IChat really is unreliable - any tweaks I can do?

I have perpetual problems using iChat:
* Oftentimes people cannot hear me when we start an iChat and I have to quit iChat and come in a couple of times before they can.
* Video is often broken and pixelated, or frozen, or excessively blurry.
* Sound often deteriorates over time and I have to quit and come in again.
These kind of audio problems never happen when I use Skype.
And it doesn't matter whether I am using my iMac G5 / iSight or my PB17.
My broadband is 1 gigabit/sec (up and down) fiber optic.
Everybody in my company (we have people all over the place - St. Louis, San Francisco, Tokyo) experiences the same problem. It's become a joke in my company to try to communicate using iChat. We almost always say, "Let's give up and talk on Skype instead."
Is iChat AV just excessively buggy? Is there anything we can tweak to get it to work reliably?
It looks like it might be fun and useful to use, but none of us have ever had any consistent luck with it.
