Inserting record twice, Pls help, urgent

Question

i am using forms 6i. i have simple dept block in my form. if i try to insert by using insert button which i created, its inserting twice the same record.
If i am trying to insert in the table which has got primary key field, its is giving frm-40508 error.
PLEASE help me

You've probably got a base table block. That would handle the insert for you, without the need for you to code an insert statement.
If so, your insert statement is duplicating the default functioality of the form, and you can remove it.
The COMMIT statement will be interpreted as a COMMIT_FORM, and will apply the changes made to base table blocks to the database, and commit those changes.
