Weird Xsan crash

Hi,
We have recently seen a very strange volume stop with Xsan 1.3. It seems that a few "panics" have forced the volume to eventually stop after 60 minutes. After manually restarting the volume a few times, it did not stop anymore and the problem did no resurface.
The issue is that we'd like to know what could be the cause of this crash and what we can do to make sure it will not happen again (with 7 editors online, these problems cause a lot of trouble).

  • Weird OCCI crash when creating a connection

    Hi Forum Users,
    I wonder if anyone experienced weird occassional crashes deep inside the OCCI / OCI like the one described below, and have any clues how to avoid them?
    I work in a environment where my backend server connects to a load-balanced cluster built on Oracle 11.
    The server uses OCCI to connect to that cluster, the connection is resolved by service name and tnsnames.ora. The OS is RHEL5.2 running on kernel 2.6.18-92.el5 #1 SMP.
    The Oracle version is: Oracle Database 11g Enterprise Edition Release 11.1.0.7.0 - 64bit Production
    My server uses libraries (an excerpt from ldd's output):

    Normally the server runs fine, connecting to the database every couple of minutes.
    Occassionally however, I see crashes deep inside the OCCI while creating a connection; all parameters passed to the createConnection are fine, and this crash occurs usually after a couple of days of running and hundreds of connections made; the server is a multi-threaded application so the Oracle environment object is created with the THREADED_MUTEXED type. But threads do not contribute to the issue as I have once seen the same crash at the very beginning, with the 1st connection, when there were no threads yet.
    The pattern for the crash is always the same, and the application logs the parameters used in a call to createConnection before calling it and they are always OK.
    Nevertheless I made several tests, passing garbage there and it never gave a crash - always an error message (even tried messing the string and using a deleted environment - always without a crash).
    Weird thing is it happens so deep in the call chain, that's why I kind of doubt it is my server's failure.

  • FileVault 2 bootup login gets weird after crash

    I have had FileVault 2 encryption enabled on my laptop for almost a year. It's worked well and I've never had trouble... until yesterday.
    My computer froze while I was saving something in Photoshop and had various other apps open. Nothing unusual. This happens once in awhile so I wasn't troubled by it. I have a TechTool Pro (TTP) emergency boot partition installed, so I booted off of that and ran Disk Utility and TTP to check the encrypted disk. Of course, I had to enter the disk's password just to mount it on the desktop.  Disk Utility said the disk was fine other than a few permission issues. I rebuilt the volume directory in TTP, as I usually do after a crash just for precaution, and it didn't report any negative or unusual issues.  Then I restarted back to my main partition.
    Instead of the usual encryption login screen appearing, I got this:
    I added a white box over the login name that was under the icon; it was my username printed there.
    This looks like a weird combination login screen of the encryption login and the usual non-encryption login. But I couldn't do anything. I could move the mouse around, but I couldn't click on anything. There was nowhere to type a password. Finally I tried typing the password anyway, and the screen changed to this:
    Again, I've whiteboxed the username and now a snippet of other text: my password hint. But you can see around the password hint are rows and rows of colored noise.  It didn't change at all -- e.g. it was not video noise -- though if I clicked or pressed a key, it would go away, and then come back with another keypress.  This was really confusing.  I had just accessed this disk a few minutes earlier!  And I'd checked it by booting via another partition, with no problems.
    On another computer, I googled possible issues and looked through the Apple support resources. I didn't see any situations quite like this, but I did get the idea to check the encryption.  I booted the troubled laptop into Recovery Mode (command-R) and turned off encryption via Disk Utility.  After that, the computer booted directly to the usual non-encryption login screen without problems, and worked fine.  It took about 3 hours for the system to finish decrypting itself (per the Security preference status) but there were no problems.
    After it decrypted, I ran all my disk utilities again (no problems found!) and tried enabling FileVault 2 again. When you enable it, you have to restart. I did that, and... it gave me the same login weirdness described above. EXACTLY the same. So I booted back into recovery mode and disabled encryption again, and everything is working fine (other than not being encrypted as I want).
    Does anyone have suggestions for what is going wrong with FileVault 2?  Is there a config or preference file somewhere that I can delete or edit?  A flag that needs to be set via command line?  A disk driver that can be reinstalled without wiping the disk?  I do not relish the idea of reformatting the drive and restoring it via Time Machine backups, though I would consider it as a last resort.
    Any suggestions will be appreciated. Thanks in advance.

