Cannot remember ipod passcode
Tried to restore the ipod to original factory settings, won't restore, error code 3194. According to apple's online support, I updated to the lastest version of iTunes and I still get the same error code. I'm completely lost as what to do here
The troubleshooting for that error contains more actions than just updating iTunes. See the actions for 3194 here:

Forgotten Screen-Lock Passcode
Connect the iOS device to your computer and try to make a backup
iOS: How to back up
Then restore via iTunes. The iPod will be erased. Place the iOS device in Recovery Mode if necessary to allow the restore.
If recovery mode does not work try DFU mode.
How to put iPod touch / iPhone into DFU mode « Karthik's scribblings
For how to restore:
iTunes: Restoring iOS software
To restore from backup see:
iOS: How to back up
If you restore from iCloud backup the apps will be automatically downloaded. If you restore from iTunes backup the apps and music have to be in the iTunes library since synced media like apps and music are not included in the backup of the iOS device that iTunes makes.
You can redownload most iTunes purchases by:
Downloading past purchases from the App Store, iBookstore, and iTunes Store
You can also Google for YouTube videos that show how to do it -

I clicked on an upgrade box on my ipad and when the upgrade was done my ipad wanted my passcode to open it back up. I have not had to use a passcode to open up my ipad since I set it up and cannot remember the passcode. Every time I try a passcode it disables for longer and longer. It is now disabling for an hour at a time.
FORCE IPAD INTO RECOVERY MODE
1. Turn off iPad
2. Turn on computer and launch iTunes (make sure you have the latest version of iTune)
3. Plug USB cable into computer's USB port
4. Hold Home button down and plug the other end of cable into docking port.
DO NOT RELEASE BUTTON until you see picture of iTunes and plug
5. Release Home button.
ON COMPUTER
6. iTunes has detected iPad in recovery mode. You must restore this iPad before it can be used with iTunes.
7. Select "Restore iPad"...
Note:
1. Data will be lost if you do not have backup
2. You must follow step 1 to step 4 VERY CLOSELY. -
My ipad is locked and I cannot remember my passcode. How do I restore/reset my device?
If you've forgotten the passcode then you will need to connect the iPad to the computer that you normally sync to and you should then be able to reset the iPad and re-sync your content to it (http://support.apple.com/kb/HT1212) - you may need to put the iPad into recovery mode : http://support.apple.com/kb/ht1808

My iPhone 4 has been disabled because I cannot remember the passcode. I plug my phone into the computer but it does not bring me to a place where I can restore.

My daughter is out of the country until May. She did not take her iPhone 5 with her. Now she needs a phone number that is stored
in the memory and cannot remember her passcode. I have tried all of the possibities that she e-mailed , but none of them work
and now I am locked out for an hour each time I enter an invalid passcode. Help? How can I get around this to access the phone?Follow the instructions at http://support.apple.com/kb/HT1212

My daughter gave me her IPad 2 and cannot remember the passcode she set. The one she cannot remember is the one when you are trying to reset the Ipad as new, so another user can start from scratch. I log onto icloud but it takes me right to the apple store and i do not get the option as described in the support section to erase all content and rest.
Another way would be to try this:
1. Connect iPad to your computer.
2. Select iPad in the iTunes sidebar, then click the Summary tab.
3. Click “Check for Update.” iTunes tells you if there’s a newer version of the iPad software available.
4. Click Restore. Follow the onscreen instructions to complete the restore process. -
I cannot remember my passcode (4 digits). How do I get around this, please? My IPad continues to be disabled.
You will need to restore your iDevice from the same computer that you most recently synced to.
Read this: http://support.apple.com/kb/ht1212
If you can't do the restore then you will need to try recovery mode.

My daughter has an old iphone4 that she can not remember the passcode to get into it and retrieve information for her iphone5...how can I restore the phone or get the password off w/o knowing current passcode?
There is no way to retrieve any data from the phone unless you can remember the passcode. That's the whole point of the passcode, to protect the data. To remove the passcode, you'll have to force the phone into recovery mode, as described here, & restore it:
http://support.apple.com/kb/HT1808
All data on the phone will be erased. -

What can you do if you cannot remember your passcode?
I turned off my iphone yesterday, and this morning it aksed for a passcode but I didnt have before so I dont know what to do?
If you can't remember your 4 digit passcode you will have to restore your iPad while connected to a computer running iTunes. once you complete the restore process you can restore your apps and data from a recent backup.

No, we can't help and there is no bypass other than to restore your device as new, erasing everything on it. Sorry.

My son has locked my phone with a passcode, I have go onto my Itunes and tried to restore it and it wont allow it so what do i do????
Depends on exactly what you mean by "it wont allow it" -- did you read and try the following from http://support.apple.com/kb/ht1212?
If you see one of following alerts, you need to erase the device:
"iTunes could not connect to the [device] because it is locked with a passcode. You must enter your passcode on the [device] before it can be used with iTunes."
"You haven't chosen to have [device] trust this computer"
If you have Find My iPhone enabled, you can use Remote Wipe to erase the contents of your device. If you have been using iCloud to back up, you may be able to restore the most recent backup to reset the passcode after the device has been erased.
Alternatively, place the device in recovery mode and restore it to erase the device:
Disconnect the USB cable from the device, but leave the other end of the cable connected to your computer's USB port.
Turn off the device: Press and hold the Sleep/Wake button for a few seconds until the red slider appears, then slide the slider. Wait for the device to shut down.
While pressing and holding the Home button, reconnect the USB cable to the device. The device should turn on.
Continue holding the Home button until you see the Connect to iTunes screen.
iTunes will alert you that it has detected a device in recovery mode. Click OK, and then restore the device.
