Did you check your security software (firewall)?
A possible cause is security software (firewall) that blocks or restricts Firefox or the plugin-container process without informing you, possibly after detecting changes (update) to the Firefox program.
Remove all rules for Firefox from the permissions list in the firewall and let your firewall ask again for permission to get full unrestricted access to internet for Firefox and the plugin-container process and the updater process.

That's the only model iPhone 5, sold by Apple as officially unlocked, in Europe. Thus, if you order an officially unlocked iPhone 5, that's what you'll get.

Yes. Apple honors the 1-year warranty on Apple products based only on date of purchase.
Id the issue is a manufaturing defect, the warranty covers the replacement.
If you bought AppleCarePlus (not just AppleCare) you only pay $49 for a replacement in case of damage caused.
If you do not have AppleCarePlus you pay upwards of 1/2 the cost of a new unit in case of damage for a replacement.

My iPhone 5 has a badly cracked display. As I'm out of the UK at the moment, I took it for a Genius Bar appointment in the USA. They told me it wasn't possible to get a replacement from them, and I would need to contact Apple in the UK to find out if I could get a replacement shipped to me.
Before I spend money on expensive international phone calls, does anyone know if it's possible to do this? Are there any other ways I can get my screen fixed or phone replaced while I'm out of the UK?

Apple has bee pretty emphatic about the warranty/country restriction. Adding to that dilemma is a broken screen...Apple does not fix that but replaces the whole iPhone as an out-of-warranty replacement. For an iPhone 5, that cost is $229 in the US...not sure about the cost in the UK.
Can the UK store ship a replacement? That would have to be discussed with the UK store, perhaps from a US store if you could get a "Genius" to be on the line and confirm the status, etc. not sure if that would work but might be worth a try if you are desperate to get the iPhone replaced while here. Consider shipping time/customs, and so on.
Just some thoughts on this problem.

Wow that is so unlucky. If I was you I would try talking to someone at the Apple Store, explain your situation and don't be shy to shed a tear or too. Its just a shame 3 weeks after your phone getting hardware problems. I know people will say, oh buy Apple Care but I feel ya and I know adding that extra money in the beginning feels pointless and expensive.
Worst case scenarion like I said you will have to pay Apple 269 for them to swap phones. Or if you have any nerd friends have them fix it for you. Its a tedious repair the way the power button has some weird hinge that needs to snap on and ribbons are so thin. I do not advise for you to attempt to fix your self unless you don't care if you break it completely.
Alternatively you could just turn on Assisitive touch on accessibility and use that. That way you wont need to pay anything to fix your phone and wont need to take it apart either.

Within the warranty period, unless the failure was caused by accidental damage it will be replaced for free.

As far as I know, all the iPhones bought online or in the Apple Store ar unlocked.
Only carriers (e.g. T-mobile, Vodafone, O2) are able to lock a phone, Apple won't do that (afaik).
This would mean that you shouldn't need to unlock your iPhone.
