How to create a form that requires thousands of checkboxes?

Hello forum members,
I have Adobe Acrobat 8 Professional.  I've been searching the internet/forums for the past few hours and I can't seem to find any information on how to create a form with many checkboxes.  I do not want to have to give individual names to thousands of checkboxes.  I'm hoping there's an easy solution to my problem.  Maybe I don't even need to use checkboxes.
Here it is...
I'm a DJ and wedding season is coming up.  For each wedding that I do, I usually give the bride and groom a list of my songs and they choose a bunch of songs that they'd like to have at their reception.  In the past, I've just been sending them a webpage word document with the songs and they'd send me their selections in a different document or directly in the reply email.  I'd like to make this process easy for them (and myself) by making a pdf document where they can simply place a checkmark beside the songs that they want and then send it back to me.  As a bonus (if it's possible) i'd like to be able to export the songs that they selected into a separate document so that I don't have to go through the entire pdf (with thousands and thousands of songs) when they send it back to me.
I would be ever greatful if somebody help me or direct me to a link with some additional information.

  • How to create a form that people can fill out

    I'm creating forms using Adobe Acrobat XI Pro with the goal  being to save the form on a network drive so users may access the form,  fill out the form fields and save the file. Currently, users are not  able to complete the form; it requires them to print out the form in  order to complete. I am very techno-ignorant, so I sure that it is user  error. Any guidance towards resources or assistance would be greatly  appreaciated

    - have they Acrobat Reader, and are they able to use it?
    - are your form correct? (permissions settings, etc etc)
    - Are you creating your form from zero or from another documents?

    Hi,
    If your SharePoint environment supports InfoPath Forms, then you can customize the form and add rules to make the list items as read only when user A submits the form.
    you can then write a form load event to check the logged in user using username() function. This logic can be implemented in variety of ways, like setting a flag when User A submits the form, or storing user A username in a form variable etc., else comparing
    User A and User B values within form Load event.
    Another way of doing this is using Views or grouping all of the User A fields within a section etc.,
    The above would take care of Form logic, and for the workflow, you can use SharePoint designer to create a custom workflow, where it will run on onItemCreate and onItemChange events.
    The logic for workflow would be if the form Submitted for the first time, the workflow will start and send an email to User B, and when User B submits the Data then onItemChange change event will start the workflow to send an email to approver to approve

    I have a 5 question form I created. I'm using drop down boxes with a Likert Effectiveness scale from 1-5. I'd like to add a "Total Effectiveness" field to the bottom and calculate this based on the drop down box selections. How can I do this? Also - can I save such a form as "static" or must it be "dynamic"?

    Better yet, Acrobat Pro.
    To provide Extended form Rights you need to purchase Acrobat Professional.

    I need to create a form that is used by multi people but each user needs to fill out different fields within the form. I need the first user to make selections from a list and save it to send to another (the selection fields then need to be locked). Then the other user checks one of the selections made by the first user and saves the document to send back to the first user. Not sure if this is possible. I would like the first form field selections not to even show as a highlighted field to the other user (like just be more like a text field). I want the document to be able to be used more than once. The list selections will change and need to be sent to the other user at least once a month or more. Has anyone done this?

    There's no built-in way to do that, but it's probably doable using a custom-made script to hide and/or lock the fields.

    I am trying to create a form that would be emailed to one person, they would complete part of it, email it on to a second user who would then complete another part based on the input from the first, and then forward it onto a third for their input.

    Interactive forms cannot be saved locally using Reader. To do local save you either have to have Acrobat on each desktop or have the form Reader-extended to enable local save in Reader. A form is Reader-enabled using the LiveCycle Reader Extensions server component.

    I have created a form which will be filled out by customers and sent back to me via 'submit' button. What I am trying to figure out is when we receive the filled out form, is there any way to have it as a pdf. To clarify, when the form is filled out and sent, the receiver opens the email and attached is a non-editable pdf. This way the customers information is no longer editable.

    I work in a manufacturing company that tests each raw material as it comes in the door. The results from the tests are currently handwritten on a template form that is printed first. The sheets are then sent to another department who types up the results and saves them individually as a Word document. There is a HUGE paper trail and increased margin for error. Not to mention the sheer time!
    I am trying to streamline the process is a little by creating a PDF form in which the results can be entered and the form emailed to the other department. I know how to create the form and add a "submit" button which opens up the Outlook client. However, I want to make sure the form cannot be saved by the lab personnel, instead I want to give them the abilty to enter the data and email.Thisis an interim solution until a proper lab management system can be put into place.
    What is the best practice around something like this? Any other suggestions or tips will be appreciated.

    I would still need the PDF form completed and emailed over but if I can collect the data externally, that would be nice.

    Hi, so I'm new to acrobat (and adobe in general) and I wnated to know if anyone had a way of doing this. So what I want to do is to create a form that takes people's information. It will take things like Names, Phone numbers, emails, etc. Then, it will ask them to choose what kind of form they want to create. So, it could be a letter, or a memo, whatever. Based on their choice, I then want to make another form, with all of their information filled in for them, appear. I've made a form for the first part, but I would like to know if anyone could explain how to do the second part, where it creates the new page for me.

    If the end user is using Adobe Reader to fill this out then no, it isn't possible.
    I can say whether it would be possible if all parties are using Adobe Acrobat but I doubt that it is.
    You chould probably look in to creating a web application that can generate a PDF.

    We have a form that requires the employee to sign and the supervisor.  The employee and the supervisor are often at 2 different locations.  We want to use formscentral for the form.  We would like the form to go to the supervisor for their signature before it comes to our department.  Does echosign have a solution for form and signature process flow?

    If you are sending a document to be signed by multiple signers you will want to define the Role for the various fields. The Role is who is allowed  to interact with that field. By default the signers will sign in the order that their email addresses were entered into the To: field. It is important to keep this workflow in mind when setting up the signature fields.
    When a signature field is placed you can set the Role for that field (who can click to sign). Right click on the signature field for the 1st signer and select edit. This will open a window where you can define the options for that field. Select the 1st signers email address in the drop down list for the Role: field.
    Repeat this step to associate all the other signature fields for the other signers.

    I am needing to create a form that will display database information and also go out to a windows shared location and read a JPG image back into the form. I creating a Rolladex of sorts.
    My question is I am total 100% new to forms I have never created one or been shown how to create one can anyone recommed some place on the interenet I could get a how to maybe a sample that I could create and test so that I can begin the learning process.
    Or if someone is really bored today and wants to walk me thru the process that would be nice.

    Be aware that most of the documentation for Forms 9.04 and 9.0.2 apply to 10.1.2.0.2 in most cases. This may be helpful to know since there are more docs for these older versions.
    That said, one of the strengths of the Forms Builder is its Wizards. Without writing any (or at least very little) code you can create a basic application. If you understand the Oracle Database and have a basic knowledge in PL/SQL you should have no problem. Of course a working knowledge of how a web server works is also help since you will need Oracle Application Server to deploy your app if you ever go into production with it.
