Audio Extension failing to load
Occassionaly on starting up, I get a "System Error Warning", explaining an 'Audio Extension - Address Error'. Although this is happily rectifiable with a paper clip restart in the side of my iMac, how do I put things right, before things gets worse?
I know I really ought to understand how to do this by now, but the whole Extensions malarky never made much sense to me. In my defence I have tried reading up about it, but alas, to no avail... :o\

You probably have some sort of "audio" extension conflict.
Except for the keyboard & the mouse-disconnect all devices from your computer.
Restart your computer while holding down the shift key.
This will shut off your extensions.
If you make it to your desktop w/o the error, this means you have an extension conflict and/or USB conflict.
The following further explains the "Address Error" in a little more detail.
Address (Type 2) Errors
Probable Causes: Address errors occur when a segment of memory is inaccessible -- either because it is reserved for another use or it doesn't exist. Likely caused by an extension conflict, insufficient memory, or corruption in an application or an application's support file (e.g., a preferences file).
Other causes could include faulty application code, a defective SCSI connection, or a bad logic board. Because these causes are less likely causes, you should for them only after troubleshooting for the most common causes.
Possible Solutions: When this error occurs in only one application, try running the application with extensions disabled. If the error does not recur, troubleshoot for extension conflicts.

If the error occurs in only one application and while extensions are disabled, try trashing the application's preferences file so the application creates a new one. If the error does not recur, a damaged preferences file was the cause.
Preference files are located inside the Preferences Folder which is inside the System Folder.
You should also try allocating more memory (RAM) to the application.

If the error occurs in more than one application, the cause may be a problem with either the system software or, perhaps, the hardware. Reinstalling your system software will likely resolve errors caused by damaged system software. If the error is caused by hardware, SCSI devices attached to your Macintosh may be the cause and you should troubleshoot the SCSI chain.

It's corrupt. Delete and redownload it if doing so is free in your country.

Soundbooth CS3: Audio driver fails to load on MacBook
On both my MacBooks (white and one-piece aluminium), I get the following alert when I launch Soundbooth CS3:
The audio driver has either failed to load or the device is no longer
connected. Please re-load the driver or connect the device and try again.
[ OK ]
I am also unable to record any sound. If I click OK in the above alert, then click on the red record button at the bottom of the window, I get a dialog which allows me to choose input:
* System Default Input
* Built-in input
* Built-in microphone
Regardless of which of these I select, the red record button has no effect.
I do not have this problem on an older iMac.
A search for the key words in the alert on Adobe's support system comes up blank.
Has anyone else encountered this, and has anyone been able to fix it?
Thanks in advance.I have not done any recording or messed with an input. I have only done editing of existing sound files. I tried holding shift at startup as well as deleting the library 1.0 folder. Still getting the prompt when the app starts. Then it crashes. Here is the report that is generated:

Why does this applet (with an audio clip) fail to load in a browser?
Hello Java Experts.
I am able to compile and run the applet in an IDE. The problem lies when I attempt to run the applet through a browser (such as Internet Explorer, Firefox, etc.). The entire applet fails to load (in the browser) when it includes code pertaining to audio clips, but works fine when it does not contain audio clip related code.
Here is a Short, Self Contained, Compilable, Example that illustrates my problem:

1) Configuring the Java Console to open for applets by default - look in it for error messages.
2) Change the name of the WAV to something that definitely +does not exist+ at the code base. (That would most closely replicate my own testing experience - since there is no WAV available here). -
Audio Extension sometimes fails to load
Occassionaly on starting up, I get a "System Error Warning", explaining that the 'Audio Extension has Failed to Load'. Although this is happily rectifiable with a paper clip restart in the side of the iMac, how do I put things right before it gets worse?
I know I really ought to understand how to do this by now, but the whole Extensions malarky never made much sense to me. In my defence I have tried reading up about it but alas, to no avail.
iMac 233 Mac OS 9.1.xFirst, do a backup of your important user data (stuff that cannot be "reinstalled"), just in case. I realize this may be difficult on the original iMacs, since it has no writable media drive, but I hope you have some method (such as an external drive) because doing backups is very important.
Then, try restarting from your Mac OS 9 installation CD and running Disk First Aid from that CD. If it reports problems it cannot fix, you may need a third party utility such as Norton, Tech Tool, or Disk Warrior.
If you just want to get rid of that occasional error message, you can try disabling the named extension. I believe it is called "Apple Audio Extension." Run the Extensions Manager control panel and you should see it listed. I use a program called Conflict Catcher instead of Extensions Manager, and it is described as an "Update for audio software on PCI Macintosh computers." Unselect it in Extensions Manager and restart. I do not know if the original iMacs need this extension. Try running that way to see if there "audio" related issues. My "mantra" for Mac OS 9 is to disable as many unneeded extensions as possible, and your system will be more stable and snappy. -

Adobe CreativeSuite Standard 5.5 has been installed but Extension manager is failing to load or fails whilst loading. Would someone be able to assist?
Below is the end of the log.txt file after the ExManLog.YES file was created.
The log file isn't always created on some machines where the .YES file is setup.
The software is installed on Windows Vista. Some machines have a Core I3 chip.
I have been reading the forum and tried changing the permissions where I can and tried the flash issue. Firefox is not installed.

