still, the first form works fine!?

  • How do you create a form that has user permissions per form field for different user?

    I need to create a form that is used by multi people but each user needs to fill out different fields within the form. I need the first user to make selections from a list and save it to send to another (the selection fields then need to be locked). Then the other user checks one of the selections made by the first user and saves the document to send back to the first user. Not sure if this is possible. I would like the first form field selections not to even show as a highlighted field to the other user (like just be more like a text field). I want the document to be able to be used more than once. The list selections will change and need to be sent to the other user at least once a month or more. Has anyone done this?

    There's no built-in way to do that, but it's probably doable using a custom-made script to hide and/or lock the fields.

  • How do I create a form in CS6 for Mac similar to a LiveCycle Designer form for Windows?

    I have been creating quite a few forms in LiveCycle Designer at work (on Windows 7). I do a lot of scripting (99% JavaScript) in the background in order to make the form function as I need it to. I purchased CS6 for my Mac at home, and have a side project where I need to create a form with a lot of the bells and whistles that my work-related forms have. Since I can't use my work laptop for personal gain, I was wondering if anyone can help get me started with how to create a "LiveCycle Designer"-like form using CS6 (LiveCycle Designer is not offered for a Mac).
    Thanks!

  • Creating a form to Add List Item and to a Document Library

    Hi,  I'm super new to sharepoint , and have what is probably a newbie question.
    I have a list and a document library.  The documents are linked to the items in the list. (1 list item may have one or moe documents associated with it).
    I need to create a form to add new list items that will at the same time upload documents to the document library.
    Do I use infopath for that?
    Thanks

    Hi,
    According to your description, when adding a new item to the current list, you might want to upload the attachments of this item to other Document Library as well.
    As a workaround, you can create an Event Receiver to capture the event when there is “An item was added”. In the Event Receiver, we can write C# code to copy the attachments
    from a list item to a Document Library.
    About how to create a simple Event Receiver in SharePoint 2013:
    https://msdn.microsoft.com/en-us/library/ff398052.aspx
    Here is the related code I use in this scenario, you can use it as a reference:
    using System;
    using System.Security.Permissions;
    using Microsoft.SharePoint;
    using Microsoft.SharePoint.Utilities;
    using Microsoft.SharePoint.Workflow;
    namespace SharePointProject13.EventReceiver1
    /// <summary>
    /// List Item Events
    /// </summary>
    public class EventReceiver1 : SPItemEventReceiver
    /// <summary>
    /// An item is being updated.
    /// </summary>
    public override void ItemUpdating(SPItemEventProperties properties)
    //base.ItemUpdating(properties);
    /// <summary>
    /// An item was added.
    /// </summary>
    public override void ItemAdded(SPItemEventProperties properties)
    // base.ItemAdded(properties);
    using (SPSite site = new SPSite("http://sp/"))
    SPList docDestination = site.RootWeb.Lists["Document"];
    SPList lstAttachment = site.RootWeb.Lists["List1"];
    SPFolder fldRoot = site.RootWeb.Folders[docDestination.Title];
    SPFileCollection flColl = null;
    if (properties.ListItem.Attachments != null && properties.ListItem.Attachments.Count > 0)
    foreach (String strName in properties.ListItem.Attachments)
    flColl = fldRoot.Files;
    SPListItem listtem = docDestination.Items.Add();
    SPFile FileCopy = properties.ListItem.ParentList.ParentWeb.GetFile(properties.ListItem.Attachments.UrlPrefix + strName);
    string destFile = flColl.Folder.Url + "/" + FileCopy.Name;
    byte[] fileData = FileCopy.OpenBinary();
    SPFile flAdded = flColl.Add(destFile, fileData, site.RootWeb.CurrentUser, site.RootWeb.CurrentUser, Convert.ToDateTime(properties.ListItem[SPBuiltInFieldId.Created]), Convert.ToDateTime(properties.ListItem[SPBuiltInFieldId.Modified]));
    SPListItem item = flAdded.Item;
    item[SPBuiltInFieldId.Created] = Convert.ToDateTime(properties.ListItem[SPBuiltInFieldId.Created]);
    item[SPBuiltInFieldId.Modified] = Convert.ToDateTime(properties.ListItem[SPBuiltInFieldId.Modified]);
    flAdded.Item.Update();
    How it works in my environment:
    Add an item with attachment in List1:
    In the Document Library, we can see the uploaded attachment there:

  • Need equivalent of Oracle Forms Row-Level LOV

    I came very close, but I am stuck on the last item.
    I am trying to build an update form. I have a select box at the top, and when I select a new value, I want it to change all of the values on the form (for update) to the record associted with the value I chose in the Select Box.
    I did this with the Master Detail wizard, although I believe that a "Form with a Report ", would work too. With the Master Detail Wizard, I choose "Edit detail on separate page", which creates the page that I am after, then I just delete the others. I then go to the last page (i.e., edit detail), and change the non-displayed primary key to a "Select List with Redirect" and then associate the desired LOV.
    It works pretty nicely one I select a value, and it performs the redirect. However; I can't seem to get the form to pre-populate upon startup.
    I tried defaulting the Select List, it does not pull in the detail data, unless I select something with the selct box.

    Just three last questions.
    1) If I want to call another similar form and have it start on the same item that I currently have selected in the select box, how would I pass that parameter, and how would I receive that parameter? I currently create a link on the first page, and use the following to call the second page:
    LinkName
    2) Is there an easier way to do what I am trying to do other than using a Master Detail Wizard, or Form with a Report. When I use the wizrd to create a single form, it seems to default to an Insert Form, and is missing the necessary function to do what I like.
    3) In my first two forms, a record will always exist before I navigate to it. However; I have another class of form, that may not have a record yet. Do I need to create this form differently, or will the approach I've taken above still work if I need the form to do an insert rather than an update into the associated detail table?
