Yes, the store didn't properly activate the card. If you don't have a rescue email address (you won't be able to add one until you can answer your questions) then you will need to contact Support in your country to get the questions reset (these are user-to-user forums).

My daughter's itouch (registered to my apple account) is asking for answers to security questions that I never answered and it won't let her buy anything from the iTunes store. I tried a hard reset to original settings but it still asks for answers. How do I reset the security questions or get rid of them appearing altogether? I can purchase apps on my phone (same apple account) and it doesn't ask me for security questions.

Okay so I have had apple for a while and I got a new apple ID not to long ago and then answered the security questions and it said that it was the first purchase with the apple ID and I have to answer them and now I don't remember the answers

If you can't remember them then go to Express Lane and select 'iTunes' from the list of 'products' in the middle of the screen.
Then select 'iTunes Store', and on the next screen select 'Account Management'
Next choose 'iTunes Store Account Security' and fill in that you'd like your security questions/answers reset.
You should get an email reply within about 24 hours (and check your Spam folder as well as your Inbox) - but some recent posts are saying that Apple have temporarily suspended the reseting of security question/answers whilst account security processes are improved, so it may take longer. -
So i sent a $25 itunes gift card to my friend through his email from my iphone and when he tried to purchase an app on both his phone and ipad 2 it is telling him to put in a sercurity code and answer his security questions when all he's trying to do is redeem the money to purchase the app!! Make sure you're using the correct code. For most iTunes cards, the code is under the scratchoff area on the back of the card. The support article from which you came:

my $25 itunes card is new not scratched yet and when i use it on my computer it keeps saying This code you entered is not recongnised as a valid code
very frustraded >=|In that case, you're going to have to put the phone into recovery mode and restore it as a new device. The only other option is to connect it to the PC it normally syncs with and restore it, but it sounds like that's not a possibility.
