NVRM: RmInitAdapter failed! (hardware problem?) [SOLVED]

Fairly recently, an older i686 box I have cannot load X but displays the bootup info, and non-graphical login prompt just fine.  The video card is an nVidia 8400GS.  After googling around for several of the dmesg errors, and not finding much of relevance, I am starting to wonder if the card itself is bad.  Anyone with experience is welcomed to chime in with suggestions.
I have tried:
* Moving the card to another PCI slot.
* Both the linux/nvidia and linux-lts/nvidia-lts packages.
* Chapter 8 of the nvidia README recommends several kernel bootline mods, none of which work.

After exhausting various options in the BIOS (acting under the belief that something was interfering with the IRQ of the card), I thought I would try installing the nvidia-304-xx driver which ended up solving the problem.  As of 06-Oct-2013, the Nvidia web site advises to use the 325.xx or 319.xx driver which is flat out wrong. 
In short, using the legacy 304xx driver seems to be required for the 8400GS.

  • Drive fails SMART status-"this drive has a hardware problem"

    I recently did a major update of software and applications to my MacPro. (it's the first model of 8 core that came out). When I do a major update like this, I make a clone using Carbon Copy Cloner of the system drive to an external drive so I have a backup until I verify everything is good on the updated internal. I have 4 hard internal drives and have been having no trouble from them. However I noticed in Disk utility that one of the non-system drives is displayed in red and gives a warning that reads, "This drive has a hardware problem and can't be repaired. Backup the data and replace the drive". It also shows the SMART status as "failing". So I have everything copied to another drive.
    I'll gladly replace the drive. However here's my confusion: when booting from the system cloned to the external drive, this same drive shows as being perfectly fine in Disk Utility. Both system drives have OSX 10.6.6. the only difference is newer applications on the internal system drive that reports failure (mostly audio related applications: Pro Tools, Reason, Roxio Toast).
    So does anyone have any thoughts? I've read some other threads and they just report the failure and say to change the drive. I've not seen anyone else have one system that reports failure and another that doesn't.

    SMART status failed is usually caused by too many suspected Bad Blocks. Some of these blocks may be unjustly accused of being permanently bad.
    If you have the liberty to re-initialize the drive, doing so with security options "Zero all data, one pass" will ensure that (if the test completes without error) the drive once more has 100 percent good blocks. The process takes several hours to complete, longer if there are a lot or bona-fide errors.
    It is also possible that the drive will produce "initialization Failed", in which case it should be retired if out of warranty, returned for replacement if still in warranty.
    A drive that has ever seen a SMART status of Failed may get more Bad Blocks in the next six months. Or maybe this afternoon. If possible, it would be good to reduce your day-to-day use on this drive, such as making it a backup drive rather than using it all day every day for 'live" data.

    Hello all! I'm using a late 2009 27" iMac (2.8 GHz, 12 GB Ram).  I've been noticing a lot of insanely slow speed lately, mostly in opening new folders or restarting. I ran the Disk Utility and it maked my internal 1TB drive as "This drive has a hardware problem that can't be repaired". Thankfully I have a 2tb backup external that has all my data on it, but now it's time to decide what to do.
    1. Try and reformat my hard drive and reinstall the backup
    2. Replace the drive with a new internal drive (and look for the best solution... thinking either a hybrid drive, SSD+internal or a 7200 RPM drive)
    3. Throw my iMac into the sea.
    4. Cry
    So basically my question is:
    Is my drive dead and should i even try to help it? (I don't qualitfy for the TB repair from Apple and my Applecare is up)
    What are the best performance upgrades for an internal. 7200 internal? 7200 Hybrid? both an SSD and an internal drive?