    Update:
    On a hunch that the problem was somehow with a configuration related to the login screen, I went into the Users & Groups preferences, under Login Options, and turned off everything.  I set the login window display to "Name and password" instead of "List of users". Then I went back to the FileVault preferences and turned it back on. I followed the steps and restarted the computer.
    It rebooted and spun on a grey screen for awhile.
    Then it rebooted again, right back to the non-encrypted login.
    I logged in and checked the FileVault preferences. It indicated that encryption was off, even though I had just turned it on.  Odd.  So I turned it on again, restarted, etc.  THIS TIME, it booted correctly to the encryption login, with none of the weirdness of before, and FileVault's preferences indicated that it is in the process of encrypting.
    I'll keep an eye on this for a day or two. If it seems resolved, I'll mark this resolved.

  • Weird random crash

    When i am playing games, sometimes i get a random crash. The crash is like the game freezes, the sound starts repeating really fast, almost like a buzz. The only thing to do is hit the reset button. Once i hit reset, the post code indicates a video error, (ami bios, 1 long 2 short). But if i power off, then power on the post screen comes up and says "The previous performance of overclocking is failed" (even though i was not overclocking!!!) I then hit space bar to go into bios, save bios and exit, and everthing is fine.
    This is generally during games, it started with far cry 2, i thought it may have been an issue with the game, but then it showed up in crysis warhead, the witcher, and most recently arkham asylum among others. In arkham it seems to happen at the end of the combo when it goes into slow motion knocking down the last bad guy. But other than that i havn't found any patterns.
    it would also happen whenever i tried to update the same game with the ingame windows live update.
    this seems to have appeared after i changed processors, but ... read on
    I am fairly confident it is not overheating, all system temps remain stable and system is well cooled.
    I have stress tessed the cpu with 4 windows of cpu burn in, for around a day long, with no issues, also stress tested with prime 95 for a long (approx 20 hours ) period with no issues and good heat dissipation, so it makes me wonder if it is the cpu?
    I have stress tested the video card with ati tool, video card stress test, furmark ozone3d, and 3dmark 06 with no issues. furmark raised the temp way higher than any game has and nothing wrong.
    I updated motherboard bios thinking maybe it was an issue that had to be straightened out with the chipset, but no difference. (before i update i had ami v1.8, and instead of 1 long 2 short, it was 8 short beeps, but both are video card related)
    all drivers are up to date..
    tested memory for 20 + hours with memtest 86+ and windows memory diagnostic, both reported no errors.
        * Motherboard brand & model- MSI k9a2 neo-r
        * CPU model (and is it overclocked?)- amd x4 940 be (at stock)
        * Video card(s) evga 8800 gt, 512mb
        * RAM modules brand & model patriot ddr2 800, 2x 2048
        * Power supply brand & model silverstone st50ef plus 500 watt
     sound blaster audigy 2 zs....
    I am going to swap the 5000 back in, and see if it still occurs, since it showed up when i change to the 940, but just wondering if anyone had any input or seen anything similar before.  But the bios beeps referring to video problems also make me wonder, maybe the board itself can't handle the cpu.

    Ok, so I think i have made some progress, it doesnt seem to be the cpu.. but it is the video card.  I did a few more stress tests and noticed that the card (using furmark) was reaching 100+ degrees C.( while cpu stayed in mid 30's)  Around 105, boom, computer crash.  I didn't think I was reaching this high of temps in games though, so I need to find a way to monitor the gpu temp in games. It is kind of weird that this started with the new cpu though.... but i put in a new cpu fan as well, I added a xigmatech hdt -s1283, so maybe it changed airflow or something?  I need to explore this a little further tomorrow.  Will also apply some mx -1 paste and reseat the gpu heatsink.  and improve case airflow if possible. 
    This is kind of puzzling still, because from alt-tabbing in and out of games, or looking at evga precision after a session, the highest gpu temps are typicall less than 80 under load in games. 

