Weird response from my feedback form

recently made a flash movie with 2 feedback forms in it. One
form works perfectly but the other much larger one comes back like
html code. I don't know what i'm doing wrong. Here is what i use in
the movie clip
on (release) {
this.loadVariables("contact.php", "POST");
this is the php code that i used
<?php
$sendTo = "[email protected]";
$subject = "Booking from Mysite.com";
$headers .= "Reply-To: " . $_POST["text13"] . "\r\n";
$message .= "\r\nFrom: " . $_POST["text13"] . "\r\n";
$message .= "First Name: " . $_POST["text6"] . "\r\n";
$message .= "Last Name: " . $_POST["text7"] . "\r\n";
$message .= "Address: " . $_POST["text8"] . "\r\n";
$message .= "City: " . $_POST["text9"] . "\r\n";
$message .= "State: " . $_POST["text10"] . "\r\n";
$message .= "Zip: " . $_POST["text11"] . "\r\n";
$message .= "Phone: " . $_POST["text12"] . "\r\n";
$message .= "Email: " . $_POST["text13"] . "\r\n";
$message .= "Pickup Address: " . $_POST["text14"] . "\r\n";
$message .= "Destination Address: " . $_POST["text15"] .
"\r\n";
$message .= "Limousine Desired: " . $_POST["text19"] .
"\r\n";
$message .= "Time of Pickup: " . $_POST["text16"] . "\r\n";
$message .= "Number of hours: " . $_POST["text17"] . "\r\n";
$message .= "Date Desired: " . $_POST["text18"] . "\r\n";
mail($sendTo, $subject, $message, $headers);
?>
It's coming out like this.in the emails.

the information is in all that code. How can i fix it so it
doesn't return the coding with the information?

you have html enabled textfields and you're sending the
htmlText property. just send the text property.

  • Allowing only one response from each emailed form.

    I am interested in creating a survey/voting scenario and allowing people to submit their responses including their vote.  My issue is that as the program stands now a person could actually fill out their form multiple times and submit multiple votes.  Is there a way to limit the responses from each emailed form to one?
    Thanks

    This was answered here: http://forums.adobe.com/thread/1109120?tstart=0

  • Collecting intact responses from a fillable form

    How can i distribute the fillable form through a company website but receive submitted responses to an email address?  I need the completed questionnaire to remain intact.  FormsCentral sorts the form's responses which doesn't work for our purposes.

    You can set up a button with a "Submit a form" action and use a mailto type URL to specify the email address you want the forms sent do. Once you have the form all set up, you need to Reader-enable it as the final step. This thread shows how with several versions of Acrobat. You then have to send the form to your users or make it available on a web site.

  • Responses from Distributed Forms

    Is there a way to send me an email notification once I receive a submitted response from a distributed form
    using Acrobat?

    Is the form being returned as a pdf or xml?  You can distribute the form, which will set up a pdf portfolio for your returned forms.  When forms are returned via email, you double click on the attachment and it puts into the portfolio.  From the portfolio you can export one or more forms (data) into an excel file.
    To export the whole form, click on File, Export and choose the file type you want.  However, if you just want the form data, I would suggest you distribute the form to create the pdf portfolio for that.

  • Responses going into wrong form

    I created a form, and then copied it and renamed it.  The original form is for vendors to view and play with, for a proposal process.  The 2nd form is for people to actually respond to.  I advertsie the 2nd form on a website, and all the links are correct. However, I am getting responses from the 2nd form dumped into the first form. I don't want to close the first form, because i want it to be viewed by vendors submitting proposals. I have done this with several other forms and I'm concernd this will continue to happen. Both forms have distinct URLs, so I'm not sure why this is happening.
    Help!!!

    Thanks.
    This is the first form, that is more of a test form, and responses are coming into here that shouldn't be coming - responses 1 through 3 should be in the 2nd form, the 4th response is a test response:  https://adobeformscentral.com/?f=A0RbyH00%2AMzZbcG5l1gfsA
    This is the 2nd form that responses should be coming into: https://adobeformscentral.com/?f=6qiuxTHSQSDl-tFtiVRbpA
    And I added you as coauthor.
    Thanks, Michelle