    You will have to see what the SMART report is for the drive in Disk Utility. If it says "Verified" then the drive should be OK. All you need do is repartition and reformat the drive. On the other hand if DU reports the drive has an I/O problem then the drive is probably bad.
    Initially why not just try this:
    Drive Preparation
    1. Boot from your Snow Leopard Installer Disc. After the installer loads select your language and click on the Continue button.  When the menu bar appears select Disk Utility from the Utilities menu.
    2. After DU loads select your hard drive (this is the entry with the mfgr.'s ID and size) from the left side list. Note the SMART status of the drive in DU's status area.  If it does not say "Verified" then the drive is failing or has failed and will need replacing.  SMART info will not be reported  on external drives. Otherwise, click on the Partition tab in the DU main window.
    3. Under the Volume Scheme heading set the number of partitions from the drop down menu to one. Click on the Options button, set the partition scheme to GUID then click on the OK button. Set the format type to Mac OS Extended (Journaled.) Click on the Partition button and wait until the process has completed.
    4. Select the volume you just created (this is the sub-entry under the drive entry) from the left side list. Click on the Erase tab in the DU main window.
    5. Set the format type to Mac OS Extended (Journaled.) Click on the Security button, check the button for Zero Data and click on OK to return to the Erase window.
    6. Click on the Erase button. The format process can take up to several hours depending upon the drive size.
    7. Quit DU when formatting has completed. Install Snow Leopard.

  • Yoga 2 Pro Webcam suddenly stopped working....(not a hardware problem!)

    Hi everyone,
    I've had the yoga 2 pro for about a year now - took a little bit of fiddling to get it working just right (clean OEM win 8.1 install and installing drivers in the right order etc) but I had it working just perfect then....
    About a week ago the webcam stopped working during a skype call. I tried reinstalling the drivers etc and messed around for 2 days trying to get it working again. The LED light comes on when it's engaged and it says it's present and working in device manager but I get no picture on skype or any other program.
    I reinstalled windows 8.1 from scratch in the hope that this would solve it but same issue.
    So I assumed it was a hardware fault and thought stuff it!
    But then last night I installed Windows 7 32 bit on VMWare to run some of the older 32 bit programs and the webcam works perfectly within this virtual OS!! So it doesn't seem to be a hardware problem after all.
    I've tried everything - installing the bison & Liteon drivers from the Lenovo website, installing the generic win 8 drivers etc but NOTHING WORKS!!
    Can  you help? I just want to get this thing working again!!

    Unfortunately I've reinstalled the driver after uninstalling it from the device manager 10 times. I also formatted and reinstalled, but that didn't fix anything.
    The camera works for the camera app + the webcam test online. I've tried rollback on the drivers, and installing earlier versions of skype with no luck.
    Very strange, all it does is show the loading sign, and never loads. 
    This is the desktop version of skype btw. I'm going to try it with the non-dekstop version tonight and see if it works. 
    EDIT: Is there any other camera source besides 'Lenovo EasyCamera'? Their are two drivers on the drivers page, which one is recommended?

  • I downloaded OS Mountain Lion 10.8... and when I was getting ready to install I get "This disk has S.M.A.R.T. errors.  This disk has a hardware problem that can't be repaired.  I have read some of the same comments below.  Why?

    I was trying to install OS X Mountain Lion 10.8... and I got an error saying, "This disk has S.M.A.R.T. errors.  This disk has a hardware problem that can't be repaired.  I saw several other questions below of people having the same or similar problems.  Does this mean that my hard drive is about to crash?  I plan to head to the place I purchased and have it checked out.  I backed up my data a couple of days ago.  Any comments?

    Yes, S.M.A.R.T. is a built-in diagnostic on the hard drive, and it is reporting a problem that may cause it to fail.
    You can see the same status in Disk Utility, and possibly get more info.  Run Disk Utility and select the DRIVE in the sidebar.  Be sure to select the DRIVE and not the volume indented below it.  At the bottom of the window, there is a field for S.M.A.R.T. Status.  If there is no problem, it should say Verified.
    You can also do a Get Info on the drive (File -> Get Info) in Disk Utility, and the Info window provides more detailed information (toward the bottom) about what is being checked.

  • IMac refuses to boot properly - hardware problem?

