File system check fails "FATAL"
Dear all,
after about three of years of running, one of my Xsan filesystems died. The fsm process stops running after about 2 minutes in running state. A filesystem check with cvfsck -w -v <Name> ends with:
Creating Daten allocation check file.
Stripe Group Daten ( 0) 0xe8def800 blocks.
Building Inode Index Database 91458560 ( 52%).
Fatal: rf_relblk write failed - Input/output error
Building Inode Index Database 91459584 ( 52%).
Fatal: Cannot insert inotab for [0x32a9e378800]! -No such file or directory
Fatal: Cannot build inode tracking tree - Input/output error.
I cannot see any misbehaviour of the hardware itself. RAID Admin is green everywhere.
The filesystem size is about 17 TB and the data is quite important. Is there any way to proceed without re-formatting?
Some more information I found in the log files:

Best regards,

Thanks for the hint. Well, the cvfsck -j command succeeded rather quickly. But the -w afterwards fails again:

I keep getting a file system check fail after a new install:
The error message states that "/dev/hda3 could not be read or does not describe a correct ext2 filesystem"
/dev/hda3 was setup as ext4 and as /, so I can't really believe this error message. Thanks rockin turtle, I did a standard reinstall instead of legacy like you suggested:
Even though my system didn't have sata, the correct approach was to do a standard install instead of "Legacy IDE".
This appears to be a bug in the installer right? I couldn't find it in the bug tracker. -

Disk Utility can't format my external HD - File System Formatter fails
Hi. I have a Freecom 1.5TB external hardrive that is connected by firewire to my Intel iMac. The drive has worked fine for a year and then suddenly the "error 36 code" appeared and nothing else could be saved to the drive.
Disk Utility told me that it could not repair the disk and that I should format it. At this stage the problems start!
DU fails to format the drive with the error "File System Formatter Failed."
I have read various conversations on this error on these forums and followed the instructions but nothing has worked. I have already
1) Partioned the disk making sure it is "GUID" - then formatted and it still fails
2) Used MS DOS to do it but that also fails.
I have noticed that the SMART status is "not recognised".
Is there anything else that I can do or am I just doing this wrong? I am thinking that I might be able to do something using the start-up disks but I don't know how to do that and don't want to jepardise my iMac whichis otherwise working fine!
Any help appreciated.
RobI went crazy last night with the same issue trying to format my Toshiba external drive. I could format as MSDOS but not Mac journaled. I tried the guid partition scheme etc... no luck. Then I read somewhere that Leopard has an issue with the partitioning schemes. I hooked it up to my macbook on Tiger and whalla, formatted no problem. Then I was able to hook it up to Time Machine and let it reformat it on my Snow Leopard version of OSX on the iMac. I'm on OSX 10.6.8.

[SOLVED] Systemd stuck at "Starting File system check"
Hi all
Backstory
My hardware:
Mobo: Gigabyte X99 2011v3 with UEFI and dual bios.
GFX: MSI GTX 970 4GB Gaming OC
HDD: Samsung 850 Pro 1TB SSD
CPU: intel 5820k
I have been using arch for about 2 years now. I recently bought new hardware, Gigabyte X99 motherboard uefi dual bios. I have tried a couple of times to install arch the old way, i.e. legacy mode or normal bios mode, however after a successful install, when I reboot my bios says "Select Proper Boot Device". Then I decided to put everything in UEFI mode and install arch using the uefi install disc. At first, I was not very successful, because I followed the arch wiki to the bone but when it comes to UEFI, instructions are a bit vague. In any case, I managed to install Arch and get uefi to work 100% by using this guide http://jorisvandijk.com/2014/installing … pt-system/ .
Problem
After installing arch successfully using UEFI mode, arch starts booting up, everything shows [OK] at the side, no errors, however, it hangs after a line that reads "Starting File system check ". I am not at home right now, however I thought I could start troubleshooting and maybe get some ideas.
I have tried the following.
- Chrooted back into my arch install using the live cd and mounting my partitions.
- Updated and synced pacman.
- Removed all journal log files.
- reinstalled Systemd.
- rebuilt kernel modules.
none of the above worked. It still hangs at "Starting fiel system check"
Any ideas.
SOLUTION
Solved by using
nolapic
Still had other problems after that, however, the initial problem was solved.

