Is it possible to individually identify a response to a Forms central survey?

I am suggesting affixing a predetermined code (6 digit) to each response, which is exportable to an excel spreadsheet.
This is for the purposes of tracking who has responded without using any other cumbersome method.

Thank you for your prompt reply, and your acknowledgement raises another issue:
If all form surveys administered through FormsCentral are just anonymously ‘sent to’ and ‘received from’
respondents (except for email acknowledgements) then may I request an enhancement to the service,
which simply attaches a unique code(apart from date & time) to the response set each time a new respondent
submits a survey form.
This same code could also be attached to email acknowledgements sent back to forms central. This way at least
individual response sets can be linked to an email address. This requirement is fundamental to proper administration

    I don't mean to hijack this thread, but Randy had mentioned a bug in Chrome regarding copy/paste. I've been searching for hours for a solution, but nothing on Google Chrome and c/p for the fillable area of a pdf document. Google forums have not yielded anything about fillable forms, so I'm hoping an Adobe forum would be more knowledgable.
    I have a fillable form online where the fillable area cannot be pasted into or copied out of while using Google Chrome. Note that the text already on the document can be c/p'ed. You can see it here: http://www.centralcallegal.org/eictaxcampaign/Coverform.pdf   This is a form for our self-help computer kiosks which use Google Chrome. The form is designed so users can copy and paste their own info into the cover sheet so there won't be any typos.
    Without looking at alternative methods of using the form (i.e. saved on desktop), is there a way to fix this copy/paste issue? It is definitely a Google Chrome issue, as c/p works fine in IE and Mozilla. Thank you.

  • Columns' positions do not match in pdf form compared to the "View Response" tab in Form Central

    I think I understand why this happened. And I am loookng for workarounds.
    For example, let's say in the beginning you have five fields (A, B, C, D, E) in your form. Later on, you decided that you need to add two more fields says A1, and C1, and now the fields are as follow A, A1, B, C, C1, D, E. After you submitted the form to Adobe Form Central, you take a look at the "View Responses" tab, and you realize the order of the columns are as follow A, B, C, D, E, A1, C1.
    I believe this happened because the new fields A1, and C1 were added later. But I am wondering what are the workarounds?

    Yes, it does work in Acrobat Reader. I'm just trying to determine if this is a recent bug in Tiger (like maybe because of the 10.4.9 update) or if it's something related to the particular version of Acrobat the file was created in.
    I haven't had to have Acrobat Reader on any of my Mac's since Jaguar.

  • Submitting responses to form central

    I created a form with Acrobat Pro.  I moved it to Forms Central.  I marked it as "open." Under the distribution tab, I downloaded it as pdf to my computer, having checked the mark to have a "submit" button.  As a trial before sending out in e-mail to others, I filled it out and clicked on "submit."  I am not seeing it show up as a response in my Forms Central.  Why not?  Did I miss a step?

    First off, this form is the first and only thing I have created with
    Acrobat Pro, so trying to learn on the fly.  I believe I created the form
    in Acrobat (from scratch, no template),  then later in experimenting saw
    the option to move the form to Forms Central.  Then I downloaded it from
    Forms Central to distribute by someone else who has our reunion mailing
    list.  That is when I notice the 'submit' button did not seem to work.
    I have continued to experiment and now see that there is something call
    "Tracker" that can be utilized to distribute my form by e-mail and this is
    where I saw the reference to being able to handle up to 500 responses.  I
    am not sure how tracker handles the forms after they are sent back to me.
    Does this have to be distributed from my computer?  I am hoping to provide
    this form to someone else who has the distribution list and then the
    responses come back to me.
    As you can see I hardly know what I am doing here, but must say I have not
    found much guidance from within the software.

    I would like to inform you that with forms central, you can convert the selected fields to PDF.
    However, your selection should be sequential. Example- You can convert column 1,2,3,4 and not select column 5; but it is not possible to select only column 2 and 3.

  • Is it possible to create one home page with links to multiple forms in Adobe Forms Central?

    I have several forms that are all related. Is it possible to have one online landing page where I can link to my other forms from?
    It would be easier to share one link than multiple.

    You could create either a web page of your own to include the links into, or you could create a form that you include the links in - maybe add them as single checkbox fields where the user could check the box stating that they have filled out the related form.  There is a "Insert or edit Link" button in the top toolbar when designing a form, select text you would like to represent the link and click the link button to add the URL to a form.
    The responses for each form would be connected to those individual forms and would not be presented together.  When you recieve responses from the various forms you would not have any built in way to connect the same user to the variuos forms but you could include a "Required" email field in all forms so that you can see who submitted which forms that way.

    Is there a way to download or view the completed form that individual would fill out? It's great having it in the spreadsheet format, i am also looking to keep the completed form on file for future reference.

    Will the "Download response as PDF form.." work for you? Open the form in FormsCentral, go to View Responses tab, then select a row and right click, you will see the item menu that I just mentioned in the list.  You will have to save it each row at a time.

    I have a form running that has captured and recorded the correct data...and shows up correctly in the response table however when I "downlaod the response as a PDF form" some data is missing and some is there.
    Any ideas?

    Were you having the same problem when exporting your responses data in different format like Excel or CSV? If if is possible, please share your form with me.
    Please do the following to share your form
    1. Click on “Design tab” on the top left corner.
    2. Click on the “Share” icon on the bottom left corner.
    3. Click on “Add Collaborator” on the popup menu.
    4. Enter [email protected] under “People to share with”.
    5. In the Message edit box, type "download response as PDF form ....missing info"
    6. Click the “Share” button on the bottom right of the dialog.

    When i have a paid account is it than possible to get the information from the different forms send to seperate emailaddresses.
    Who dont have a account from formscentral but just need the information directly in there own inboxes?
    Because we're looking for a good and simple way to make qoustion & complain forms. But these need to go to different people in the company.

    You can have notifications sent to multiple email addresses, but each will need to have a FormsCentral account.  They can be free accounts.  What you do is create your form and set up the email addresses that you want the information to go to to be Co-authors or Contributors.  The co-author/contributor accounts will then need to log into FormsCentral, go to the Options tab and check the email notifications checkbox.

  • Is there a way to revert to the original form responses in Forms Central?

    I was reviwing responses in Forms Central, and I noticed a few cells had been shaded as "Manually edited." I am the only person with access to our responses, so I'm guessing that as I was using my scroll button on my mouse, I accidentally changed something. (My mouse has been acting up lately, and the computer has registered a double click when I only single clicked.)
    Is there a way to revert back to the original form responses? I caught this too late for be able to just use "undo."

    OK, then maybe you made (by accident) some changes prior to your current editing session. You might have edited the form, made 'change1', closed the form, and reopened it. At this point you cannot use Edit / Undo to undo 'change1'.
    It looks like you need to undo your changes manually. You could take a look at the View Responses table history to see what you had before.  At the View Responses tab, click on the clock-history icon on your lower right corner, you will see the History bar and you can select a previous version to see.

    You can embed the form on your website, but you need to make sure that javascript has been enabled in the browser. You need to copy the embed code and add it into your HTML code. If you would like FormsCentral to generate embeded HTML form without using javascript, you may post a feature request and vote it. Hope it helps! Thanks!
