SharePoint Online - Trying to add an app from Organization's Catalog causes "Something Went Wrong"

Hi Everyone,
I registered my app with appregnew.aspx, and I uploaded the app file to the app catalog. I even successfully installed my app to the sharepoint site. But when I tried to use it I realized I gave it no permissions.
So I went back to visual studio and gave it some permissions. I uninstalled the app from my SharePoint site, then deleted the app from the app catalog. I uploaded my new version to the app catalog just fine.
Now when I try to install the app I get "something went wrong". What happened?
I also tried another version of the app where I removed all the permissions but that still didn't fix anything. Any ideas on what is happening?
This is an online instance, so I have no access to logs.

Make sure you have all placeholders as below:
1. DeltaPlaceHolderLeftNavBar
2. PlaceHolderLeftNavBar
3. DeltaPlaceHolderPageTitleInTitleArea
4.  PlaceHolderPageTitleInTitleArea
Also each of them should not be set to Visible=False.
The code for the content placeholders are:
<SharePoint:AjaxDelta id="DeltaPlaceHolderLeftNavBar" BlockElement="true" runat="server">
<asp:ContentPlaceHolder id="PlaceHolderLeftNavBar" runat="server">
</asp:ContentPlaceHolder>
</SharePoint:AjaxDelta
<SharePoint:AjaxDelta id="DeltaPlaceHolderPageTitleInTitleArea" runat="server">
<asp:ContentPlaceHolder id="PlaceHolderPageTitleInTitleArea" runat="server">
</asp:ContentPlaceHolder><asp:ContentPlaceHolder id="PlaceHolderTitleBreadcrumb" runat="server" Visible="false"></asp:ContentPlaceHolder>
</SharePoint:AjaxDelta>

Apps with autohosted components (including Access apps) actually host their DBs in Azure, though (that's what "autohosted" means) so if you don't have the server connected to Azure then those kinds of apps will fail to install.  If the only apps that
are failing are Access/autohosted, then that's probably the issue with your setup.  Developing on Office365 would take care of setting that up for you, though.