  • Weird MAC Crash; anyone who can decipher the log for me?

    My Macbook Pro (mid 2009) with a recently clean installed Mountain Lion experienced a weird crash. I have never had one like this: suddenly the screen turns white and informs that the computer has been restarted due to a problem. As I restart, the log below pops up.
    I had several software running behind: Safari 6.0.2, VLC media player, Microsoft Mac Outlook, Soulseek Mac, and Text Edit.
    I have no idea how to decipher this or where to identify the problem. Is it a virus? (I have firewall ON) Or worse: is my hard disk dying?
    Any help in troubleshooting this appreciated!!

    That panic was not caused by third-party software. If it's happened only once, and is not repeated in at least a few thousand hours of use, a reasonable choice would be to ignore it. Machines don't work perfectly. They never will.
    If the problem is recurrent, the possibilities are (1) a damaged OS X installation; (2) a fault in a peripheral device, if any; (3) an internal hardware fault; and (4) an obscure bug in OS X.
    You can rule out the first two possibilities by reinstalling the OS and testing with non-essential peripherals disconnected and aftermarket expansion cards removed, if applicable. Sometimes a clean reinstallation (after erasing the startup volume) may solve a problem that isn't solved by reinstalling in place, without erasing.
    If you've recently upgraded the memory, reinstall the original memory and see whether there's any improvement. Be careful not to touch the gold contacts on the memory modules when handling them. If necessary, clean them with a mild solvent such as rubbing alcohol.
    The Apple Hardware Test, though generally unreliable, will sometimes detect a fault. A negative test can't be depended on. Run the extended version of the test.
    In the category of obscure bugs, reports suggest that FileVault may trigger kernel traps under some unknown conditions. Most, though not all, of these reports seem to involve booting from an aftermarket SSD. If those conditions apply to you, try deactivating FileVault.
    Otherwise, make a "Genius" appointment at an Apple Store to have the machine tested. You may have to leave it there for several days. There isn't much point in doing this unless you can reproduce the panic, or if you can't, it happens often enough that it's likely to be repeated at the store. Otherwise you may be told that nothing is wrong.
    Print the first page of the panic report and bring it with you.
    Back up all data on the internal drive(s) before you hand over your computer to anyone. If privacy is a concern, erase the data partition(s) with the option to write zeros* (do this only if you know how to restore, and you have at least  two independent backups.) Don’t erase the recovery partition, if present.
    *An SSD doesn't need to be zeroed.

  • Xsan crashed. Anybody at Apple interested in the core dump?

    Dear all,
    my Xsan system has a very bad crash yesterday. I've two core files now:

    We intend to buy AppleCare for the XServe machines, but haven't done yet, as we've bought it in September only. And the unversity's money is allways seperated in fiscal years. I assume (and I've been told by the dealer), that there's a one year warrenty, isn't it?

  • Xsan crash

    Hello. I have two Xsan clusters. One cluster has four Xserve and one Xraid. That cluster has been cleanly installed with Mac OS X 10.5 and Xsan 2.0. The other cluster has four Xserve and two Xraid, and has been upgraded from Mac OS X 10.4 and Xsan 1.4 (?) to Mac OS X 10.5.8 and Xsan 2.1.1.
    I just updated the clusters with the latest softwareupdates. I ran the command "softwareupdate -i -a" and rebooted "/sbin/shutdown -r now" the first cluster. That went fine. I then ran the same command "softwareupdate -i -a" and rebooted the second cluster. That did not went fine, the metadatacontroller and two client nodes stuck on booting with "panic version mis-match between kernel and cpu". The backup meta-data controller booted fine.
    This is what I have tried:
    1) I tried to reinstall the primary meta data controller on a spare drive. It couldn't access the data on the Xsan at all, in Xsan admin the Volumes and Mounts are gone, however, the LUNs are there. I may try to create a new file system from Xsan Admin but my guess is that i wipe the data from the old LUNs then.
    2) I tried use the not crashed secondary meta data controoller. Same as above.
    This is what I will try:
    3) Try to check if there are any fancy command line tools to scan the LUNs for a file system without creating new ones.
    4) Try to reinstall the software update patches again - boot on the spare drive and install the patches on the crashed drives to try to fix them.
    Do you folks have any other ideas?