Failed to load extension com.NAI.SysCallExt
In system preferences, I made 'cd' my startup disk.
Now, when I try to override and startup from hard disk, computer screen ends up with a black box in middle of screen.
command-v yields text that includes this statement several times in yellow: "Failed to load extension com.NAI.SysCallExt.
I want to start up from hard disk, but always get black box on screen and/or this statement (when I use command-v.

I decided that (1) since my essential docs were backed up, and (2) it seemed obvious what the major instigator was (TechTool 3.0.3), I decided to take the risk and use DiskWarrior NEXT, rather than taking a different strategy.

DiskWarrior successfully, and as far as I can tell, completely recovered my directory and files, so that I am now up and running again.

Even though a warning bulletin was posted and several discussions occurred on this site, surely Apple and MicroMat could act more proactively to keep their customers informed. That said, it was MY fault for not having at least consulted the latest information before using the disk utility. -
"Failed to load Audio Unit" errors on opening project, but plugins actually load OK
I'm experiencing an issue where Logic Pro X will give an error saying "Failed to load Audio Unit" when loading a project, but when you actually click "Continue" and go on into the session the plugins are there and work fine - playback and bouncing all work fine, and the plugins were listed in the error dialogue all work fine.
Some notes on the error:
- it's happening on a range of different plugins on my system - ilok-protected plugins (Soundtoys, Slate among others), UAD plugins, and non-dongled plugins (Plugin Alliance stuff, among others)...
- seems that as a project is worked on over time, at some point the error starts to happen on a save, and then becomes apparent on the next reload... it also translates between systems - a couple projects where this is occurring were started in a different studio (where the error first showed up), and working on them at my studio the error is still apparent.
- also it seems like the error doesn't happen every time a project is loaded. If I'm launching Logic fresh and opening a project, the error will occur, but if I close and open another project and the come back to the first project again, the error won't show up on the first project anymore... but if I quit Logic and open that project again, it'll start happening again.
Has anyone else noticed this error?Start Logic and hold the control key to disable the audio engine.
You should be able to load the project now, and disable the offending plugin. When done, turn the audio engine back on in the audio drivers preferences. -
Command+V = Failed to load extension?
Ok so here's what I get after command+V:
Failed to load extension com.apple.driver.applemacriscAGP
Failed to load extension com.apple.driver.applemacriscPCI
Couldn't alloc class AGP
Then it keeps repeating:
Still waiting for root device
Does anyone know what to do? If I just let it boot up normally (without hitting Command+V) I can't get past the initial Gray screen with the Apple on it. I have tried many things to no avail. I have reloaded the OS many times.

Logic8 w/ BFD2 upgrade "Fails to load Audio Unit"
I had BFD 1.5 and now I installed BFD2. I got the latest update. Now my old songs (Logic 7 songs) do not recognize the BFD1 audio unit. The message is: "Failed to load Audio Unit, please contact the manufacturer for an updated version or further assitance. (-10110).
The drums aren't loaded or anything. This means I would lose my old song settings for BFD, too tedious to fix.
Runnning OSX 10.5.2 with Logic 8 on a G5 with 3GB.Actually not- just remembering that I had this problem immediately on 2 different computers with exactly the same up to date software: my MBP and Powermac. These must actually be legitimate disagreements in the software that Apple will have to address and the best thing to do is to leave feedback.
My reasoning is this: I was running Sampletank(ST) v. 2.2.2 without problems when I installed samplemoog, then samplemoog wouldn't load but ST can load it's instruments the same and everything was fine. Then I installed ST 2.5 and it wouldn't load. So it seems that manufacturers are getting ahead of the old A team- but to forgive is divine and thus forget the upgrade for the old version.
There are plenty of Sampletank XL users out there so perhaps you could report running v. 2.5 without problems? Obviously something trips Logic up and it may well be the upgrade. It's possible someone who just installed the newer version wouldn't have the problem but that's beyond me. -
Sometimes fails to load the right codec (or chipset) for audio
Sometimes fails to load the right sound codec (or chipset) for at boot, I think that it's same issue with alsa and OSS.
At boot time (toshiba u300) alsa (or kernel maybe) fails to load sound.

