Survey:Scoring Choices

Hi,
I have been trying to figure out how to create and get the total final score for a simple survey form with 35 statements, each having a choice of 5 responses, without success.
Each question has 5 radio buttons that are grouped together (exclusionary group?). The survey taker is asked which response most closely describes how the statement applies to them: the "always" response is scored 4, the "often" response is scored 3, "sometimes" is 2, the "rarely is 1 and the "never" is 0. As a radio button group only one of the five available choices can finally be chosen. I've used the binding tab to assign the correct value to each of the 5 radio buttons in the group.
This is where I get lost: I think I would need to write some JavaScript (prefer JS to FormCalc, since I've used JS before in other apps) that would say something to the effect: if buttongroup1button1 is clicked, put 4 in a variable, else if button2... But the event to use, plus the correct JS syntax is beyond my beginner skills.
Maybe there is even a simpler way. It I can figure out the script for one button group, then I can replicate it for the other 34 statements/buttongroups. Then hopefully a small script added to a "submit" or "total" button to add up all the variables holding the 35 values, to get and display a grand total.
I am attaching a sample of the Designer form.
Any suggestions, examples to get started would be much appreciated.

I am trying to add a feature so that if the user fails to complete one or more checkbox groups (leaves them blank or empty), then instead of the total being calculated, they would see a reminder to "please complete all choices"
I have been trying to script this in FormCalc but getting error messages. I can't even figure out how to get the text box (initially invisible) to show if the first checkbox group (named "q1") is empty:

    Then go to the NewForm.aspx or EditForm.aspx of your list , insert a CEWP below the ListFormwebPart. In the <content></content> tag of your CEWP add the following script:

    As I mentioned this is not the best way since you have to add multiple label ID's corresponding to every "Specify your own value" label that you want to change....

  • Captivate 5 Survey Result Question

    We are trying to create a Survey in Captivate 5 which will not report out to LMS.  I am trying to find a way to get the Survey results (2 Question per slide 18 questions total) to display to the learners.  For instance, Slide 1 is A) B) question, slide 2 is 1) 2) question and then starts back with A) B) on Slide 3.  I need a total of A) B) 1) 2) for the learners.  We are using this in place of a handout if possible.  I would assign 1 point for each question and then have them tally at the end for the learners.
    Let me know if anyone has any thoughts.  I have searched the forum and did not find anything similar.  I appoligize in advance if this is answered somewhere else.

    I can see the confusion.  We are not scoring in the sense that this is pass/fail.  What we want to do is have the answers tally for the learner.  The goal is that if you received 5 - A)'s, 3 - B)'s, 2 - 1)'s and 1- 2)'s those results would display.  So if I have a questions such as below:
    1. Please pick which best describes you:
    A) Easy to work with
    B) Difficult to work with
    2. Please pick which best describes you:
    1) Task oriented
    2) Free thinker
    The answers selected would then tally up at the end for the learner.  The questions repeat with different choice selections between A) B) and 1) 2).
    Does that help explain what I am look at doing?

  • How To Create a Personality Survey

    I would like to create a personality self-assessment survey
    using the Survey quiz feature of Captivate. I can set the survey
    questions up with a 1-5 scale from Disagree to Agree for each of
    the questions. I'd like this survey to show the survey taker the
    results of his or her choices as a total on the results slide so
    that they could then be interpreted--e.g., "If you scored from
    10-20 you are likely to be introverted and withdrawn," etc. No
    matter what I do, however, the results slide always shows a score
    of 0.
    I know Likert surveys are not graded; I just want Captivate
    to tally up the question responses according to the 1-5 scale and
    report that total on the results slide to create a kind of self
    assessment. Any suggestions would be greatly appreciated.

  • I need help writing a dive scoring program in C#, Help Please?

    The state diving commission wants to computerize the scoring at its diving competitions. I have to write a program to automate the scoring of dives. Requirements:
    After each dive, the user will be prompted to enter the:
    diver's name;                    
    diver's city;                    
    degree of difficulty (ranges from 1.00 to 1.67); and                     
    scores from five judges (scores can range from 0 to 10).
    If an invalid score is entered, an error message will be displayed. The user will be prompted for the score repeatedly until a valid score is entered.
    The program will then display the following information:
    Diver's name                    
    Diver's city                    
    Dive final score: This is calculated by dropping the highest and lowest of the five judges' scores. The remaining three scores are added together, and the result is divided by 3 and then multiplied by the degree of difficulty.
    The program will then prompt the user if she/he wants to process another dive. The user can type "Y" or "y" to continue, and "N" or "n" to quit.

  • Survey Export to Excel - Not all columns are exported

    I have Survey in SharePoint 2013 with questions expecting different answers - radio button options / Ratings / Multi line text, etc. Settings choosen - Read thier own responses, create and edit their own responses
    I have full control over the site where the Survey runs.
    When I choose to export the results to excel (Office 2010), I notice only ratings questions and answers are exported.
     1. Question with Choice answers are not exported
     2. Questions with multi line answers are not exported
    Do anyone face this issue?

    I found the problem area. I had marked "All Response" as the default view. In this view, for some reason does not export response for all questions.
    Once I changed the default to Overview, I am able to export all the responses for all questions.

  • Create survey/poll with text fields

    I want to create a survey in a meeting with text field type answers.  How would I do this?  I am using the hosted version of Connect and saw some reference to a Presenter add-in for PowerPoint, but don't seem have that.  We would like to have fields such as:
    Participant information (name, contact information
    Suggested future topics
    What should I use to do this since the Poll Pod obnly allows for multiple choice or multiple answer options and not key-in fields?

