User fill out form before document opens.

My company is looking for a way to make our customers sign an NDA before they are able to view our documents. This would be a simple form that can be electronically signed and gives access to open the rest of the PDF. The user would be restricted from opening the PDF until they have signed the form. We would like to be able to receive notification that the NDA has been signed and keep a copy for our records.
I've seen this before, but have had a lot of trouble finding a solution online.

Here's one way: make a PDF file with your form fields. Make a Button and set the Mouse Up action to Submit a Form.
In the Submit Form Selections, select the HTML radio button, then add the URL to a cgi script, plus "#FDF".
For example: if you had a PHP file that parses incoming data, then sends it with mail(); you'd use "http://www.myurl.com/emailme.php#FDF".
Then post your PDF file on your server.
If you know PHP well, you can even have the PHP file return a "Content-Type: application/vnd.adobe.xfdf" header and send data back to the PDF file, which can be displayed in a PDF field.

I am very new to Dreamweaver and this is likely a very basic question, but never the less, I can't find the answer on Google or in the Help. I want the form below to always be e-mailed to [email protected] and also to the e-mail address supplied by the user filling out the form. The user supplied email address will be in the text box id "SalesEmail" listed below. How can I accomplish this? The user filling out the form would be my salesman, so they need the information, as do I.

User being able to download filled out form in pdf format

No, respondents can only get the email with text responses. There is no way for them to get a PDF copy of what they filled out.

Formscentral doesn't support direct delivery of filled forms via email at this time. You can however use Acrobat to add an email link to a form instead of a submit button. In this way the PDF will be returned to the email of your choice.

I have created an interactive PDF form for a client's website. Her patients are able to fill out the form and submit it to her. She is able to receive the form, but cannot save it to her Mac using Adobe Reader for the Mac. She says it goes blank when she tries to save it.
I created the form in LiveCycle Designer ES 8.2. I double-checked in Acrobat 9 Pro, Extend Features in Adobe Reader, and it says "This document already has enabled usage rights in Adobe Reader".
Does my client need to have Acrobat in order to save the filled-out forms?
I will attach a test copy of the form.

Opening filled out forms on non Adobe programs (like Preview) causes info to not appear. How can I fix this?

I didn't distribute using FormsCentral, I sent my form out manually. I have now received lots of email responses with my filled out forms. How can I easily save these as individual PDFs that are readable and printable, and not missing an information?

yes we are using Adobe Reader / Acrobat on Windows XP / 7. We don't use a third party pdf product.
I have repeated the following YouTube video and the PDF file in the email is emty (you can see the form but without the filled in text):

Is it possible that the Adobe Product wants to connect to Adobe server to enable the feature if I save the file? We don't have a direct internet connection - we have to use a http proxy with authentication.

FF 29.0.1 bug - filled-out-forms

Hi, ever since I started using Firefox, I'd often have tabs already-open with already-filled-out-forms that would, every single day without fail, have my data retained (I'd fill out the form, hit the Back button, and the data would still be seen in the form. Every morning, I'd open the same tabs with the same data and the tabs would have my data prepopulated in the forms as I left it the day before). This was very important because I do some activity every morning and don't want to have to fill out my forms on a daily basis.
The 29.0.1 upgrade this weekend broke the preservation of this data. Now, I have to manually fill out the forms again. I do like the UI and UX upgrades, but why is it that this critical "feature" to me has been removed? The preservation of data was a paramount part of the Firefox experience for me.
If this is intended behavior, I'd like to hear it. If this is not intended behavior, I look forward to 29.0.2 fixing the issue. It was never a problem in versions 15+ up until 29.0, but 29.0.1 is a game changer for me.
(And by the way, I know there are ways around it, but having to manually click on autocomplete buttons or use form filler tools defeats the purpose of this already existing up until this version.)

tamarsaw, <br />Yes. That or something similar is easy to do. I can understand you would rather just use Firefox than have to try troubleshooting it but the effort and results should pay off in the longer term.
It is just a matter of transferring and overwriting files in the new profile. It is also possibly worth keeping backup copies maybe put a spare copy somewhere safe such as the Desktop or Documents.

Remember you could for instance have the current profile retained for historic reasons. A totally clean profle for testing purposes, and a clean profile but with current open tabs and history/bookmarks.
The new sync is worth using that will keep backups on a server. However it has sometimes been unreliable and troublesome in the past it would be wise not to become over-reliant and rely on it totally for anything very important. When you start Firefox from the profile manager you choose the profile to use. Or you have icons/shortcuts that use specific profiles

Exporting or saving the filled-out form to a specified location.

