Milliseconds From 1st of Month to Now

Is there an easy way to get the number of milliseconds from the first day of the month to the current time?

Can't seem to get it to work. I am SQL Handicapped.
Tried inserting that portion into my statement
t (below) but get errors. I want it to come back as
"iMTD".
