ORA-00476 RECO process terminated with erro

Question

When running the code below the 10g/Linux instance errors with
Error: ORA 476
Text: RECO process terminated with error
Cause: The distributed transaction, two-phase commit, recovery process died.
Action: Restart the instance
There's almost 21 million rows in tbl stage action. tbl v_map's empty before running this.

Can someone tell me what's wrong with this code? Why would it cause the error? Is it the COMMIT that executes 21 mln times?

I know...I didn't think it was the code that's causing this error, until it happened twice in a row in test environment and then in dev. Unfortunately, I don't administer the servers and don't have DBA access, but DBA told me that this has caused the servers to go down in all three cases.
