SIP reason = "This call leg has been replaced"

Hi All,
I am trying to troubleshoot an issue with calls being dropped, and have noticed that on some of my dropped calls I see the following in the BYE message:
ms-diagnostics: 10026;source="Lync.domain.local";reason="This call leg has been replaced";component="MediationServer"
ms-diagnostics-public: 10026;reason="This call leg has been replaced";component="MediationServer"
ms-endpoint-location-data: NetworkScope;ms-media-location-type=intranet
The call path is PSTN-->Gateway-->Lync-->Trusted application Server-->Agent
I cant seem to find much on what the "This call leg has been replaced" message means, if anyone can help me out it would be appreciated.

All,
I have now had some time to analyse the problem further. I have produced a more detailed way of reviewing the data. By using a custom SQL view (see attached) in the LCSCDR DB I can
now connect Excel to the database and using Power Pivot quickly breakdown the data and look for some form of trend. Now I can see a the precise time, client versions and affected users. I have compared this information with the Syslog outputs on the Audio
Codes gateway.
It seems that all the problems I have generally occur when clients are connected to only one of my front end servers. There does not appear to be a similar number of problems on the other one.
Can anyone else please confirm if they too have an Audio Codes gateway when suffering with this issue?
I now have CU6 applied to all of the Lync infrastructure. This has not resolved the problem but has reduced the frequency which the errors are occurring.
Does anyone else have any further thoughts on this?

    Make an appointment at the Genius Bar of an Apple store and press the problem. Elevate to store manager if necessary. Maybe the charging circuits are defective resulting in overcharging which is causing the battery problem.

    I had purchased an iPhone 5 last month and got the OS updated to iOS 7, yesterday.
    The data cable was of very bad quality and when I took it to the retailer for replacement, they said that its not covered under the one year warranty.
    Secondly, I have also been experiencing network coverage issues with this phone. So, I decided to contact apple care through the web. When entering my serial number/IMEI it says, "We're sorry, but this is a serial number for a product that has been replaced. Please check your information and re-enter your serial number. If your information is correct, you may need to contact us."
    Does anybody have any idea what the reason behind this could be.
    - First time with iPhone and frustrated.

    If you purchased the iPhone from Apple or an authorized Apple reseller,
    contact them for assistance. If you purchased elsewhere, go back
    to that retailer and get them to give you a new iPhone or your money
    back.
    If you have checked the serial number carefully and re-entered it
    and that gave you the same error message, contact Apple if purchased
    from Apple or authorized retailer; otherwise you have been scammed
    and need to pursue the matter with the business you obtained the
    iPhone from.

    In most cases, what you said is true...however, this is not always the case.  Take myself for example...
    1: I posted my phone online to sell
    2: Someone saw my serial number and called AppleCare to get a replacement phone
    3: An inferior AppleCare representative bypassed a component validation (check serial number to the device on hand) and approved the replacement under my AppleCare plan.
    4:  After noticing this (about 4 days after it happened) and  a trip to Apple and a call to AppleCare, all was resolved.  My phone is back to being covered under AppleCare+
    5:  I learned my lesson.
    So, as you can see, there is a chance that it can be something else.  Although, if the person selling the phone notices, it shouldn't take long to resolve it. Chances are that it is what you said.

    I bought my iPhone 4 couple of month ago and after selling my mac to get a new MacBook I wanted to sync it with my itunes... issue, itunes seams to be showing that no APP will be present on my phone once I sync it. So I wanted to check the support page... What a surprised, when I enter my serial number I got this message:

    What the H !? I bought the phone, brand new at an apple store probably 3 month ago or so.

    First, in regards to iTunes content an iPhone can be synced with an iTunes library on a single computer only, and photos can be transferred from a single computer only. When transferring iTunes content and photos from another computer, all iTunes content and photos transferred from a different computer will be erased from the iPhone first. An exception is if you transfer your iTunes library from the old computer to the new or different computer following the instructions included with this link.

    All iTunes content on your iPhone - all music, movies, tv shows, and 3rd party apps should be in your iTunes library on your computer. Did you transfer your iTunes library from your old Mac to your new Mac and if so, how was this done?
    If the 3rd party apps on your iPhone are not in your iTunes library on your computer which means you didn't have Sync Apps selected under the Apps tab for your iPhone sync preferences with iTunes on your old Mac, first you need to authorize your new Mac with your iTunes account with iTunes if you haven't already done so. With your iPhone connected and without syncing, at the iTunes menu bar go to File and select Transfer Purchases From - the name of your iPhone. This will transfer all iTunes content from your iPhone that was purchased/downloaded from the iTunes store that is not currently available in your iTunes library on your new Mac. After confirming all apps on your iPhone are in your iTunes library on your new Mac, select Sync Apps under the Apps tab and make sure all apps currently installed on your iPhone are selected below.

    When I try to log in to my iTunes store account i get the following message:

    I've waited a while and tried again (as in a week or longer), and the account is still disabled.
    Can anyone tell me how to get the ID enabled again, since as of now I'm unable to buy / download any content from the store. Which makes my iPhone a sad phone
    My assumption is the problem is i have a rather desirable account name: i am constantly receiving notifications of failed logins to my account, presumably by scripts trying to hack it. But i have a secure password, so no problem. But perhaps this has led to the account being deactivated?
    Any help / or reactivating the account would be great.

    I have had an iTunes account for years with no issue. Lately, I have been receiving the dreaded "This Apple ID has been disabled for security reasons" message every few weeks on my iPad 2(currenly, my only iOS device). I go through the process of reauthorizing my password and info and it works again for a while, then happens again some time later. It's getting annoying. I'm not sure what's changed. Only two computers are authorized to use my account and I am the only one who can log into either of them. No one knows my info and I live alone.
    The only thing I can think is that it's a bot spamming my e-mail address and passwords in an attempt to hack my account. If that's the case, am I really going to have to go through this every few weeks just to use my iPad? Don't get me started on how stupid it is to have to type in your password to update existing apps. And that's usually when I find out, not when I actually purchase one.
    Is anyone else having this issue? I'm wondering if it's more widespread. Some time ago, a chinese app was mysteriously purchased on my account. Apple refunded the purchase and reset my password. That was about the time that all of this started. The reviews on the app left no doubt that it was happening to numerous people and I have no idea how they would have forced a purchase on so many user accounts.

    Try this  - Reset the iPad by holding down on the Sleep and Home buttons at the same time for about 10-15 seconds until the Apple Logo appears - ignore the red slider - let go of the buttons. (This is equivalent to rebooting your computer.) No data/files will be erased. http://support.apple.com/kb/ht1430

    FYI, they do not want/need your IMEI number.