  • Format of response from distributed form

    Hi guys,
    I have been working with a trial of Acrobat X Pro and Livecycle ES II, to determine if we can use it effectively to replace paper forms in our workplace. We have also recently deployed Alfresco, an open source CMS, which is very tightly integrated with what we would like to do with Adobe forms. I am a little confused though, at trying to figure out what controls the format of the response.
    With the forms I create in liveCycle, when distributed, the response seems to be either a pdf or an xml file. The first few forms I designed arrived on the server as pdf's when I tested them (which is exactly what I wanted). The last couple I have created (using liveCycle) however, seem to always arrive as xml. I have gone through every page of the Distribute... wizard with what I feel is rather a fine toothed comb but cannot find anywhere to control this.
    Today I have discovered that Alfresco is unable to parse the form fields in the liveCycle PDF's (this is to do with the Tika parser and PDFBox, but isn't really related to this disccussion directly), so I have switched to Acrobat which, in my testing so far, does create forms parsable by Tika. When I distribute the form from Acrobat though, the responses always seem to be in xfdf format (which seems to boil down to xml anyway).
    I am sure that actual PDF's were created at some point. I still have test distribution folders full of pdf responses. Does anyone know what I am missing which controls the response??

    Hi Bill,
    I know the data format would be alot more useful if we were just needing to get the data into a database or some other process or application, but the need here is moreso for the actual PDF. This is also required to be an automated process, so importing the data files into acrobat manually is not an option.
    When you say are we collecting data by email or web script, I am not 100% sure I understand. When we choose distribute form... there are three choices - collect responses on Acrobat.com, collect responses in my email inbox, and collect choices in our own server. We choose the latter of the three (own server) and specify the CIFS path to a folder on the Alfresco server.
    Essentially all I need here is to find out which setting allows me to have pdf's (not xml or fdf or xfdf) collected in the responses folder when the form is distributed using the "own server" option. When I go through the distribute wizard, the only options I set seem to be: How the data is collected (own server), the path to the server, whether or not to collect name and email and the server profile name. (although now I think of it, I believe that originally I was leaving the "collect name and email" checkbox ticked originally when I was seeing actual PDF's collected, so I may try that now. If that's the setting controlling the response format though I would be surprised and find it incredibly unintuitive).

  • Read responses from a form

    How do I read responses from a completed PDF form?
    I used adobe acrobat pro extended 9 to create a form. Was confused at the end as all the methods for publishing the form and collecting responses were more involved than I wanted.  I selected the email option.
    As a test I made a copy of the blank form.  Then completed it, saved the resulting document.  How do I read the responses from a completed PDF form document?
    I want to manually distrubute the PDF form document. Then manually receive the completed form document back. ( copy it to an "in box" folder ). Then use a program of some sorts ( the adobe SDK? ) to automatically pull the response data out of the completed form.
    thanks,

    Thanks a lot.   I understand better now. I opened the completed PDF with acrobat and the response data was automatically associated with the original document.
    Very nice.

  • HOW TO REMOVE RESPONSE NUMBERS FROM A DUPLICATE FORM ONLY

    HOW CAN I REMOVE THE TOTAL NUMBERS ON A FORM THAT I DUPLICATED. WHEN I REQUESTED TO DUPLICATE THE FORM AND BOX CAME UP THAT ASKED IF I WANTED THE FORM ONLY OR THE FORM WITH THE RESPONSES AND I SELECTED FORM ONLY. I OPENED THE SURVEY AND DID A LIFE TEST TO SEE IF ONCE I SUBMITTED IT WOULD ERASE THE TOTAL OF THE PREVIOUS WHICH WAS 198, BUT IT ADDED 1....HELP!

    Can you confirm that there is only one response showing in the View Responses Tab?

  • CNN Feedback Form Does Not Load

    Several weird things with CNN site.
    First, when I was on it around 1:45 pm or so I typed in a term into its search box, and kept getting in the message box in my lower left corner screen that it was transmitting data from something like falsemail[dot]net, and so I finally clicked on the "x" in the address bar to stop the transmission, and then the site loaded properly.
    However, when I tried to report this apparent error to CNN through its feedback form for technical issues I couldn't get pull up the form, had trouble backpaging and, once I was able to get out of the site, the URL listed from the front page button was this odd wyciwyg://13/http://www[dot]cnn[dot]com/ What is that?
    Finally, when I tried to access CNN contact us as a link through a google search there was huge latency, and unable to reach its page.
    I cleared my browser, cookies and cache but can't seem to pull up the CNN feedback form to report its technical glitches.
    Is this a Firefox browser issue or CNN site issue?
    Thanks for any insight folks may have.

    Thanks for such a quick response.
    The link you provided does work for me.
    The one that doesn't work for me is here: http://www.cnn.com/feedback/#cnn_FBKErrors
    When I select "technical" from the drop down box for the Error category it all seems to go haywire.

  • Help with Dreamweaver CS4 feedback form

    I am trying to get my first feedback form working properly.
    There are two problems:
    1. I can't click in the boxes to enter info. The only way to access them is to tab through.
    2. When I submit, it brings up a second blank feedback page in Safari. In Firefox, it just loops back to the empty feedback page.
    Any help would be appreciated as I running out of hair to pull out.

