Your account is locked out bcoz of too many wrong attempts.

Remote Desktop Gateway 2008 R2 - logon attempt failed
I've already read through a lot of threads regarding this. Our RDGW has been working for approx 2 years. Suddenly now, some clients start to get the "logon attempt failed" when they are using rdgw. It does seems to be an increasing problem..
- Redirection in IIS is OK, checked out!
- Blank page appears when i try to logon to http://rdgw.server.com/rpc - This is OK.
I see NO non-normal entries at all in event viewer on the gateway server.
The only thing I get in event viewer on the client is:
TerminalServices-ClientActiveXCore/Microsoft Windows-TerminalServices-RDPClient/Operational:
EventID: 1026 - RDP CLientActiveX is disconnected (reason= 50331649)
EventID: 1025 - Connection with multiple transport is disconnected(not correct - google translate from locale)
This is the only thing I can see in the logs, it pops right after I get the: "The logon attempt failed"
I think a certificate issue is excluded since most of my clients can connect - all certs er valid.
We got people externally and locally that are experiencing this issue (I've forced rdgw to be sure on the local clients) So most likely this problem has nothing to do with external/internal.
On those computers who are unable to logon using rdgw, none accounts works(i've even tried domain admin). So the problem is not user-based either.
Since the "the logon attempt failed" pops within a second I was'nt sure if the traffic even got to our RDGW, so I checked with wireshark, and I can see that the gw is responding in ssl back to the client. Still there is no entries in the log on the rdgw
server..
Any suggestions?

On the RD Session Host server, open Remote Desktop Session Host Configuration. To open Remote Desktop Session Host Configuration, click Start, point to Administrative
Tools, point to Remote Desktop Services, and then click Remote Desktop Session Host Configuration.
Under Connections, right-click the name of the connection, and then click Properties.
In the Properties dialog box for the connection, on the General tab, select the server authentication and encryption settings that are
appropriate for your environment, based on your security requirements and the level of security that your client computers can support.
In the Properties dialog
box for the connection, on the Log on Settings tab,
uncheck the box Always prompt for password

Lion server corrupted sudo: 3 incorrect password attempts
A couple of weeks ago I noticed that I was having a problem with sudo on my Lion Server, I went back as far as I could in Time Capsule and restored the entire OS, but the problem seems to have been there since the original install a few months ago. Sudo works fine on my MacBook Pro with Lion 10.7.4 Client.
I have been using sudo for years, and I've never had this happen.'

( 0 ) I can log out as system admin, and log in as root with no problems, but sudo at the Terminal always fails
( 1 ) I have turned off access to root, restarted, turned it back on umpteen times

( 4 ) looked at /etc/sudoers using visudo and it looks normal
( 5 ) I am the sys-admin for the server and $id shows that I am in the admin group
( 7 ) Server.app -> Menubar -> Tools -> Directory Utility -> Edit -> Disable Root User
( 8 ) Re-Boot
( 9 ) Server.app -> Menubar -> Tools -> Directory Utility -> Edit -> Enable Root User
( 10 ) Server.app -> Menubar -> Tools -> Directory Utility -> Select my shortname on the left scrolling area, delete Authentication Authority from right side display of my record, then save, quit, and re-boot.
( 11 ) Enter the requested old and new passwords when booting to restore the Authentication Authority
Any ideas other than the http://support.apple.com/kb/HT1528 notes on how to turn it on/off
Most everything I have done on the lion server has not required me to use sudo, but I use it frequently on osx lion client with no problems.
Something is either corrupted, or somebody forgot to turn it on ?
I hope I can get this fixed, I sure don't want to have to re-install the server software.

Open Terminal.app (Applications > Utilities) and paste the two lines to the Terminal. Hit ENTER and insert your admin password (it dond't show any output to the Terminal, so just type blind and hit ENTER. AND you also have to had set a password, it won't work with a blank password).
If the first command was successfull it would print "not loaded" and return to a new line, ending on $. Then enter the second command, and hit enter.

Incorrect password attempts enforced
Hi All,
I have activesync policy set on exchange 2007 to enforce 10 failed password attempts, however on my IOS devices, the policy enforced turns to 5 when exchange account is setup.
Any ideas?seems like a activesync problem, android phones are getting the same policy, my exchange admin refreshed something and the correct password attempts is shown now.

The message above says the following
REPAPI-26525 Session connection attempt failed for name (@name)
Cause: A connection could not be established to the
specified database using the provided connection string.
Action: Check that the user, password, connect string, names
services, network, and any remote site listener process are
properly installed and working.
The description of the messages can you find under the "Oracle
Lite" Program Group (if you have at least OLite 3.5 installed)
and take "Oracle Lite Message Help" entry (HTML)
It seams that you do not specifiy any password within your
StoreConnectionOpen command. Do it, take whatever you want (if
you have again 3.5 installed; 3.6 is checking the password if it
is correct [new feature in 3.6])

