Workflow stays in executing mode

Question

I am facing an issue where the workflow status goes into executing mode forever. I have a 3 activity w/f, each with a manual action. 1st manual action is shown correctly with the w/f in suspended mode. This works till the 2nd form is submitted. After submitting the 2nd form, the 3rd form is displayed on the screen but the w/f goes in executing mode forever. The w/f diagram shows pending approval on the 2nd screen/form. On submitting the 3rd form, an error is displayed saying object not found in repository with no change in the w/f status (executing). There is no issue with forms, I tried switching forms but it doesn't help. Below are the 3 manual actions:
Manual action 1:

Hi,
I don't know the solution to your problem, because each time I did a manual action was for calling a WF in the background.
But you could try to see 2 things:
try to put this :
<Owner name='$(WF_CASE_OWNER)'/>
instead of <ref>$(WF_CASE_OWNER)</ref>
and you could try adding itemType='suspend' to your manual action.
I don't have other ideas..