Hi Jason,
thx for your help so far. I have printed out the Arch beginners install and followed it to the bone and focusing on the UEFI parts. Arch installed without a single error, everything went smooth. Aftwerwards, I exited from the chroot environmount, unmount and reboot and again, it hangs on the startup. This time I took screenshots.
I present to the board, exhibit A, arch starting up after a fresh install :
http://oi61.tinypic.com/2m4x82o.jpg
I now present to the board, exhibit B, arch starting up for a second time, after restarting the pc :
http://oi59.tinypic.com/10ehvki.jpg
Afterwards, I restarted again. Went into bios setup and checked my boot priorities. This is what I saw:
I now present exhibit C, my bios boot options :
http://oi62.tinypic.com/24ys0bd.jpg
I tried all the options listed for my Samsung SSD, with and without UEFI as well as the option that was the default aka Linux boot manage". Each one did exactly the same.
I have a few ideas :
1.) Leave out swap.
2.) Disable fscheck.
3.) I have a feeling that my SSD is reading/writing too fast and that fscheck is having trouble ?
I am now trying option 1. In other words, I am reinstalling arch, again, following the beginners guide to the bone and focusing specifically on the UEFI parts. This time, I am leaving out swap. (I have 16GB of DDR4 memory). I have about a 70% feeling that the outcome will be the same. However, I will be back after that to comment on the outcome. Please leave feedback and help me through this where you can ! Thanks

When i was partitioning my hard drive i ran out of power (so it closed in the middle of the process). now when i try to partition it says Couldn’t modify partition map because file system verification failed.
You'll have to select the entire external hard drive by the hardware name itself and then apply a new partition table to it under the Partition tab of Disk Utility. Erase and format the drive.

Error message "file system formatter failed"
Hi
I just bought a MyBook 1TB external drive to use as a vault for Aperture. It comes formatted FAT32, however, it appears you can't use FAT32 for vaults. So I have tried to reformat it to MacOS extended, and MacOS extended journaled, however it fails after a few seconds with the message "File system formatter failed".
I am using Leopard, iMac20", 2.33ghz intel core 2 duo, 2gbRam. Any ideas , or is there some other third party software I could try?
thanksOpen up Applications / Utilities / Disk Utility.
In the column on the left there will be several lines for each storage device that presents itself to the system as a "disk". The first such line for any drive names the manufacturer and describes the drive hardware. That's the hardware description line.
Now a hard drive might be partitioned into several "logical volumes", each of which looks like a separate disk to the operating system. Each of those will be listed just under the hardware line for each of your drives -- indented a bit.
For drives that support removable media (such as your DVD drive), there will only be a volume line if there is currently media inserted in the drive.
Your main hard drive, the one you boot from, likely is partitioned into only one volume so there will only be one line following the hardware line and it will bear the name of your boot disk -- most likely "Macintosh HD".
Click on the hardware line to see the partitioning information for that particular physical disk drive at the bottom of the window.
Click on one of the volume lines indented below it (e.g., the "Macintosh HD" line) to see how that particular volume is formatted in its particular partition of the physical disk drive.
Partitioning is how the total physical space on the disk drive is divided and organized into one or more logical volumes. Formatting is how a given logical volume is structured for use by the operating system.
For PowerPC based Macs, the "default" partitioning should be Apple Partition Map (APM). For Intel based Macs the "default" partitioning should be GUID.
In either case, the "default" formatting should be Mac OS X Extended (Journaled).

