I tried the plugin using the instructions from the website

Line 85 and line 87 call the exact same JavaScript, the difference is that the JS in line 85 has been minimised.
Because the jQuery library (jquery.min.js) has to come before any plugin library, I suggest that you replace line 87 with line 85 as follows

How do I get the Questions to show in the 'View Responses' TAB, and then show up in the Summary Report?

There are more than one Applications folders. At the root level of your system is an Applications folder for all users, where most installations go. Every user, including you, also has a separat Applications folder.
Check all of them.

Columns' positions do not match in pdf form compared to the "View Response" tab in Form Central

I think I understand why this happened. And I am loookng for workarounds.
For example, let's say in the beginning you have five fields (A, B, C, D, E) in your form. Later on, you decided that you need to add two more fields says A1, and C1, and now the fields are as follow A, A1, B, C, C1, D, E. After you submitted the form to Adobe Form Central, you take a look at the "View Responses" tab, and you realize the order of the columns are as follow A, B, C, D, E, A1, C1.
I believe this happened because the new fields A1, and C1 were added later. But I am wondering what are the workarounds?

Yes, it does work in Acrobat Reader. I'm just trying to determine if this is a recent bug in Tiger (like maybe because of the 10.4.9 update) or if it's something related to the particular version of Acrobat the file was created in.
I haven't had to have Acrobat Reader on any of my Mac's since Jaguar.

Headings of columns in the responses tab

I can not see field names at the title of columns in the responses tab, the headings are empty. The same in Excel download, thi first row is empty. Why, and how can I fix the problem?

Open the form and select "View Responses" tab, on your lower right hand corner, you should see a "clock" icon, click on it, and that should bring up the history bar on the left side. You should be able to click on the any dot shown there to go back in history.
Please take a look at https://forums.adobe.com/docs/DOC-4071 , you should be able to see some screen shots of the history clock icon and the history bar. It is possible that there is only one version of the (new) form, and in this case, there is no history to go back.

Query on Responses Tab in Process tasks

I have a process definition for a resource object(GTC) which is already deployed, where there are some user defined responses added under responses tab of Process Tasks.
Like...
*Response Code: GCPROV.ADAPTER_FORMNOTFOUND
Response Desc: OIM Process Form not found in OIM
Status: R*
How to add these type of response codes and how they are triggered for example in above case?
How to create these type of OIM side responses?
I checked in error message definition, but didnt find these responses.

Well Responses actually let you do these type of decision making logic at OIM level. Say you would the expect the code to process a requesting user's ID to get evaluated as a tragte user's manager or not. Now your adapter does the processing and returns TRUE and FALSE based on this decision. So what you will do in OIM is, map the FALSE to R and description like Only manager can raise requests. For TRUE youcan map to C and response like Request is created by a manager.
And for the default response sin GTC GCPROV.ADAPTER_DATE_PARSE_ERROR,GCPROV.ADAPTER_PROCESSNOTFOUND. They are some code generated responses which do come with the GTC xml's.

Can i insert an image in the View responses tab for each formula ?

In the View responses tab, i would like to insert an image as a thumbnail for each formula to identify my products for a multiple products survey. Is it possible ?

View Responses tab = serial number

I would like a non repeating nine digit number to appear with each form and view that serial number in the table of the View Responses tab. The numbers do not have to appear for the customer to see. What are the chances? I cannot tell you how much I would appreciate a feature that I could toggle on and off. Each row of data from a form that might is submitted could have a field that populates with a 9 digit number which I envision as 000000001, 000000002...000000099, 000000100, 000000102 and so on. I would want the each form row to already. From the point of view of tracking forms having each form submission assigned a serial or tracking number of some kind would be priceless. I understand that right now Adobe FormsCentral does not offer the ability to serialize each form, but if only Adobe did offer a possible solution. Could it be as simple as a back end MySQL database that does nothing but source a vast array of sequential numbers. A random non repeating number generator would not be as helpful. I like the simplicity of the Adobe FormsCentral forms. They are lacking this simple feature that would make the forms we create so much more powerful and useful. I would love Adobe just that much more.

How to save to PDF form View Responses tab without including blank fields?

Is there a way to save in PDF form from View Responses withouth including blank fields? More specifically, my form has several fields that have action (Show/hide) depending on the values. Once form is submitted and I am reviewing it from View Response tab, I use "save as PDF" button to save/download my completed form, however when I open it, it shows blank fields as well. Is there a way to somehow specify not to include blanks. If there is no way it would be a nice feature to have. Please let me know.

No, the PDF is always going to show all fields, the "Show/Hide" does not affect the PDF we generate.
I will soon post a Form for users to submit feature requests, keep your eyes out for an announcement in the forums for it and you can submit this request.

Motu Ethnic instrument has some pretty good ones too.
Make sure you use a controller for volume as the
authenticity of an Accordion sound is as much in it's
extremes of mid note dynamics as the sample itself.
But the option I would advise you would be to get a
real one! You can pick up a usable accordion for
about $100 and there dead easy to play (provided you
just use the right hand) when purchasing check that
the bladder has no leaks by drawing in or pushing out
air without pressing a note (i.e. you shouldn't be
able to). You'll have to pay a bit more for a MIDI
accordion though (sorry bad joke).
Stick a condensor or even a dynamic (if you don't
want key noise) in front of the sound holes (say half
a foot away) and I guarantee you'll beat anything you
have to try and program even if you're inexperienced.
The problem with that advice Christian, respectfully, is that there are accordions and there are accordions.
A regular accordion sounds very different from a musette, concertina, and as I recently discovered on a film I scored last year a bayon.
So if you need a variety of them that is not a solution.

Accordion Interaction: Additional Tabs

I would like to use the Accordion interaction or the Tab interaction. But I need two more tabs. I have been able to add one. How can more tabs be added?

There is a minimum and a maximum number for most interaction buttons. In case of both the Accordion and the Tabs minimum is 2 and maximum is 5. You cannot got beyond them.

I know the response columns can be rearranged in the report but can the order be "pre-ordained" as part of form design?

Sorry I am not aware of your form NOT created in FormsCentral.
FormsCentral doesn't support to rearrange the column order in Form Design tab, if it is an imported PDF form. The column order in PDF form depends on the order of the field in the PDF structure. You can post a feature request from http://forums.adobe.com/community/formscentral?view=idea

How come I cannot see the meeting response tab at the bottom in Calendar

Hi, my company uses Microsoft Exchange 2007. Let say someone invited me to an event. I accepted the event either on the phone or Outlook. In Outlook, it clearly showed that I had accepted the event, and also provided me with the options to change my response (like tentative or deny). When I clicked on the same event on my iPhone, it didn't give me those options (accept, deny, tentative). It gave me the option to delete the event, as if I was the event organizer. I checked the same event on my iPad 2 running iOS 5.0.1 and the same thing happened.
On my Android device, I was able to change my response to the meeting just like how it was supposed to be. Could this be a bug with the iOS Mail application?
I think it might have something to do with changing the initial event date or time and sending updates. Somewhere along the way, it's not syncing up correctly.
