Exchange 2013 Update Crashes: System.AccessViolationException
Currently we are trying to update Exchange Server 2013 from RTM Release Version 15.0 (Build 516.32) to CU4 (SP1). Each time we try to run the installer Configuring Microsoft Exchange Server. This happens for any of the CU Updates. Exchange is
installed on the Domain Controller, Windows Server 2012.

For security and performance reasons, it is not recommended that installing Exchange Server on DC server.
Please use dcdiag command-line tool to check if your DC server run normally.
Please check if there is related error in application log, when the issue occurs.

Problem was that if you have more than one ActiveSyncVirtualDirectory, the installer for Update 6 fails because it's not expecting an array of virtual directories to be returned. Solution is to remove the "extra" virtual directory, perform the
installation, and then re-add the virtual directory.
None of the other types of virtual directories are susceptible to this, only ActiveSync.Hi,

Exchange 2013 update fails from SP1 to CU5
Hi,
Trying to upgrade my exchange server 2013 SP1 to CU5 but not able to do it successfully it is giving below error while analyzing prerequisites .
Server is in test environment and it has 2 network interfaces.
Error:
There was a problem accessing the registry on this computer. This may happen if the Remote Registry service isn't running. It may also indicate a network problem or that the TCP/IP Netbios Helper service isn't running.

This issue occurs if any of the following conditions are true:
The built-in security group “NT Service\Eventlog” does not have permissions on the folder %SystemRoot%\System32\winevt\Logs.
The built-in security group “NT Service\Eventlog” does not have permissions on the registries and its sub keys,
HKLM\Software\Microsoft\Windows\CurrentVersion\Reliability Analysis
HKLM\System\CurrentControlSet\Control\WMI\Autologger\EventLog-Application\
To solve this issue, please take the following steps to give requires permissions on the log folders and registry
Step 1:
Restore the default permissions on %SystemRoot%\System32\winevt\logs.
Authenticated user - List folder/read data, Read attributes, Read Extended attributes, Read permissions
Administrators - Full control
SYSTEM - Full control
EventLog - Full control
To restore the default permissions on folder %SystemRoot%\System32\winevt\logs, follow these steps.
Right-click on %SystemRoot%\System32\winevt\logs and select Properties.
Select the Security tab.
Click Edit button and click the Add button in the permissions dialog box.
In Select users, computers, or Groups dialog box ensure that under object types Built in Security Principals and the location as local computer name is selected.
Enter the object name as "NT SERVICE\EventLog" without quotes. And click
OK. This group should have full control on the folder.
Once EventLog group is added add the rest of the groups with above mentioned permissions.
Step 2:
Provide the permission to registry,
Open Registry Editor
Locate
HKLM\Software\Microsoft\Windows\CurrentVersion\Reliability Analysis, Right-click it and Select
Permissions...
Checked whether you have Eventlog group. If not, add “NT Service\Eventlog”, then give Full Control, then click OK.
Locate HKLM\System\CurrentControlSet\Control\WMI\Autologger\EventLog-Application\, right-click and Select
Permissions...
Repeat same in the No.3 step.
Note: Before you modify the registry, make sure to back it up and make sure that you understand how to restore the registry if a problem occurs. For information about how to back up, restore, and edit the registry, click the following article number

MBOX K9VGM-V after Flashing the BIOS successfully Live Update crashes system
I recently purchased a barbones computer with the K9VGM-V motherboard. After installing all the drivers on the disk, updating Windows 2000 competely through Windows Update I decided to try Live Update. The first thing I updated was my BIOS from Ver. 1.6 to 1.7. The flash was completely successful. However when I went back to Live Update(the latest version btw) the minute I executed the program my system completely crashed! It was like I had hit the reset switch. I decided to format my HD and reinstall everything. The BIOS works fine and the system is entirely stable, but even after starting with a clean slate I tried the Online version of Live Update and the same thing happened. My computer crashed and restarted itself. This happened after the BIOS flash so it must be related to that. I am just not sure how or why. Any suggestions or input?