File system checking exit code 8
Hi guys ... after done a filesystem checking I've got:
+0100: Incorrect number of file hard links
+0100: Checking catalog hierarchy.
+0100: Checking extended attributes file.
+0100: Checking volume bitmap.
+0100: Checking volume information.
+0100: The volume Macintosh HD was found corrupt and needs to be repaired.
+0100: File system check exit code is 8.
+0100: Error: This disk needs to be repaired using the Recovery HD. Restart your computer, holding down the Command key and the R key until you see the Apple logo. When the OS X Utilities window appears, choose Disk Utility.
After restart the machine as suggested, my filesystem check exit without finding any problem, so ... what is the problem in fact? ...
By the way: I do not have any strange behaviour on my mac, I normally run filesystem and permissions repairs in order to keep "him" clean and healthy.
Thanks in advanced,
JorgeThis seems to be a bug. You may be able to work around it by starting up from another physical device. Make sure you have a current backup before doing anything.
If you have a "clone" backup on another drive—not a partition on the same drive—start up from it by holding down the option key at the startup chime and selecting its icon in the Startup Manager display. Launch Disk Utility and repair your primary startup volume—not the one you started from.
If you have a Time Machine backup on a locally-attached storage device—not a network device such as a Time Capsule—there should already be a Recovery system on that device. Start up from it and repair in the same way as above.
Otherwise, use Recovery Disk Assistant to create a Recovery system on a spare storage device, such as a USB flash drive, and start up from that device. All existing data on the Recovery device will be permanently erased. -
File system verify failed, now what?
I was about to upgrade to Snow Leopard from 10.5.8 and before that tried to back up my drive to a new G Drive. Using Carbon Copy Cloner the process quit in the middle when the G Drive unmounted itself. I ran Disk Utility Verify on the G Drive and it showed up ok. I then tried again and CCC again stopped but this time with an error message I do not recall. Then I tried SuperDuper and the backup hung on an App (Garage Band) for 15 minutes and I aborted the back up. I ran Verify on the G Drive again and it was fine so I ran Verify on the HD and got the following message:
Disk Utility stopped verifying “Macintosh HD” because the following error was encountered:
Filesystem verify or repair failed.
Invalid volume file count
(It should be 1111231 instead of 1111232)
Invalid volume directory count
(it should be 242475 instead of 242474)
The volume Macintosh Hard Drive needs to be repaired.
Error: Filesystem verify or repair failed.
I have not tried the repair yet as I'm nervous I'm going to lose something. The drive isn't making noise but is slow. I've read suggestions of booting from the installation disc and then running Verify. My disc is 10.5.4, will I encounter issues after since I'm 10.5.8 now? Can I get a little hand holding here?
Machine is iMac, 2.16GHz Intel Core Duo, 4GB 667 MHz DDR2 SDRAM, OS 10.5.8
Thanks,

No, you need to start from your Leopard Install disc and run Repair Disk. See #6 in Formatting, Partitioning, Verifying, and Repairing Disks. -

AKI am able to install oracle ebs successfully, but not able to open the home page. Error coming is "ip address not found".Can you ping the IP address, hostname.domainname?
According to me the issue is that my server is on workgroup and not on any domain. I have given the domain name When installer was asking for domain name. I have also modified the host file for the same.Could you please post the contents of the hosts file here (you may edit the hostname/domainname values to something different as we need to see the format of the entries).
Now i have two options:
1. If i put the server on the domain which is passed to installer when asked.
2. Now, what all i can do to login on application with keeping the server on workgroup. You do not have to join any domain, and you can simply set this from:
- Right click on 'My Computer' > Properties > 'Computer Name' > Change
- Set 'Computer Name' to <hostname>
- Click on More
- Set a 'Primary DNS Suffix of this Computer' to <domainname.com>
thanks for your prompt reply to my previous post. I really admire your work on forums. You are welcome :)

Binary file content check fails!
Hello,
I am finding it hard to capture the content of a binary file.
Here is my code:
if(!(null == myInstance.getBin_File().toString())
&& !(myInstance.getBin_File().toString().equals(""))){
// Do something
}else{
// Do something else.
} When the binary file is blank, control still goes into "if" instead of "else"!!!!
How should I change the "if" condition so that, whenever the binary file is blank, I wanna execute "else"
Thanks
SrinivasThe purpose of toString() is (supposed to be) to return a string identification of the object. That doesn't necessarily mean the contents of the object; in fact, it normally wouldn't. For example, suppose an object holds a 100,000-word dictionary. It's toString() response could appropriately be "[100,000-word widget dictionary]", and not every word in the dictionary.
So, if you're checking to see if the "xstring", whatever that is, is empty, then using toString() is almost certainly not the way to go, and only would be if "xstring" is really poorly designed and you shouldn't use it anyway. -

I have deleted and reformatted many of these flash cards and not experienced any new FSCK files. The flash cards from new ASA's have all passed.
This leads me to belive that the master flash card used in manufacturing, that was used for the creation of the imaged flash cards, in final production had corruption at times. And these FSCK files that were on the master used for duplicating, were then copied with its FSCK files, to the flash of the shipping ASA's.