I have to say that before Arch it had installed Ubuntu 10.04 with OSS (problems with keyboard sound keys with alsa), lately before change to Arch I must to restart sound system before it works frequently with this procedure; «soundoff», then «soundon» absolutly freezes the laptop so I had to turn off it and turn on again, then «rm /usr/lib/oss/starting», and last «soundon», then it sometimes works.
Now my only way to have sound is to restart the system time and time again until it works.

Blacklisting, in the context of kernel modules, is a mechanism to prevent the kernel module from loading. This could be useful if, for example, the associated hardware is not needed, or if loading that module causes problems: for instance there may be two kernel modules that try to control the same piece of hardware, and loading them together would result in a conflict.
Some modules are loaded as part of the initramfs. mkinitcpio -M will print out all autodetected modules: to prevent the initramfs from loading some of those modules, blacklist them in /etc/modprobe.d/modprobe.conf. Running mkinitcpio -v will list all modules pulled in by the various hooks (e.g. filesystem hook, SCSI hook, etc.). Remember to add that .conf file to the FILES section in /etc/mkinitcpio.conf (if you have not done so already) and rebuild the initramfs once you have blacklisted the modules, and to reboot afterwards.

IMac won't boot - Failed to load extension com.apple.driver.applemacriscAGP
Hello, so I have an iMac G4 17" (running 10.4) and it was unfortunately not connected to a very good surge protector. The power surged and a fuse blew and now when I try starting it up it chimes and then just sits at the gray screen with the apple logo. Here's what I've tried:
Booting into verbose and single-user modes, this is what happens:

Still waiting for root device
Still waiting for root device
Still waiting for root device etc.
Booting into Open firmware mode:
I'm able to do everything just fine, including resetting the nvram
Resetting PRAM:
Works fine, chimes when appropriate
Starting from Tiger install DVD:
Works just fine, I'm able to access all the utilities and was able to copy files off to a flash drive with the terminal. I used Disk Utility to repair the drive and repair the permissions, both of which ran successfully. I did a clean install of Tiger and then repaired the drive and permissions. I selected the new install on the hard drive as the default start-up disk. Still it sits at the gray screen and I still get those same errors in Verbose and Single-User modes.
I found the source code (a cpp file) for the com.apple.driver.applemacriscPCI, but don't really know how to compile it and what headers are necessary etc, and even if I did I don't know where I would put it since the problem can't be on the hard drive after I did a clean install. I have not yet tried completely reformatting the drive before an install but doesn't the installer do that anyway?

Sometimes, a computer and its owner get lucky and when a surge disables a
Macintosh, a reset of the Power Management Unit (PMU) can help restore the
normal functions. Often, when this works, the computer may not go past the
grey screen at all, until the PMU is reset.
Instead of a simple surge protector, my computers are powered through a
backup power supply; an uninterruptable power unit with battery backup.
These also have automatic voltage regulation. Forget common protectors!
On the iMac G4 series, the PMU reset button is under the metal plate on the
bottom and is hard to see. There may be instructions with images available
online, perhaps in Apple Support's knowledge-base. This would also be in
the official service manual in pdf electronic documentation, for a tech repair.

You may have to see if the Apple Hardware Test can show error codes and
other hints of hardware damages; and perhaps a tech visit is not far off...

