Apart from the $1 temporary store holding charge what happens when you try to buy something ? Are you getting any error messages ?
The $1 will be charged each time that you add/change your card details, it's to check that the card details are correct and valid and that it's registered to exactly the same name and address as on your iTunes account - it should disappear off your account within a few days or so.

If you are unable to remember your password, security questions, don’t have access to your rescue address or are unable to reset your password for whatever reason, your only option is to contact Apple ID Support, upon speaking to an operator you should explain that your problem is related to your Apple ID, this way you will not be charged for assistance, even if you don’t have an AppleCare plan.
You will need to show patience with the procedure and be prepared to demonstrate without question that the account belongs to you. Don’t expect access to be restored immediately and if you aren’t the owner of the Apple ID registered to the device the account won’t be reset.

Go to the iTunes Store and select "Purchased" from the Quick Links side bar on the right. Go through all the tabs to download again for free

Are you talking about changing the keyboard layout when you are working in Windows? If so, you need to tell people how you are running it (bootcamp, parallels, other) and whether you have already done stuff like

You won't be able to change the rescue email address until you can answer 2 of your questions, you will need to contact iTunes Support / Apple to get the questions reset.

I was about download music when it then asked me for my security questions. The problem for me is that I've never answered these questions ever in my hole apple experience and even if I did, I've 100% completely forgot about them. I then decided to email a link to deactivate my questions however this email from Apple had never come, I'm know left unable to download music, apps, and books.

You need to manage your Apple ID and change your security question there.
