Voicemail Unavailable - Fixes aren't working

So I 'waited a bit' to upgrade to iOS4, so that hopefully the 'kinks were worked out'.
I upgraded recently, and now I'm having the 'Voicemail Unavailable' issue.
Searching these forums, I came across people a month ago having this issue, and it seems that most people were able to solve it via the 611 call reset, a website reset, or a 'dial 1 and set it up/reset', or even a 'remove password' via dial 1.
I've tried all these.
On my wife's 3GS, she also had the issue, she dialed 611, 1, 3, 3, got her password reset, and it just 'instantly worked' for her.
When I go through that same process, My voicemail screen goes to the 'Please setup your voicemail'. But the 'play' bar is still there as well. When I then click the 'setup' text, it asks me to set a password and greeting. But when I try to save the greeting, I get the dreaded Voicemail Unavailable.
Given the lack of any mention of this issue for the last month on this forum, am I unique to be having this issue still? And for none of the suggested fixes to appear to be working?

A small reply to myself. I just updated to 4.0.1 to see if it fixes the problem.
Now the 'playback bar' doesn't show up, so it acts like my wife's phone.
But upon clicking 'save' after setting a greeting, I get the Unavailable message. Ugh. Help?

IOS 7 install keeps coming up with error 3004 - Fixes aren't working, any thoughts?

I've tried installing ios 7 a few times now - it downloaded fine but when trying to install it is coming up with Error 3004. The suggested fixes for Error 3004 don't seem to be working. I made sure that my iTunes was up to date before downloading the ios update.
I'm also worried that I've now made the problem worse by letting it try to update to ios 7.0.2 this morning, which obviously didnt work because it didnt have ios 7 to update from....
Would love any assistance to fix the problem!

Can you check that your Wi-Fi connection is working correctly?
Do you get that error right away at the beginning of the download or much later?
Have a look whether there is plenty of free space (several gigabytes) on your iPad to receive the new iOS 7 upgrade. Remember that the upgrade is downloaded in a compressed form.
It's difficult for me to give precise advice without knowing what caused the error.
If all fails, try to do the upgrade via iTunes with your iPad connected to your computer.

I tried this and still have the same problems. If I open Address book or the Mac App Store I get the library rebuild popup and a hang. I created a fresh user and then things are better apart from the printing issue. Its clearly some 3rd party software issue but the crash logs are meningless to me. I used the excellent Etre check app from http://www.etresoft.com/etrecheck so I have a list of what is being loaded and can compare clean and crashed user info but as I cant work out which of the startup items, launch agents, launch daemons etc that are causing the problem.
I have eliminated Dropbox, Mac Keeper and Witness, and it isnt related to my exterrnal LED cinema dispay or my external thunderbolt drives, or any USB devices if thats any help to anyone else. Info from Etrecheck follows -

USB Ports Aren't Working

I have a HP pavilion g series laptop. I'm not sure what it is or anything, that's all I know about it.
But anyway, my USB ports aren't working.
I tried plugging in my mouse and i tried plugging in my xbox 360 controller, the lights come on for a few seconds on both of them, but then they went off and my computer won't pick them up.
What do I do to fix it this problem?

Try to go into device manager and see if you have any yellow !s or red triangles near the usb ports. Remove all the usb ports and hubs and reboot the machine. The drivers should reload automatically. Do the hard reset by removing the battery, unplugging the power adaptor, holding in the power button for a full minute, putting the battery and power adaptor back in and try it again.

I can't set up my voicemail on my iPhone 4. I get a message "Voicemail Unavailable". All other functions are working properly on the phone.

I hate to confess, but it was as simple as Mail being turned off in Settings/iCloud. Don't know how it got that way, but it did. Reminds me of the old saw, when your computer won't start, first check to see if it's plugged in.

HP HP 15-r006la speakers aren't working

I recieved my new laptop but the speakers aren't working. Volume mixer shows activity even though I can't hear anything. When I plug-in my headphones, they work just fine.

I understand that you are getting no sound from your speakers, but that headphones work fine. I am happy to help.
I assume you are referring to the internal speakers on the notebook, if I am wrong let me know. With that in mind please take a look at this page:

Ipad mini (iOS 6.0.2) wifi fixes don't work

please don't reply to this with the fixes that are already posted a zillion times in these forums. they aren't working for me (and it looks like a lot of other people). my question is what to do once we're beyond all those. i received an ipad mini for xmas, and it's running iOS 6.0.2, which is supposedly better than its predecessor (how that can be, i'm not sure). i love this device enough to continue to put up with the issues, so returning it is not an option. however, i'm hopeful that someone out there has talked to a mac genius and discovered some trick to keep the wifi connected for longer than a second or two intermittently.
if that's not the case, then when can we expect 6.0.3 or 6.1 to actually resolve the issue? or (gulp...) is it hardware related?

Since the vast majority of iPad mini users are not having WiFi problems then, if you are having consistent WiFi problems on all networks, you very likely have a hardware problem with your iPad mini. This probably cannot be fixed by any iOS software update. The iOS 6.0.2 software fix likely was for a bug that only showed up with certain access points or network configurations. The only way to get your problem fixed is to get your iPad mini replaced by Apple. The fastest way to do this is via the Apple store but you can also arrange for replacement if you contact Apple support.

the default keyboard shortcuts aren't working at all. Even when I go in to change these, it won't allow me - so there seems to be an error occ
urring somewhere. Would anyone have encountered this before and know how to resovle it?
Also - when I'm typing in a text box, the return key won't work (usually hitting the return key allows you to move to the next line). Does anyone know how to fix this?