    Hi all, I'm having some problems with my iMac (purchased last October) and was hoping some kind soul could shed some light.
    Here's where I'm at now.
    - The iMac doesn't boot. When I press the power button, it chimes, but then nothing happens and I just see the white/grey screen.
    - I have disconnected all 3rd party peripherals, and only have original keyboard and mouse attached.
    - No matter what keyboard combo I try, nothing happens either. I managed to eject the disc once by pressing the mouse button, but no other key combos work.
    - I must have rebooted about 30 times now, and only twice did something happen:
    1. Once it sat on the white screen for a bit, and then displayed a grey folder with a questionmark in it, but then nothing else happened.
    1. Once I did get it to boot from the CD, and I got to the installation screen. At this stage I tried a couple of different things. First I tried Disk Utility, and the HD is indeed listed in the sidebar (although it's listed as 'disk0s2' rather than 'Macintosh HD'. When I picked the First Aid tab, both Repaid Disk Permissions and Verify Disk Permissions are greyed out (i.e. I can't press them). When I went to press Repair, I got the following error message.

    +Checking HFS Plus Volume+
    +Invalid node structure+
    +Invalid record count+
    +Catalog file entry not found for extent+
    +Volume check failed+

    So, I could do nothing in Disk Utility. I then thought "well, I'll just surrender then and reinstall OS X", but when I went back to the Install screen and proceeded, and it asked me where to install the OS, there was nothing to choose from. Usually 'Macintosh HD' is listed along with any other disks. Not this time. So, I shut down the computer again, and now I can't back to the screen (i.e. it refuses to boot from the CD again and instead just spits out the disc).
    So then I phoned Apple. But because I don't have their service plan (and the free one only lasts 90 days after purchase), he couldn't help much. He did say though that if it spits out the disc, it could be hardware problem, and my warranty would cover it.
    So, should I just give up, realise my HD is screwed and my data is gone, and hand it in for service? Or can anyone here shed some more light on my situation?
    Even a "yes, you're screwed, hand it in" would suffice, if that's indeed where I am.

    And I have been backing up, but I just got back from vacation, so my last backup is 2 weeks old and the things I was worried about saving was the work I did on my vacation photos and video. Also, you'd be kicking a girl when she's down, which would be even worse.
    I think I'll definitely hand it in now, and ask the tech to rescue whatever data he can. I don't care if it costs me a bit, if it means rescuing my vacation photos.

  • DEVICE ERROR 365- HARDWARE PROBLEM !!!!!

    Hi all, thanks for you help.
    1. I plugged my charger into my Blackberry 8900, return to it and the phone turns off. 
    2. I reset it- pulling the battery out, putting it back in. 
    3. It then says: DEVICE ERROR 365: RESET 
    4. I've tryed to reload the OS on my blackberry but my mac and bb desktop software says :Cannot connect to bb. So I cant update it..NOTHING  !!!
    5. I called the carrier of the phone - ORANGE. They say that device 365 is  a hardware problem- need to buy a NEW blackberry -_________-. 
    Would I be able to send the phone to Blackberry for repair or to have it replaced ?. 
    PLEASE HELP....If there is a way to fix the hardware please help me.

  • Possible NIC hardware problem?

    Hi,
    I have two sun servers, both solaris 9, that act as firewalls.
    I have installed only OS Core. The firewall software is Checkpoint.
    These two servers are in cluster (checkpoint xl).
    I have many other sun servers configured in this way that work perfectly.
    But with these two servers I have a lot of network problems.
    For instance, if I issue a nestat -rn I see a lot of entries (about one hundred!!). Do you know what this happens?
    My security admins think that there could be a nic hardware problem.
    How can I be sure that the network cards are ok?
    If I run, on both servers, from ok prompt:
    test net
    I get
    rejecting alloc-mem?
    What does this mean?
    The servers are:
    - SUNFIRE V120
    - SUNFIRE V240