We got strange issue with Exchange 2013 MP. Some components in DAG are in state "Not monitoring". For example:
"Exchange 2013 Service Components" on DAG members not motitored:
Mailbox Count in organization is null:
And DAG object is in the "Not monitored" state too.
MP was installed as described in documentation (account, profile, etc.).
Proxy on DAG members enabled.
Any ideas how to troubleshoot this issue?

I would like to suggest you recheck Exchange 2013 Management Pack Pre-requisites and Considerations.
Before importing the Exchange 2013 MP into System Center Operations Manager, there are some pre-requisites that have to be met:
You have one of the following versions of System Center Operations Manager deployed in your organization:
System Center Operations Manager 2012 RTM or later
System Center Operations Manager 2007 R2 or later
You have already deployed SCOM agents to your Exchange Servers.
The SCOM agents on your Exchange Servers are running under the local system account.
The SCOM agents on your Exchange Servers are configured to act as a proxy and discover managed objects on other computers.
If you are monitoring Exchange Server 2013 Database Availability Groups (DAGs), ensure that all DAG members are monitored by Operations Manager.

Our employez sync there Exchange mailbox on Iphone and Android phones, it works well despite the fact that when you go on the phone and check your contacts you can see that some informations are missing like the postal code.
I have checked on the outlook of the users and the information of postal code has been setting up.

The error explains that the virtual directory 'PowerShell' already exists, so we need to remove the 'PowerShell' under "Default Web Site" and "Exchange Back End" firstly, and then continue to upgrade the Exchange 2013 CU6.

add the account to the admin group and continue to install the CU5.
If the issue persists, I recommend you download a new CU5 for Exchange 2013 and reinstall it to check the result.

Exchange 2013 CU6 MSExchangeDiagnostics Crash
Hi,
On new fresh install of Exchange 2013 SP1, service MSExchangeDiagnostics crash then restart each minute!
Configuration: 2 Exchange Servers with DAG and 2 DB. Same error before DAG configuration.
I try to update with CU6 but error persist.
I can stop this error by disabling MSExchangeDiagnostics service, but not a good workaround...

Windows Scheduler Service was disabled by specific GPO (against CONFICKER ATTACK).
Enable Scheduler Service then restart server and message disapear!
Hope this error message could help someone in the futur.

Visual Studio (2012 / 2013 etc) crash on updating VSO / TFS bug on check-in
I'm posting about this bug today after triggering it no less than 5 times in a row (because of my bad habit to press Ctrl-S often) each time losing my check-in comments and having to start again after restarting VS2013.
Simple to reproduce, all you need to do is open a bug in Visual Studio, make a change to the bug outside of VS (e.g. by associating it with the build) then saving the bug. Crash.
For example:
Open project with pending changes
From Team Explorer - Open Work Items / Open Bugs (or any view containing items)
From Team Explorer - Open Pending Changes
Drag and drop any bug to the "Related Work Items" section below comments
Write a comment
Hit Ctrl-S
Crash.
Also trying to update the bug / item itself after checking in (e.g. to close the item) will cause this.
This bug has been around for at least the last two major version, can someone please fix it? (Save me throwing another mouse through the monitor :])

Base on steps that you provided, I made a test in VS2013 update 2 with TFS 2013 update 3 and VSO, it works fine.
So, I think the issue is in your VS or environment. Please refer to these steps below to narrow down this issue and deal with it.
Create a new user account and run VS, then do the test, if it works fine, I think there are something wrong with your profile and you could create a new account and copy current user info to it.
Check your OS with some tools, such as
Sfc /scannow
If your environment is OK, let’s check your VS.
Use
devenv /log to gather log and check the error form log file.
Try other commands to fix, such as devenv.exe /safemode, devenv.exe
/resetskippkgs, devenv.exe /installvstemplates, devenv.exe
/resetsettings, devenv.exe /resetuserdata
The devenv.exe tool is located under your VS location/Common7/IDE.
If the issue still exists, please repair or update your VS.
On the other hand, the issue of the bug has been around for at least the last two major version, which is related to TFS, I suggest that you should open a new thread in TFS forum.
