Poor iTunes store in country of residence...can I change?

I am South African and live in Qatar and am on the Qatar istore. The Qatar store virtually deems any Apple products almost useless even though we have paid the same for the product as the available content is so poor. Can I open another iTunes account in a different country without having an account there?

Sorry but no. You can only use the iTunes Store of the country in which you are physically present and have a valid payment method.
As to the content available in your country, Apple can only sell what the content owners are willing to license for sale there. I'd suggest you contact the owners of the content you would like to purchase and urge them to make it available in the iTunes Store in Qatar. I see a lot of apps, music and films available in the Qatar iTunes Store, and you can of course import your own CDs, so I don't see how an iPhone et. al. could be classified as useless.