    Adobe Presenter 7 is a product sold separately from Adobe Connect. Adobe Presenter can be purchased from many resellers.  It is a plug-in for MS Powerpoint and has the ability to upload many times of quizzes and polls in lots of types of formats.  You can download a 30 day trial from here:

    Adobe Presenter is only available for the Windows platform. MS Powerpoint for Mac does not allow third-party plug-ins like Presenter.

  • Need help with PC build for use with Photoshop CS4 Extended - Have I made the right choices?

    I'm very new to the inner workings of a PC...this will be first time building one from scratch.  I know enough to be dangerous but not enough to be confident.  I will use the PC primarily for editing, secondarily for iTunes management.  I've heard that I should have, at the very least, an SSD for PS, other apps and the O/S.  I'll also have a hard drive for data storage.  Do I need yet another SSD for scratch?  If so, any recs for one?  Also, my current build (which does not include any SSDs) is about $800.  What can I skinny down, without sacrificing a huge amount of speed/performance?  I am looking for major guidance here...I'm ready to order, but I keep second-guessing my choices.  I want to make sure I get this right the first time around.  I want this machine to last me a good long while.  Any help at all will be HUGELY appreciated!

    Make sure the motherboard is the third revision as the earlier two had problems with the i5-2500K (Sandy Bridge)
    I prefer Asus motherboards but doesn't mean the Gigabyte is a problem. (I note that it scored 4 eggs...)
    May not make a difference in Photoshop but for Premiere the Geforce video cards with GDDR5 are preferred.
    The LiteOn iHas 224 does lightscribe which is a nice touch for clients/gifts.
    There is no room to save as I would consider your build minimum specs for a nice photoshop experience.
    As for SSD drives...my research of reviews and articles makes me feel the technology is not ready for mission critical work. So if you are running a business then I recommend not using SSD drives. Rather look at Raptor 10,000 rpm drive for Windows and Photoshop and use the Spinpoint for storage. If not business then I've read the intel x25 SSD drives are solid but I have no personal experience with them.

    You are more likely to get this to work in Numbers because it can connect between tables and has things like check boxes for multiple choice.
    If you are intending to use this information seriously I recommend you use a relational database to get a much better handle on your work and also to ultimately tie everything together, project details, hours, billing etc.

    My form asked survey takers to rank a list of choices from 1 - 5 as their favorite and least favorite respectively.
    I would like my summary report to show how many ranked a choice as #1, marked a choice as #2 and so on. Is there a way to do that?

  • Creating a survey in acrobat, processing on hard copy, and data compiling via Acrobat scanning

    Hi there,
    I am new to creating PDF forms using Acrobat Pro 9. I have been asked by my manager to sort out a way we can create a survey (for staff)and have the data input to the survey, processed and compiled ready for an analyst to create a report of the data.
    Basically, this is a survey of staff by staff, anonymous and to be completed on hard copy (Printed PDF with survey template).
    So, I have started to think about the best way to go about this, can I create a survey in Acrobat, print it, fill out the multiple choice questions, scan it in Adobe, and allow acrobat to gather the data?
    Personally I would like to use Acrobat (as I am enjoying learning it), but is this the right tool for that job? Is there a feature that enables multiple choice surveys to be created, printed, filled out, scanned again (with answers filled in), and compiles the data?
    I hope I have explained that clear enough...;-)

    By itself Acrobat does not "compile data".
    By the way nothing compiles data from a picture. All scanners output a picture of the scanned paper (that is, an image file such as TIFF).
    Acrobat can perform Optical Character Recognition (OCR) of the picture of text.
    The OCR output is the characters - no format, tables, rows, columns, no text styling, etc.
    So harvesting useful information can pose something of a challenge.
    You may want to consider manually compiling the results.

    How can I make a survey automatically identify the user in the results (so that it won't be anonymous)?

    I am not sure what information to provide.  Here is what I am trying to do.  I am trying to create an HR questionnaire with multiple choice questions and several multiline textboxes to capture comments.  I want to send it out by email address and/or embed the questionnaire somewhere on the company's intranet website.  But we don't want the questionnaire to be anonymous. If someone makes a particular comment, we want to be able to communicate with the person who made that particular comment.  How would we be able to trace the person? By email address, or by user ID?

    Hi everyone. I am trying to create a survey in forms central and am having difficulty with one function. I would like for their to be a list and for respondents to be able to prioritize which items they would like in a future knowledge management system. For example, the question would read "please prioritize the fruit you would like for a snack" apple, banana, orange, pineapple, berries. Is there a way to do this?? I have tried to create this with a rating scale but the program would essentially allow every single option to be #1 priority. Any help is appreciated!!

    What if you used a series of drop-down lists or single-choice lists?
    For example:
    What is your preferred fruit for a snack:
         Banana
         Apple
         Blueberries
    What is your second preferred fruit for a snack:
         Banana
         Apple
         Blueberries
    It doesn't exactly meet your need, but it might help.
    There *is* a way to do this, but it involves a large number of show/hide rules. For example:
    What is your preferred fruit for a snack?
         Banana
         Apple
         Blueberries
    You would also build the following questions, with the rules to only show them if appropriate:
    What is your next preferred fruit snack? (show if Banana was chosen for question 1)
         Apple
         Blueberries
    What is your next preferred fruit snack? (show if Apple was chosen for question 1)
         Banana
         Blueberries
    What is your next preferred fruit snack? (show if Blueberries was chosen for question 1)
         Apple
         Banana
    ... and so on.
    Cumbersome to set up... but it works.