Password change fails in SQL Developer with verify function...
A couple of months ago I enforced a password verify function on our 11.2.0.3 databases and also one legacy 10.2.0.4 database.
At the time I tested on my account (which had elevated privileges...doh!). Now some users are hitting expiry, they can't change it via SQL Developer.
If I create a user with 'create session' privilege and set their profile to one that uses the verify function (see both below), I then log in to SQL Developer (we have tried with versions 3.1 (Windows) and 3.2 (Linux) with same failure results.
BTW,.. the password verify function enforces the following:
password must be minimum of 8 characters
password must not be the same as the user name, or user name (1-100)
password must contain at least a single digit
password must contain at least a single character
1. Works = I log into the local server and run command line SQLPlus, type 'password' and update. I can successfully change my password.
2. Fails = I log into the local server and run command line SQLPlus, type 'alter user <me> identified by <newpwd>;' I get:

ORA-28221: REPLACE not specified
This error is because the account does not have the 'alter user' privilege. I'm okay with this, as I don't want our users having this privilege.
3. I start SQL Developer 3.2, type 'alter user <me> identified by <newpwd>;' I get the same ORA-28221 error as above. That is fine, and as expected.
4. Now in SQL Developer, I type 'password', set a valid password, but I get 'Failed to change password' in the Script Output tab.
I have a database 'after servererror on database' trigger set, and querying the database table it is logging into, I see a record with a date stamp matching my failure with a server_error=28221 (the same as above).
So I'm wondering if I'm doing something wrong here, or if this is a bug in SQL Developer. I don't want standard users having 'alter user' privileges, but I do want to enforce password verification.
I get the same result on three 11.2.0.3 databases (haven't tried any more but suspect same results for others) and one legacy 10.2.0.4 database, and using SQL Developer 3.1 and 3.2.

User Password change fails in OWA 2013
User Password change fails in OWA with this error: Your password couldn't be changed. Make sure the old password you typed is correct and that the new password meets the minimum security requirements.
We are migrating from Exchange 2007 to Exchange 2013. Have mailboxes in both environments. OWA 2007 password changes succeed (user mailbox is still in Exchange 2007). When the user mailbox is moved to Exchange 2013, password changes fail
with the above error.
We have the Exch 2013 servers are on Windows 2012 and we are running Exch 2013 CU3. We have made changes to the Default Role Assignment Policy to prevent users from changing Contact information and setting user photos, etc. We are not exactly
sure when user password changes stopped working, or even if they ever did work, although we recently installed our Prod Exch 2013 servers alongside our 2007 servers without any RBAC delegation implemented and a quick test of a user password change was successful.
I reversed all the changes to the Default Role Assignment Policy but the password change still fails.Hi,
Please try the following steps in your CAS server:
1. Click Start > Run and type regedit and click OK.
2. Navigate to the "HKLM\SYSTEM\CurrentControlSet\Services\MSExchange OWA" key.
3. Set the ChangeExpiredPasswordEnabled value from 1 to 0.
4. Close regedit and re-open it.
5. Set the ChangeExpiredPasswordEnabled value from 0 to 1.
6. Close regedit.
7. After you configure this DWORD value, please reset IIS. The recommended method to reset IIS is to use IISReset /noforce from a command prompt.

One of the user is getting the below error when the person is trying to login ELM system. what could be issue for the specific user and how do we resolve this issue?
"Not able to login PIA - An error has occurred. You may attempt to sign in again. If your attempt fails, please contact your System Administrator"Its works for other users, its only happened for one specific user only? I didn't check the APPSRV.LOG file? what needs to be checked in APPSRV.LOG, I can ask my infra team to check, because I don't have app server access to check log file.

I was attempting to do a recovery with the discs I had bought from HP and I got this message "Recovery Attempt Failed" with options to restart recovery or view details. So I restarted the recovery and inserted disc 1 as it asked. It went through the process again and it came up with this same message "Recovery Attempt Failed". This time I clicked view details and it came up with notepad titled CTOERROR.flg with the following:

Prior to the Increase Capacity for Outbound Network Calls update, my Azure websites running in
Standard mode experienced occasional connection attempt failed errors. Since this update the errors stopped but starting on 12/30/14, these errors started up again on both
South Central and East regions:
"A connection attempt failed because the connected party did not properly respond after a period of time, or established connection failed because connected host has failed to respond [IP]:80"
It occurs when attempting to consume an external web service over HTTP. This issue is not reproducible on our local environments nor did it occur with our previous shared hosting provider.
On Azure, the same web service call can result in this error or will return a response successfully.
There doesn't appear to be any pattern as to why the same call would work one time but not another.

I am getting this error " Unable to create a new Message Layout : 500 attempts failed." when I am trying to add my custom region to seeded Message Layout.
Is that all the error message you are getting or java error stack is also there? It looks like a custom message as I couldn't find it in fnd messages. DO you have any other custom code on this page or is this a custom page?

i can copy and paste from Mozilla to word in second attempt only first attempt fails and paste a zero sized dot on the word document.more ever i can not drag fevicon from address bar to my desktop.

My daughter's ipod touch is locked due to too many incorrect password attempts (thanks to her little brother). It is displaying the red bar that states ipod is disabled connect to itunes. Problem is we don't have the computer that the ipod was originally synced with and she doesn't want to reset the ipod. In this instance we know the password.. so is there anyway to get the ipod to ask us one more time to enter the password? I'm not sure how many incorrect attempts were made, but it doesn't state anything about try again in so many minutes.. help, please!

Place the iOS device in Recovery Mode and then connect to your computer and restore via iTunes. The iPod will be erased.
iOS: Forgot passcode or device disabled
If recovery mode does not work try DFU mode.