    Hello. I rebooted the MDC first and when it did not boot I waited. But then a power failure rebooted the other machines for me so the only one up was the backup but not working backup meta data controller.
    However, now I have access to the file system again. I solved it this way:
    * Install a fresh Mac OS X 10.5.8 with Xsan 2.1.1 on the MDC on new drives.
    * I mounted one of the mirrored systems drives at the MDC too.
    * I copied all relevant files from /Library/Filesystems/Xsan/config to the new MDC - MyFileSystems.cfg config.plist automount.plist notifications.plist fsnameservers fsmlist.
    * I set the correct IP-adress on the internal meta data ethernet (so the IP-adresses in the configuration files match the real ones)
    * I rebooted the MDC.
    I have no user name resolving (since I have not joined any directory service like OD/AD/LDAP etc) but the files are there.
    I smiled when I found out that the command line tools are all starting with the letters cv, which do not relate to Xsan, not even StorNext but the original CentraVision file system. One may change the marketing name of a file system, but the roots stay.   • Itunes acting really weird then crashes

    When i'm using itunes to update my ipod touch and add songs sometimes it will act weird and the fonts all change and stuff disappears then itunes crashes. I have the newest version of itunes

    Thanks for the quick response.
    I did just that. Deleted the com.apple.dock.plist under my user home directory/Library/Preferences.
    While it reset the dock, the problem still remains. You can view the problem by
    1. Setting the dock to the smallest possible size via the Preference Pane
    2. Turn on magnification and bring it closer to the left side of the adjustable bar
    3. Now, use your mouse to drag the dock by the separator
    Observation: The dock will become smaller than is "set" by the bar in Preference Pane
    4. Click on the bar in the preference pane, set at small, the dock will resize itself again
    5. Choose the dock to be set on the right
    6. Choose it to be set at bottom
    Observation: The dock is a LOT smaller than it could ever have been.
    7. Click on the smallest possible size in the preference pane, the dock resizes itself
    8. The dock keeps getting smaller over time still. =(!

  • Weird system crash when trying to install Photoshop CS2

    It gets to "Running installer script" and then nothing. I only have a movable cursor at this point. I can't even force-quit. I'm left to force a shutdown with the power button.
    I've tried Hardware test and the computer passed. I check the disk in disk utility - was fine. I even tried copying the photoshop disk to the HD and run it from there... crashed the same way.
    Anyone have any ideas?

    Frank, not sure if this is quite your problem, but there is a known issue with Photoshop CS2 and Intel based macs after the last security update. If you install Photoshop CS2 then the computer will no longer boot properly.
    The solution is to start up in "safe" mode (by booting the Mac with the shift key held down - this may take a while as "safe mode" involves a full directory check and repair if necessary) . Once it has booted remove Adobe VersionCueCS2 from /Library/StartupItems.
    If this is not done then the only way to boot the computer is by Netboot, CD boot or Safeboot.

  • Weird IOs crash with SpriteVisualElement

    Setup: FB 4.7, Flex Mobile project, AIR 3.5
    I'm making a IOs app mostly using starling with a classic display list screenmanager (extending SpriteVisualElement) on top to show some UI graphics. Everything runs great execpt when exporting a ad-hoc version or compiling with standard instead of fast. Any removeChild related method call inside my screenmanager even called by children causes an immediate crash. Changing my screenmanager to extend Sprite instead and adding it to the stage directly fixed the problem.
    Is there something a SpriteVisualElement is supposed to do to avoid this or is it a plain and plainful Flex bug?

    Have you tried rebooting or resetting the iPhone yet?  Might help if you haven't given it a shot already.