I have created a pdf form using Acrobat X Standard. Currently, the way I have designed it is- the filled out form is attached to the email when the user clicks submit button. I am using some java code that I found through this forum. I do not know much about java.
I want to change this. In addition to emailing the completed form, I would like the form to automatically get saved to a pre-defined location when the user cliks submit button. Is there anyway I can do that. The problem I see in this is the file name. How would that be handled?
May be I am asking something impossible. But your thoughts would be appreciated.

It doesn't mention though that it can be used in a certified document, in which case the trusted function stuff isn't necessary.

Attaching a filled out form

How can I attach a filled out form to an email? Currently my email button attaches the form as an XML. The end user needs to file the filled out form to a file but in it current XML format does not make it user friendly reading as if it was in the pdf format.
Any ideas would be helpful.

You can also send the entire PDF. Here are the steps:
- Open the Form in Designer
- Add a button object to the form
- Update the caption to something like 'submit the form' or 'email the form'
- Change the Control type of the button to Submit
- Click on the Submit tab and choose PDF for the 'Submit as' type
- update the submit URL to mailto:[email protected]
*Please note that the user submitting the form will need Acrobat (or) you can Reader Enable the form if the user only has Reader. This is required since Reader can't save PDF by default.
Usage rights can be added using LiveCycle Reader Extensions (or) you can add usage rights in Acrobat (advanced > Enable usage rights in Adobe Reader). *Please note that if you use Acrobat to Reader enable the form, there is a licensing restriction on the number of users that the form can be sent to.

When you fill out a filled out form in Adobe Reader, it is saved automatically when you quit the app (unless you crash). You could email the filled form, and it will open correctly filled out on Reader or Acrobat on Mac or Windows. (Warning: It may not look like it got filled out if you view it in Mail, Dropbox, or another iOS app, but it will appear correctly in Reader or Acrobat.)

I'm fairly new to APEX and I need help with a few things based on forms.
Firstly, I would like to automatically retrieve a foreign key ID from the database to enter it into the form. I'd like it to be the latest ID created by the current user if that makes sense.
secondly, for one form I would like it to automatically complete the form based on the first entry if it already exists. E.G. I go into the site and enter my personal details, then I make a request. Next day I want to make another request, I would like to be able to enter my User ID and the form to fill out automatically so I don't need to do it all again (I still want it to be re-submitted to create a new ID).
and then lastly, I would like to know if its possible and if so how to delete data from the database from 2 or 3 different tables when the cancel button is clicked. I'd like to have the user fill out 2 different forms on different pages, then on a third page have them confirm and when the confirm button clicked an email sent but on the third page if they decide to cancel then i'd like it 2 delete what they've just entered.

Coding for static fill-out form

I am creating in Flash CS4 (AS3) a static fill out form for a website that I am involved in. The questionnaire helps to determine whether a person is suseptible to drug and alcohol abuse with either a postive answer (not likely to develop substance abuse) or a negative answer (likely to develop substance abuse)
There are six questions on this form that MUST be answered either yes or no.
I have created the form with all art and buttons and it is ready to implement in Dreamweaver but need the scripting to make work.
The six YES buttons are assigned instance names of cb1, cb3, cb5, cb7, cb9 and cb11
The six NO buttons are assigned instance names of cb2, cb4, cb6, cb8, cb10
and cb12.
When the participant answers all six questions either yes or no they will hit a Submit button (with an instance name of submit) that will direct them to a new web/flash page with appropriate answer.
If the participant answers TWO or more questions with YES and hit submit they will be directed to a new page/doc in the website with the negative answer.
If the participant answers ONE YES and hit submit they will be directed to a new page/doc in the website with a positive answer.
If the participant answers ALL NO and hit submit they will be directed to a new page/doc in the website with the same positive answer as answering with ONE YES response.
I assume this is a very complex coding problem. I am a beginner/intermidiate flash person with extreme novice understanding of coding. I know what stop(); does in AS3 and understand simple clickTag coding etc.
Would appreciate any help with AS3 coding for this document.

Save a filled-out form to Word - saved as blank

I purchased ExportPDF so that I could save a filled-out form to Word. I needed to do this because, though the form has fields that allow seemingly infinite text to be inserted, the text is not visible when printed. The document must be submitted by US Mail. When I converted the completed document to Word, it erased ALL of the text I added and unchecked all boxes. If this is all it can do, I need to unsubscribe and get a refund. That is totally worthless to me.