    I would just like to make the point that even though you may have great stability with a number of systems without applying firmware and OS patches, it does not mean that the system you are currently experiencing these issues with will not benefit from firmware upgrade. Some of the systems may have different component revisions that require firmware upgrade. Some of the features that are included in firmware upgrades can improve asr ( automatic system recovery ), hardware diagnosability, hardware integrity and generally improves chances of discovering and isolating faults. So it would be a good idea to implement a plan to upgrade all your systems with the patest firmware.
    I would put this system on alert and make a service call with Sun, asr ( automatic system recovery ) can detect early signs that a component is failing and just clearing the disk from asr should not be the only step taken here, the disk may need to be replaced.
    I am currently looking through the internal hand book for some references for you, there should be some good spectrum stuff that I can point you to, I'll update shortly.
    One more thing, when you were configuring Solaris for FW-1, there are a number fo kernel parameters to change, misconfiguration of any of these parameters could be a cause for FW-1 to report errors.
    On the output from netstat -rn, I wouldn't worry about that too much, if the system is a gateway for a large number of hosts it will probably have an enrty for all networks being visted by hosts on the client side.

  • Stuck with grub problem (SOLVED)

    Today i tried to upgrade my Arch Linux, it downloaded everything that should be downloaded, it was installing upgrades but at the installing grub it stucks as like this
    "Copying /boot/grub/grub.cfg.pacsave to /boot/grub/grub.cfg"
    I've waited it for about 15 minutes, it didn't move on. So i did ctrl+z to break. Then i tried to update again. But it gives an error which is:
    error: failed to commit transaction (conflicting files)
    python: /usr/bin/2to3 exists in filesystem
    Error occured, no packages were upgraded.
    Please help me guys!

  • Am I able to find my iTunes serial number? I need it for the problem solving as my problem is solely bad on my iTunes.

    iTunes has no serial number. If you are trying to get to Apple phone support, that is linked to the purchase of Apple hardware, so you need to enter in the serial number of an Apple device. Otherwise, for assistance with problems with iTunes itself you would need to call Apple Support and they would almost certainly charge for the call. I would instead suggest you post the issue you're trying to address here in detail. Someone here can probably offer suggestions.

  • PROBLEM SOLVED--CS5 WIN7 X64 INSTALL ERROR DEPENDENCY FAILURE

    PROBLEM SOLVED!!!!!!!!!!!!
    I just spent 9  hours straight on this, but will was stronger than "its". Now I have  both cs4 and cs5, but hopefully it will work if you just have cs5 as  well. I also did not try runnning cs5 installer first after implementing  the fix, maybe that will work too..And when getting the cs5 error, I  went to re-install cs4 but got an error as well so that is why I took  the steps I did. On 7x64 do the followin
    First  run the cs5 cleanup tool and reboot.
    Second,  go into c:\program file(x86)\common files\adobe\installers . Delete  everything in it
    Then go into you registry  (start, run, then type regedit) and expand to this registry key:  HKEY_LOCAL_MACHINE\SOFTWARE\Microsoft\Windows\CurrentVersion\Installer\UserData\S-1-5-18\ Components  ...(if this isnt there, try an install again, then go to the folder  that you deleted the files from a moment ago and extract the log from  the zip file and open it and go to the end. where it says error 1402  will be followed by a unknown\components\(a bunch of numebers right  here)\(more numbers)  ...
    Copy the numbers  directly after the components\ and go back into registry and click edit  then "find" paste those numbers into the box and hit find..when you get  to the key, scroll ALL the way back up to components again. Right click  components and hit permissions.. Go to advanced and then the ownership  tab up top.. click edit, hightlight administrator(s), check the box to  "replace subcontainers" and click apply. respond yes to everything and  close that and hit apply again if asked. then click the add button above  the advanced button you just clicked. when the new box appears, click  advanced and then click find now. Highlight Administrators and SYSTEM  and hit OK. Then give both of them full rights and hit apply. Then click  advanced again and in the first window check the box that says "replace  all child objects...." and hit apply. accept the prompts and hit okay.
    Now  go into c:\windows and on the system32 folder, right click and go to  properties. Go to the security tab. Click advanced and then "owner" and  click edit. highlght administrators and check the box "replace all child  object....." again and hit apply.... 
    Now give  Administrators, SYSTEM, AND TrustedInstaller full rights by clicking  edit above the advanced tab in the first properties box.. Then click  advanced again and in the first window click replace all child  object....." again and hit apply.... 
    Now  do the same for the winsxs folder and (for good measure i did syswow64  but not sure if its necessary)... 
    Now you should  be able to install both versions..(as i said, I installed cs4 first,  maybe it was not necessary)... 
    If you dont  like the idea of just blindly giving administrators full rights to all  system folders, you can spend MANY MANY hours running the installs over  and over and getting the faulty registry value that caused it to fail  and create specific permissions for that and then go on to the next  value that fails......what a pain

