Seq Header Duplication

Hi all have come across this error, and believe I have successfully fixed this problem after many hrs of trying every possibility.
What I have found that fixed this problem for me was,
Remove all spaces or replace with underscore in naming of movie files before placing in DVD Pro session.
example the files where:
SD Def prog dv pal.mov
Rename as:
SDDefprogdvpal.mov
(or underscore)
SDDef_prog_dvpal.mov
Issues I thought it was:
naming was to long, to many characters,
one of the names of video was:
Std def look at Apple Pro Res 422.mov
After closing gaps to:
StddeflookatAppleProRes422.mov
The build was successful.
9mbs per sec, one pass works fine.
The DVD pro session was a CODEC test
so it was a mixture of files exported from compressor in standard DEF 16:9,
DVPAL progressive
DVPAL interlaced
AppleProRes422
8bit_uncompressed
DVDstddef.m2v (one pass 9mbps)
So please post back if this fixes your problem,
or re-create the error with a new session DVD Pro
and wording of movie/video file has more than 3 gaps
"SD Def prog dv pal.mov"
Also for every new DVD Pro session setup a new encoding folder, and build folder can be the same.

Sounds like you did the trouble shooting you should, make sure to delete MPEG/PAR files then re-encode. Also perhaps delete prefs/repair permissions