**SOLUTION** DLLML.exe Fails to load at startup (application error)
Hello everyone!
I purchased the SB X-Fi Fatalty Champion Edition soon after release, and think this is the best card I have ever used to date. Absolutely love it, unbelievable sound.
Now a year or two ago I noticed a certain file failing to load when I started windows.
This file, DLLML.exe. To my understanding this file is used to load EAX emulation support into older games/modules (essential loads a "middle-man" file between the game's sound files/.dll's, and the hardware SPU [Sound Processing Unit, not a real acronym, but that's what I call it since GPU and CPU are similar distincti've acronyms so if you see it used anywhere, it came from me ] to allow EAX support in non-EAX enabled games), and/or to add EAX effects in your audio files using the Wave Studio app.
I've pinpointed this issue to either a Windows Update, or the Creative auto-update (which could mean it is an error caused by an installaion combination of multiple apps at once) because this issue did not arise until I did these two things (did Windows Update, then Creative Auto-Update, then restarted computer).
Now I've primarily had this issue with Windows XP x64 but I have had this issue at times with Windows XP x86 as well.
After you install the software suite, it adds an entry in your Windows Startup to load the module (Start/Run. msconfig. Startup tab. Look for STARTUP ITEM: DLLML). The entry looks like this:
WinXPx86 looks like: "C:\Program Files\Creative\Shared Files\Module Loader\DLLML.exe" RCSystem * -Startup
WinXPx64 looks like: "C:\Program Files (x86)\Creative\Shared Files\Module Loader\DLLML.exe" RCSystem * -Startup
Under most circumstances this module loads fine, but because Creative, for reasons why I have not delved into, loads some of their DLL files differently than the Windows standard, which may also be a cause of this issue (extremely unlikely, I believe it is one of the 2 things i mentioned above).
Either way, after I did those 2 things, the DLLML.exe startup entry failed to load every time.
But I fixed it.
Now what you have to do to resolve this issue, involves editing the startup entry in the Windows Registry.
If you are scared to go into it, have no idea what I am talking about, or never edited the Registry before, don't worry. If you follow these steps EXACTLY, you have nothing to fear.
I will be using default installation parameters for file pathing, so if your Operating System is installed in a different dri've letter other than C:\, then just change the letter to the one your OS is installed onto.
Capitals used for emphesis on actions to take:
-Hit the START button, then select RUN
--------If RUN is not in your start menu, you need to enable it by right-clicking and open area of your TASKBAR, select PROPERTIES. --------Click the START MENU tab, then the CUSTOMIZE button. Click the ADVANCED tab. In the START MENU ITEMS box, scroll down to the entry that says SHOW RUN. Check the checkbox to fill it. Then OK/APPLY/OK, and you should be back to the desktop. Then go to START/RUN
-type the word REGEDIT
--------This opens the Windows Registry.
--------DO NOT change ANYTHING in this other than the exact directions I specify or you may corrupt your registry or even worse, corrupt your system, which means F&R time (Format & Reinstall). Treat it like a stripper. You can look all you want but don't touch or there could be dire consequences.
--------Now the fastest way to find this is to manually navigate to the specific "key" or "entry" you need to modify. Mouse-click the +'s to navitage further into the "hi've" following these maps:
-FOR WINDOWS XP x86 USERS NAVIGATE TO: HKEY_LOCAL_MACHINE\SOFTWARE\Microsoft\Windows\Curr entVersion\Run
-FOR WINDOWS XP x64 USERS NAVITAGE TO: HKEY_LOCAL_MACHINE\SOFTWARE\Wow6432\Microsoft\Wind ows\CurrentVersion\Run
--------Now you are looking for a specific entry NAME called RCSYSTEM. Depending on which version you are running. They are:
-FOR WINDOWS XP x86 USERS: "C:\Program Files\Creative\Shared Files\Module Loader\DLLML.exe" RCSystem * -Startup
-FOR WINDOWS XP x64 USERS: "C:\Program Files (x86)\Creative\Shared Files\Module Loader\DLLML.exe" RCSystem * -Startup
-------Now we are going to edit the entry. What we need to do is get rid of that * (asterix) character in the name. I don't know why it is there, I'm thinking it may be a coding error. But in my WinXPx64, this was the culprit.
-Double-click the RCSYSTEM key to edit it, and edit it so it looks EXACTLY LIKE THIS (If you're paranoid about doing something bad, COPY and PASTE this line pertaining to your installed OS. KEEP IN MIND THE INSTALLATION DRIVE LETTER, CHANGE FIRST LETTER IF NESCESSARY)
-FOR WINDOWS XP x86 USERS: "C:\Program Files\Creative\Shared Files\Module Loader\DLLML.exe" RCSystem -Startup
-FOR WINDOWS XP x64 USERS: "C:\Program Files (x86)\Creative\Shared Files\Module Loader\DLLML.exe" RCSystem -Startup
-click OK button. Now it is changed.
-------And we're DONE EDITING the Registry entry!
-now go to FILE in the top-left menu, then select EXIT
-RESTART your computer
-------You should not receive the "DLLML.exe failed to load" (application error) again!!
To check to see if it loaded into your system properly (as if not getting the error message anymore was proof enough it loaded properly), when you reboot back to the desktop, do the "three-fingered salute" (ctrl-alt-delete), then select the PROCESSES tab. Click on the IMAGE NAME heading to sort it alphabetically, and look for the name (WinXPx86) DLLML.exe or (WinXPx64) DLLML.exe *32.
If you see it listed, it loaded!! CONGRATS, IT BE FIXED!!
If this did not work for you, then your system is posessed, get a priest. I don't think a witchdoctor would give your comp good vibes O_o

EDIT: As a side note, I also noticed that since after this fix, my computer doesn't hang after sending the restart/shutdown killsignal, so I am wondering with people having similar hanging issues, if their issue could be resolved something similar to this.
Just a thought for any admins/mods/users out there that know what they are doing that maybe wanna test that out.

After a Windows Hotfix on my XP SP3, when shutting down my PC I would wait for a program and then get an error that EAX was not responding before finally shutting down. On next reboot I lost sound on my PC. After running tests, the Direct Sound test failed pointing at EAX.
This solution cured the problem. Again, kudos, job really well done!!! -
[Solved] Ncmpcpp Visualizer Failed To Load (Blank)
I've install ncmpcpp and mpd and I've followed each guide to enable visualization. Unfortunately I don't know what have cause it to failed to load.
Everytime I hit '9' to reveal it, it just showing blank terminal..for your information all of my songs just played fine.