    I have been trying to install CS5 MC now for weeks. I first got it to install everything except After Effects and Premiere Pro. Then trying to follow all the suggestions on this forum I wound up not being able to install any part of it at all. Now..
    Well...following your kind suggestions as much to the letter as I could I have now been able to install everything...except Affter Effects and Premiere Pro. Sigh.
    At least I can get back to work on everything that doesn't require AE and PPro.  I still need to get them installed and working.

  • New Drive has a Hardware Problem?

    Hi,
    The old hard drive in my iMac finally failed (it was a Seagate with a known issue, but I never got it replaced). Today I put in a brand new drive, exactly same model as the old, but I still get the S.M.A.R.T. error "This Drive has a Hardware Problem that can't be Repaired".
    Any clue what could possibly be wrong? I may have misplaced the EMI foam slightly (after some time, the fan spins up), but could that really be the cause? There were no fan problem with the old drive installed.

    I don't understand the SMART report, but unless you got a replacment directly from Apple, it wouldn't have been an exact replacment. That drive, even if it's the same model, has to have a special firmware for the internal temperature sensor and that's only available from Apple. If you've read the OWC blog for the late 2009s that says that you ony need to replace the drive with one from the same manufacturer in order to use the same sensor cable, I believe that's not correct.
    The spinning up can be cured by using the free SSD FAN Control. But no idea if that's related to the SMART error. I only know about the temp sensor issue related to this. If this doesn't fix it, maybe you got a bad drive or the problem is located elsewhere, maybe on the SATA controller on the logic board. No idea.

  • Looks serious. possible hardware problem. unable to install any OS.

    Hi,
    Powerbook was working absolutely fine. Stupidly, I attempted to watch a partially (97%) downloaded (with Bittorrent) movie (.wmv -- WM3) in Windows Media Player 9 in full screen mode. Half way through the movie freezes. I try and get out of full screen, nothing changes -- the whole machine is frozen. I restart. During boot there's a serious error -- kernel panic I think -- telling you to restart machine in various languages including Japanese. Restart. Freezes during start up window progress bar. Restart. Freeze.
    I start up from the Jaguar disk 1 start up disk, go into Disk Utilties, repair disk -- errors found, and fixed apparently. Restart. Freeze.
    I start up from DiskWarrior 4 start up disk and start it on the drive. Step 6: "Overlapped files detected" with a number after, and it starts taking ages. I left DW working for nearly two days until it seemed to have frozen (still on step 6).
    I connect the sick PB to an iMac via firewire and start up the PB in disk target mode, the PB's drive appears on the desktop of the iMac and I'm able to easily with no problems copy off all my personal files. At that point I thought all was saved -- I was out of hot water, no further real problems. All I had to do is wipe the drive and reinstall etc.
    I reformatted the drive from the iMac choosing to zero aswell. The PB's drive appears on the desktop empty and looking fine.
    Start up the PB using Panther retail install disk one click install, it goes through checking the install disk then starts to check the target disk or actually install on the target disk -- at that point kernel panic -- 'restart machine' in various languages including Jap. Start up again using Panther disk, this time choose Disk Utilities to attempt to refromat the drive again, kernel panic before Disk Utilties even gets fully open.
    I start up the PB using Jaguar retail install disk one, choose Disk Utilties -- fine. I reformat the drive, this time not bothering with zeroing. Fine. Go through to install Jaguar. Again it checks the install disk fine, then immediately after, kernel panic.
    I tried partitioning the drive into two using Disk Utilties on Jaguar start up disk. My thinking was, maybe the start of the drive is damaged. Then tried to install onto partition two: panic. Just to check, tried to install to partition one: panic.
    It's a 60GB drive. I installed Jaguar onto it ages ago successfully. The only difference was when doing it ages ago is I used the PB's internal CD/DVD drive. That had since broken and I've been using an external firewire CD/DVD drive -- all the above various Jaguar / Panther attempts were from the external drive. I just received a 2nd hand internal CD/DVD drive I bought on eBay -- I was blindly/stupidly hoping that using an internal CD drive would solve my problem but it hasn't.
    What's the problem? It seems as if the initial Windows Movie Player crash has caused this, but that can't be can it? That was a software crash and I've since reformated the drive. There shouldn't be any remnants of that in anyway. So it's a hardware problem? But it was working fine before the crash and nothing has happened to the PB between the crash and now. But there's no two ways about it -- right now the PB seems terminally broken.
    Any suggestions how to get my PB working again?