  • Weird Dock crashes

    Just a quick note regarding some random Dock crashes you may run across. It seems that if you have any sort of animated desktop, such as having the Desktop and Screen Saver perfpane change the desktop picture at regular intervals AND you have the Dock set to hide automatically AND you happen to be accessing the Dock when the desktop changes, the Dock will crash and reload. The event gets recorded in the console, system and CrashReporter logs.
    No biggie, just that your Dock and Finder will freeze for a few seconds till the former reloads. Also, and this applies whenever you get several consecutive crashes of any component of the O.S., I prefer to restart the system entirely just to clean out any crud left behind by the crashing apps. Be sure to carry out HD maintenance at regular intervals, as well.

    What are we looking at BigD? Your link doesn't show your Dock.
    When you say "every time I have to force quit a program or if a program crashes" that sounds like you have deeper problems, though possibly connected.
    FYI, I have had to force quit no than a handful of times in several years, so you need to get to the root of this problem first, and be less worried about the related ghosting remnant in the Dock.

    So i have iphone 4.When my phone notifies for smth like battery % or iCloud backup after i unlock it springboard crashes..the part that ****** me of is that when it crashes for battery i have charge it or else im stuck in safe mode...i have a broken home button so in safe mode without activator my phone is complety useless...Any thoughts?

  • Weird: safari crashes admin acount not others after reinstall

    Hi,
    got a problem. giving my old G4 400 to my brothe. rather than go through all of the files I just wiped the drive. Then reinstalled my version of 10.2. repaired disk permissions. updated per disks to 10.3. repaired disk permissions. web update to 10.3.9. repaired disk permissions.
    Now Safari crashes when launched and affects other programms (they won't start) but safari will actually start 2nd time around.
    then i created another user acount and that seems alright.
    also disc permission repair claims to repair keeps correcting the same permissions every time it runs. also tried running disc utility from install disc. same.
    reinstalled operating system (as described above)
    same.
    any suggestions?
    the mac used to run 10.3.9 fine for a long time.

    I've been having the problem stated below (which I asked Amethyst18 in another topic "Re: Safari won't launch"):
    Can you expand on the new user accounts? I'm having trouble with Safari (it launches but can't do anything else except for Quit). I tested Safari using a new user account,and it worked fine. So I expect by what you've said here, that I'm having some sort of trouble with my main user account?
    I want to be careful not to create a more difficult situation as you've mentoned in your 3rd paragraph below (...which .plist files are causing my conflict).
    Anyway, I've been searching for a solution, and the one you've mentioned below works fine! So it was my Safari Preferences file that was the issue.
    If I copied my bookmarks files to the desktop, can they be reloaded into Safari so that I have the same UI of bookmarks and functionality (except for the Preferences, I realize) that I was using before I had this problem?

  • Weird LabVIEW crash

    I wonder if anyone can help me with a problem I  am having right now.
    My application has several free-running loops on the main VI (logging, UI etc etc).
    I am getting strange crashes where some of the loops seem to hang, while others continue. For example a subVI will hang, and then the main UI seems to become unresponsive but the logging loop happily carries on. The logging loop does not have any UI component to it, so is it possible that I am somehow causing the UI threads to hang?
    One of the free-running loops (shown in picture) updates an elapsed time counter on the main UI, this is how I know this particular bit of code is freezing. Any ideas why? 
    I can't really post the rest of the code unfortunately.

    1) make sure you are using the latest a greatest version of the VISA driver. There was a bug in one version that would hang teh UI thread.
    2) "Non-NI encoder card". If you are using dll calls to interface with that widget, check what color the node is. If I remeber correctly non-thread safe dll calls are orange be default indicating they run in the UI thread.

    You were right in the DLL calls were orange (something new I learned today, thanks again!).
    I am using a (rather old) LV wrapper driver provided by the manufacturer. I see that in configuring the DLL call there is now a radio button to select whether to run the function in the UI thread or any other thread. Do you know if changing this (it was on UI thread and hence the orange node) may help or does this thread-safe behaviour need to be compiled into the DLL?