  • Feedback form script in RH8

    My form (see below) triggers a PHP script (see below) which is supposed to send the form data to my email address. It works perfectly in a regular web page, but when I've integrated into my WebHelp project the form fails to send anything and show a series of PHP errors (see below). I'm no PHP expert so I'd be grateful if anyone could advise me or suggest an alternative method of producing a Feedback Form in a WebHelp page.
    FORM:
    <form action="sendmail.php" method="post" name="Feedback" id="Feedback">

    Hi Guys, just got round to looking at this again.
    The form now sends the email, but my "thank you" page isn't displayed. Also, if the required fields are not completed, my "error" page isn't displayed either.
    I'm testing this form after it's been published to our web server, which is on the same server as our intranet and the forms on the intranet work fine using the same script. I'm convinced that RoboHelp is doing something weird with the script.
    Any further help would be much appreciated.
    Thanks
    Jonathan

  • JSP/Servlet Feedback form confusion

    Hi,
    I hope someone can advise me. I have a jsp which collects feedback information from a form. I then want to call a java servlet to read this information, store it in a bean and send an email. At the moment when I click submit in the jsp form, the servlet isnt even being called.....Is it totally necessary for this kind of scenario that my servlet contain doget and dopost methods?
    Many thanks.(code below)

    Is it totally necessary for this kind of scenario that my servlet contain
    doget and dopost methods?Yes.
    Those are the methods that the server will call to invoke your servlet.
    Also your methods should not be static. It needs to create an instance of the servlet to invoke it.
    You could easily implement your doGet/doPost methods by just calling main(request, response) from them.

  • HTML Snippet for FEEDBACK FORM does not work the way I expected

    Hello folks,
    I have used following code as a Snippet to create a FEEDBACK FORM, so people can send their comments. I was hoping that when people write their comments and click Send, the comment will be sent directly to my e-mail. Instead, after clicking Send button, the Mail Manager pops up. So it is pretty much the same as if they click on the e-mail address link. Is this normal? I thought, that these comments will be sent directly via that FEEDBACK FORM, not via Mail Manager.
    Thank you so much for any tip or response.
    This is the Snippet Code:
    <!DOCTYPE HTML PUBLIC "-//W3C//DTD HTML 4.01 TRANSITIONAL//EN">
    <html>
    <head>
    <title>form</title>
    </head>
    <body>
    <FORM METHOD="POST" ACTION="mailto:[email protected]?subject=Please email me with your feedback"ENCTYPE="text/plain">
    Please enter your name
    <INPUT TYPE="text" NAME="name" SIZE="30">
    Please enter your email address
    <INPUT TYPE="text" Name="usermail" size="50">
    Add any comments below
    <TEXTAREA NAME="comment" ROWS=6 COLS=60></TEXTAREA>
    Clicking "Send it" will launch your mail application
    <INPUT TYPE=submit VALUE="Send it!">
    <INPUT TYPE=reset VALUE="Reset Form">
    </FORM>
    </body>
    </html>

    The action is usually a php file.
    Otherwise, simply create a button and make it a mail link in the Inspector.
    If you cannot figure this out, you'd better refrain from using it.
    Or start learning how to use forms.

  • Upload image to directory and data to database from the same form

    Hello all
    I'm playing at building a shopping cart (never done one
    before) from a tutorial by Gordon Knapp at www.webthang.com.
    I have created the site and it works perfectly. Now I want to
    extend it.
    There is a form that allows the user to add data to the
    database. I would also like to be able to upload an image to a
    directory on the server and add the data to the database at the
    same time from the same form. It would be good if the input field
    "<input name="product_picture" type="text" id="product_picture"
    tabindex="6" size="50" />" could be populated automatically by
    the filename of the image being uploaded, but this isn't a
    neccessity.
    I have looked at various ASP Upload packages and I can upload
    an image alone but none of them make it clear how to upload the
    image and add data to the database at the same time.
    As this is a private project I need any suggestions to by
    free.
    The tutorial shows how to create the site in Ultra Dev but
    worked perfectly well in DW. The page concerned looks like this. It
    is named 'admin_add.asp':
    <%@LANGUAGE="VBSCRIPT" CODEPAGE="65001"%>
    <!--#include file="../Connections/con_ecom.asp" -->

    I want to send out a questionnaire to 150+ individuals, then compile responses from all parties into one spreadsheet. The form uses drop down options and text fields.

    You can compile the data from multiple PDF forms into a single spreadsheet in Acrobat, if that's what you're asking. I can't say for sure where that command is in Acrobat 9, but it should be somewhere under Document or Advanced...