    It's intalled. So it turns out I was lying when I said "The only difference was when doing it ages ago is I used the PB's internal CD/DVD drive." -- I forgot to mention there was no AirPort card in the PB when I installed Jag ages ago.
    Also a small note: it seems an internal CD drive was necessary in the end. When the Panther install restarted and requested disk 2 I couldn't get it to recognize disk 2 from the external drive -- had to use the internal one to continue (although I didn't spend too much time/effort trying to get the external one to work but on the face of it it didn't seem to). (Although I can imagine there may be a way of installing stuff from disk 2 after having customized the install to not require anything from disk 2 for the first main automated install possibly? I may sell the internal CD/DVD drive if so as otherwise it was a waste of money).
    Having installed Panther I put the AirPort card and the 2nd memory back in and started up -- all's apparently OK, but of course there's good reason not to be convinced of that. I didn't customize the Panther install as I wanted (because I didn't expect it to work). Not sure if I dare try wiping and reinstalling.
    Obviously I'd feel happier knowing what the problem was/is. Looks like the AirPort card had something to do with it. But even when that was out the PB couldn't start from OS9. When formatting the drive (from OS X installer software) I'm sure I had the OS9 check box ticked. Even if I hadn't it wouldn't stop the OS9 CD working would it?; the hard drive just wouldn't appear once started up from the OS9 CD.
    So I now have Panther up and running with both bits of memory in and the AirPort card in. I haven't messed around with/in Panther at all yet but on the face of it it seems fine.
    Any ideas what the problem was/is? Any suggestions how to find out? Should I just ignore it and forget it ever happened (and cross my fingers and hope I never need to mess around with OS installs again)? Would be nice to know what's going on though. For a start it looks like if ever I want to install OS9 on it I won't be able to -- which actually may be something I want to do in the future possibly.

  • Potential keyboard hardware problem T410

    Dear all,
    since yesterday I have been experiencing a problem with my T410, which is potentially due to
    a hardware issue with the keyboard.
    A few specific keys are not working anymore. These seem to be the Power On key, the Fn key,
    the Alt Gr key, and the ThinkVantage key.
    I am assuming it is a keyboard hardware problem, since I cannot use the Power On button to
    start the laptop. Fortunately, this is possible using the fingerprint reader start-up.
    When the laptop is running (using Windows 7) everything is working fine, except the mentioned
    buttons. I cannot access any Fn button functions (e.g. the reading lamp) or Alt Gr symbols (e.g.
    the @ symbol which is on my keyboard on the 2 button) or power down with the Power button.
    I tried various easy fixes:
    * Making sure all drivers are up to date
    * Remove and reattach the keyboard
    * Static discharge procedure (unplug all power, 10 presses and 30 sec holding of power button)
    * Checking Bios settings
    * Replace battery
    Unfortunately, these did not change the situation.
    I am wondering if the simultaneous malfunctioning of these specific buttons points to a certain
    hardware problem. I hope a replacement of the keyboard might solve this, but it could be an
    issue with the motherboard.
    Any hints would be welcome.

    Dear all,
    thank you for your suggestions. Unfortunately, it was not the keyboard. It appears to
    be a more serious problem. In fact, since yesterday the laptop does not boot correctly
    anymore. The boot process is started, but I only end up with a black screen, and some
    randomly colored first few lines at the top of the display.
    It all points to a major malfunction. Unfortunately, the warranty expired 3 months ago...
