Sorry, but what do you may by "why I have a balance"? What "balance" are you referring to? Do you mean that the iTunes Store shows that you now owe them money? If so, did you account for any sales taxes when you calculated that you had sufficient balance on your card to cover the purchases?

Did you purchase this device from Apple or an authorized reseller? It sounds as if you had a device that was jailbroken/hacked to operate on your carrier. Upon updating the iOS, it is now locked back to the original carrier the device was locked to.
Only the carrier the device is locked to can authorize an unlock. You will need to make contact with that carrier and see if they provide unlocking services, and if they do, if you qualify for an unlock. Otherwise, you are out of luck. One that carrier can take care of the unlocking.

You need to immediately pull credit reports to see what else he has done. Then "freeze" the credit report at all three agencies. That way even you need to lift the freeze to add credit cards or other credit items to your SSN.

I'm not entirely sure what you are asking, but if it's a debit card then I don't think that they are still accepted as a valid payment method - they are not listed on this page and there have been a number of posts recently about them being declined. Do you have a credit card that you could use instead ?

Try connecting to iTunes via USB and Restore. There are reports of this working with non-contract AT&T iPhone 5s.

Some country's might not include tax in their prices, and if you changed or added your credit card details whilst making the purchase then a small temporary holding charge may also have been placed - in which case that should disappear within a few days (http://support.apple.com/kb/HT3702). Some apps also then allow you to make in-app purchases (though you can disable that via Settings > General > Restrictions)

I downloaded Firefox, Greasemonkey, and a few other things for the MW game but when I logged into FB to play at the top of the main FB page it used to have options "turn on&off" ,settings, updates, etc. Now I don't see it there anymore. Is Greasemonkey running? It's a little to easy to disable by clicking the monkey icon (which causes it to have a faded appearance).
For each script hosted on userscripts.org, when you are on the page with the description, you will notice a Discussions tab. You usually can read posts about new bugs and workarounds for the script there if the above is not helpful.

You could try creating an iPod version of the films.
Use CTRL and B to display the Menu bar shown above. Alternatively, use the ALT key, followed by the F key, the W key and finally the I (eye) key - that's (menu)/File/Create New Version/Create iPod or iPhone version.

Once the card is redeemed, the balance is on your account, not the card.
