I think you should contact the apple support team. There might be someone who illegally worked out the code and activated it. The Apple support team is great, they send you e-mails personally, and are pretty kind when teaching you how to solve the problem. If one of your apple products is not out of date, contact them. Even if it out of date, bring it to a nearby genius bar, I think they will help you.

You will have to contact the Apple Store you bought the card from and ask, but if you scratched off the coating to expose the redemption code, then no, they won't take it back.

Change how it syncs by enabling Manually manage music and videos, then you can drag and drop the music that you want on your iPhone. Revert it back to test to see if that resolves the issue.

Sorry, but there's nothing you can do. iTunes Cards are valid only in the country of purchase. So you cannot use that card in the Swiss iTunes Store. Sell or give the card to someone back in Spain.
