Trying to create a form

Newbie here. I am trying to create a form and have the information emailed to me. When I hit the submit button, it opens my outlook email. I have searched online and looked @ videos and am sure I have everything correct. What am I not doing? Thanks in advance.

    Okay, I loaded the page under a different name http://abilityaxis.com/test.html. Viewing the source code doesn't show the yellow.
    For demonstration purposes I copied a small amount of the text and changed the colour of what is yellow in Dreamweaver to this pinkish colour (I tried yellow but you couldn't read it)

    Hi,
    You have the choice between xml or pdf, in later versions of designer you can choose with a dropdown on the email button Object palette, the "Submit As";
    In earlier version you had to edit the XML Source and change the format from xml to pdf (or vice-versa);

ORA-0000: normal, successful completion"
    What am I trying to do:
    I try to create 2 forms on the same page, each one based on tables that have PK column named "ID".
    From what I understand, HTML DB is trying to create for both forms the same "P1_ID" (P[no_page]_[column_name], right ?) hidden item in order to keep the value of the pk column...
    I think this is not right...what do you think ?

    You cannot use multiple automated row fetch or automated DML processes on the same page where any table column names are in common between them.

  • Trying to Create Tabular Form with apex_item... and apex_item.select_list

    I am trying to create a tabular form that uses multiple altec_item functions (select_list, select_list_from_LOV,...). Whenever I attempt to create a new page and select Tabular Form, I am forced to select a table and columns. So I have just selected any table and once the sql is created, I overwrite it with my own sql. When I try to apply the changes, I get an error anytime I use apex_item... for a particular column. I have tried the same creating a standard report with the same results. I have read at least 20 examples but none say where to start from when creating the tabular form and just jump right in to the code. Can anyone please point me in the right direction?

    You can start with creating a simple classic report. You can use apex_item functions in your query and later set your column attributes to "Standard Report Column". Basically, you are creating a updateable report.

  • I am trying to create a form in acrobat pro 8 and it can't be filled in in reader 9

    When I open the form in reader, it says it is a pdf/a file.and the form fields do not show up unless I go to edit/preferences in reader and tell it to never view documents in pfd/a mode.  I am putting this document on my website for users to fill out, and don't want them to have to do that.  How do I create the form that is not a pdf/a document?  I have been messing around with this for a day and have not been successfull.  I have an old document from last year that apparently is not a pdf/a document and it works fine.  I cannot make this year's document open correctly in reader 9.  http://www.houseofsweden.us/docs/DecNightsSignUps2009.pdf  This is a very simple form created from a word document.

    When originally creating the pdf, you would need to choose another pdf conversion setting. In Word if you use the pdf menu, change your settings there: Adobe PDF > Change Conversion Settings. I would use High Quality Print instead. If you use the File > Print method, click the Properties button next to the Adobe PDF printer selection.
    For your already created form, you can change the file so your users will not encounter issues. In Acrobat 9, which hopefully is similar in process to 8: Advanced > Preflight > Profile tab > PDF/A compliance > Remove PDF/A information.(You'll have to unsecure your form first).
    You can read about PDF/A files in the Help.

    My table has a list of names with product information in corresponding columns.  I want to create a form that I can import this into where every row makes a new part of the form list I create.  Is this possible?

  • Trying to create order form in Acrobat 6

    Basically everything is exactly how I want it except for one thing. I want to assign $ values to the different size choices (4x6, 5x7, 8x10) so that the "Cost" column automatially updates and then would be multiplied by the "Quantity" column.
    I have done some research and I think this involves a custom javascript as opposed to the built in calculations. I'm not very familiar with writing out javascript so not really sure where to start.

    I am working on a very long document, over 100 rows and about 18 columns, which will exist only as a pdf. I need to make 6 of the columns interactive so that they can be filled in with one of three letters by the end user.
    I created the design in InDesign CS 5.5 as a table. That worked well for design control, but Acrobat X 10.1.3 does not recognize any of the cells in the PDF document. Is there a different InD export option that will make a table convert to a fillable form? I used press quality, then opened the pdf in Acrobat.
    I have worked around by creating a short drop down list with the three letters in it and copied that into the cells using the Place Multiple Fields command. Then I can adjust position easily enough.
    *Except* my client would prefer that the end user just type one of the letters into the cell/field rather than doing a drop down selection on each of the several hundred cells. She'd also like to be able to copy and paste the final filled-in information into her original excel document.
    Am I missing something in Acrobat? I don't do fillable forms often and have never had one this massive.
    I am working on a Mac.

