If you are shipping back I assume you called Apple and they are sending you a box (as that is what they do). When you get it you will find:
1) Details short instructions of what to do.
2) Paper Clip
3) Foam packaging
4) Box (with foam in it too)
5) Sleeve in Box inside
6) Tape
Instructions will say to
1) back up phone (if you can via iTunes)
2) Use paperclip to eject and keep SIM (put in spare phone if you have it)
3) Put phone in sleeve.
4) Put sleeve in padded box.
5) Put box in padded shipping box.
6) Use tape provided to tape shut.
7) Remove outer shipping slip to reveal return slip (you don't have to pay for anything)
8) Go drop off for shipping or call FedEx for pickup.

When sending in the phone for service you want to remove the SIM Card and do not put any accessories in with the box that came with your iPhone.