Creative Suite 4. What do you mean by OS? - I work on Windows Vista PC.
When I hit the enter key nothing at all happens - the cursor doesn't move at
all.
None of the shortcut keys work at all. As I've only just done a training
course in In Design, I've only had 2 days of actually using it! so I don't
know many of the shortcut keys - but any that I have tried don't work e.g.
CTRL & Z, CTRL & W etc.

My headphones aren't working on macbook.

My headphones aren't working on my macbook. They work on other things, but whenever I plug them in now the sound comes out of the computer speakers. I was just trying out a new outside microphone when I noticed that the audio wasn't working in my headphones, but only on the computer speakers. I unpluggled the microphone and even restarted the computer, but the sound still comes from the computer's speakers and not the headphones. How do I fix this?

    "Resetting SMC on portables with a battery you should not remove on your own".
3. Applications > Utilities > Audio MIDI Setup
    Audio Devices window
    Side Bar
    Click the Built-in Output.
     Plug in the headphone. Try to adjust the volume slider.
    Under Mute any of the boxes checked? If so,uncheck it.

Sleep/wake button and volume buttons aren't working..

iPhone 3GS.. The sleep/wake button and volume buttons aren't working.. Can't shut my phone off, can't put it in sleep mode, can't higher or lower volume or can't put phone on vibrate. Does anyone know how to fix this? It just stop working out of the blue.. I tried to reset my phone to network setting but that doesn't do anything for my problem and I am not able to restart phone by hold sleep/wake button and home button.. Try the standard fixes to rule out a software problem:
- Reset. Nothing is lost
Reset iPod touch: Hold down the On/Off button and the Home button at the same time for at
least ten seconds, until the Apple logo appears.
- Restore from backup
- Restore to factory settings/new iPod
if still problem that indicates a hardware problem and an appointment at the genius Bar of an Apple store is in order.
You can also turn on Assistive Touch in Settings>General>Accessibility to add those buttons to the iPods screen.

Notifications on my iphone aren't working

Notifications on my iphone and ipad aren't working, even though I have triple checked the notification settings on both devices. The settings are as they should be, do not disturb is OFF, and still I don't receive notifications until I manually open apps.

Both my usb ports aren't working, Please help!

I bought a 500gb external hard drive that is usb powered. it was working fine a week ago but now i get a message saying a device draws to much power, therefore all my usb ports aren't working for anything any more. Not even a simple wireless mouse. I am using a macbook pro late 2011. also.....when i use the usb port closest to the charging port, the lights on he external hard drive turn on (yet i still get the message). When i use the other usb port, no lights turn on at all. does this mean one of them is just ruined completley??.

It actually did. After I uninstalled java and restarted my computer for the fifth time, the usb ports actually responded! I re installed java and the problem was fixed.

What can I do about My iPhone 4 speakers that aren't working?

I bought my iPhone 4 in November of 2010 and the phone fried before my one year warranty was up. I took it to the apple store and after they found no solution they gave me a replacement. I am wondering did my year start over or not? My speakers aren't working I'm not sure if its in the warranty to fix broken speaker but, I'd like to try because I cannot play the wonderful songpop or talk on the phone. Please help

The Warranty duration is the Warranty of the Original iPhone. If that Warranty has now lapsed, then the replacement has a 90 day Warranty from when you received the replacement iPhone.

After upgrading, multiple applications aren't working properly

I recently upgraded from OX 10.3 to Tiger, and a lot of my applications aren't working properly. My computer is almost exactly 2 years old (I bought it in early January 2005) and otherwise has been running fine.
When I use Safari, every time a page loads a box comes up saying, "Keychain Not Found. Keychain "mcurtiss" cannot be found to store "Safari." I've tried clicking both cancel and reset to defaults, but either way it still keeps happening. I've checked under Keychains and the mcurtiss keychain is there, so I don't know what else to do to fix that.
I also have Firefox, but now that isn't working properly either. It keeps freezing and I have to Force Quit (at which point it's written in red text and says "Not Responding). This happens at least 2-3 times every time I try to use Firefox.
DVD Player is doing basically the same thing-- working very slowly or freezing-- and I have to Force Quit it as well. It also says "Not Responding" in red when this happens.
It's happened a few times with other applications, but with those three it's basically all the time. Can anyone give me any ideas of how to fix this?

Spotlight among other things will scan the bookmarks and add them to your list of bookmarks. To tell if Spotlight is still indexing, go to its menu (the blue circle with white magnifying glass in the upper right), and see if you see anything other than Spotlight and the search field when there is nothing in the search field telling you that indexing is happening. If you search for a known file and it isn't visible in Spotlight that could indicate your Spotlight index is corrupt. To reindex Spotlight, go to Apple menu -> System Preferences -> Spotlight, and remove and add the hard drive to the Privacy section.
If you are getting errors which can't be fixed with Keychain First Aid, and the problem seems to be especially bad with Safari, it suggests that the file that manages you cookies has gotten corrupt. How full is your hard disk?
Is your data backed up?