  • I am trying to create a form so that it is easier for people to fill out timesheets

    I know that i will most likely require an IF function of sorts within it,
    Basically what i want to do is create 2 dropdown boxes, 1 of them will contain company names and the other will contain job numbers,
    Now I want to create it so that when you select a certain value for the first box only a selected number of values come up in the 2nd dropdown box, and so on and so forth.
    I.E.
    Mcdonalds - dropbox pulls up values specific to mcdonalds
    burger king - dropbox pulls up values specfic to burger king
    but they all need to be the same box, just the first box specifies which values will be shown in the 2nd box
    I hope i am explaining this well enough that someone can help me!

  • Trying to create two form buttons side by side?

    Hi all...
    Kind of a strange question...  I've been messing around with this one for way too long now and thought I'd seek your help...
    Anyway,  one <form> with two submit buttons on them.  One is a "submit" button and the other is a "cancel" button.  I do have them working fine.  The "submit" button forwards the <form> data to the .php file for processing (and works fine).  However, the cancel button which does nothing more than calls on the index.html.
    Problem:  I can't get them to sit side by side on the form...  Or at least not without making the "cancel" button fail.  Remember this is all one form (or is it?)...
    The last lines of code for the "submit or cancel" buttons are below...

    If you are learning, I would not worry too much about JavaScript, just concentrate on PHP.
    Normally I would use
    <input name="cancel" type="button" value="cancel" onclick="window.location='index.html'">
    which simply saying that when the element (<input>) is clicked on, change the URL to 'index.htm'.
    The reason I used the function in my first example, is because DW supplies the function auromatically by following this procedure
    1. In design view, click the cancel button. In code view you will see the element highlighted and the status bar will show <input> highlighted as in
    Also in the properties window under the status bar, you will be able to change the button attributes.
    2. With the button highlighted, click on the Tag Inspector-->Behaviours tab, click the '+' and 'Go to URL' as follows
    This will pop-up a dialog.
    3. In the 'Go To URL' dialog leave the 'Open in' field with the default value and enter the requested URL in the 'URL' field, using the browse feature if desired.
    4. Click 'OK' and in the code view you will see the added JS function and the behaviour attached to the button. In the Behaviours panel you will also see the behaviour attched to the selected element.

  • Trying to create fillable secure form in Acrobat X

    I have Acrobat X Standard and I am trying to create a form that can be filled out then submitted but not printed or saved. When I set security and try to distribute it I get an error saying it can't distribute because of form security settings. I have recently updated to 10.0.3 but it did not fix the issue. Or is it that this is something Acrobat X standard won't do? It may be that I'm doing something wrong too since I am new to this........

    Then I try to create a form based on this procedure and it fails with the above error. I can create this procedure in the PORTAL_DEMO schema without problems. I can also create a form in the WARRENP provider based on PORTAL_DEMO.GIVE_RAISE and that works also.
    I'm on Portal 10G (9.0.4) on Linux, infrastructure database is 9.0.1.5.0.
    Can anyone tell me what I've missed in my setup?

  • How can I create a form which accepts answers in 2 different languages?

    Hi,
    I'm trying to create a form where the users can answer either in English or Hebrew (with the ability to answer some of the questions in Hebrew and some in English)
    Currently on the form options it's set for English language (Didn't see an Hebrew option there) and when I test the form I get this error for the fields I try and fill in Hebrew: "The field contains unsupported characters. Please modify your entry."
    Is there a way to allow both languages to be inputted into the fields?

  • Problem when creating a Form with Launch Task Form Wizard

    I'm using jdeveloper 11.1.1.6 over Windows 7 64bits for my BPM project.
    I'm a new with BPM and i have a problem when trying to create a task's Form with the "Launch Task Form Wizard". I'm trying to create my Form based on a custom template, i already have imported the template as a library, now after i have selected the template i follow the wizard and i reach the wizard's rowX-columnX where i should choose the field i want in my form, but the problem is that i don't see any fields in that part of the wizard, the window is empty :(
    I have created a business object based on a XSD, then i created a process data object based on the previous created business object and finally i used this data object as a parameter to the human task.
    What could be my problem ?? :( :(

    I have also faced this issue few times. There could be a namespace issue / schema issue. Please check if you are able to see task:task in the schema section anywhere in the .task file you created. If that is not shown, the fields will not appear while creating human task form.
